[Ischemic stroke and the ways of compensation of cerebral circulation in patients with nonspecific aorto-arteritis with lesions of the brachiocephalic arteries].

In patients suffering from nonspecific aortoarteritis associated with impairment of the common carotid arteries, cerebral blood flow is compensated for at the expense of an increase of the volumetric blood flow in the vertebral arteries. During the first three years since the onset of the first disease symptoms, the blood content in the basin of the internal carotid arteries is reduced. Brain strokes occur most frequently within that period. They mostly develop without any preceding transitory ischemic attacks. The origin of neurological symptomatology depends to a considerable measure on the condition of the anterior and posterior communicating arteries.

[Dynamics of neurological manifestations of occlusive diseases of the branches of the aortic arch in nonspecific aortoarteritis].

A retrospective analysis was made of complaints and focal neurological symptoms in patients with nonspecific aortoarteritis of brachiocephalic vessels. It is demonstrated that the latter ones appear at the early stages of the development of stenosis of the main vessels. This allows early elaboration of indications for angiologic examination of patients. It has been noted that surgical correction of the blood flow in aortic arch branches in the early phases of the disease materially improves prognosis.

[Surgical tactics in combined lesions of the aortic arch and its thoraco-abdominal segments in nonspecific aortoarteritis].

Seventy-one patients with unspecific aortoarteritis and affections of the aortic arch and its thoracoabdominal segment were treated in the period between 1984 and 1989. All the examined patients had different variants of affection of the brachiocephalic arteries, in 57 (80.3%) of them there was a coexistent affection of the thoracoabdominal segment of the aorta, and in 14 (18.7%) coexistent affection of the abdominal aorta and the lower limb arteries. Operations were conducted on 56 (78.8%) patients. Reconstruction of the brachiocephalic arteries was carried out in 12 (21.4%), of the thoracoabdominal segment of the aorta and the renal arteries in 30 (53.6%), and of the abdominal aorta and lower limb arteries in 9 (16.1%) cases. Five (8.9%) patients underwent two-stage correction of the blood flow in the branches of the arch of the aorta and its thoracoabdominal segment. The clinical picture, the volume of the affection, the methods of examination, and techniques and the results of reconstructive operations are described in detail. The authors present their point of view on the performance of surgical treatment in stages in coexistent lesions differing in volume.

[Characteristics of vascular changes and hemostatic reaction of the body in nonspecific aorto-arteritis].

Laboratory criteria to differentiate diagnostically nonspecific aorto-arteritis with other forms of obliterating arterial diseases have been established by hemostatic, morphological and cytological investigations. Factors of plasmic hemostasis and platelet functional activity are able to indicate etiological cause of aortal lesion in noninvasive clinical examination. Identification of platelet--forming reaction type shows that nonspecific aorto--arteritis is characterized by "red" platelet formation and fibrinization of the body. Atherosclerotic stenosing proceeds by "white" thrombus formation associated with enhanced activity of platelet adhesion and aggregation.

[Surgical tactics in nonspecific aortoarteritis of the thoracoabdominal region].

Sixty-six patients with nonspecific aortoarteritis of thoracoabdominal localization were treated at the department of vascular surgery of the Vishnevsky Institute of Surgery, AMS USSR from 1984 to 1989. Operations were performed on 49 (74.9%) patients. Percutaneous angiodilatation of the renal arteries was conducted in 3 (4.5%) patients. Nonoperative treatment was carried out in 14 (21.2%) patients. In view of concurrent affection of the branches of the arch of the aorta, reconstructive operations were performed in 16 (24.2%) cases on the brachiocephalic trunks: aorto-biocarotid prosthetics (6), aortocarotid prosthetics (1), subclavian-carotid autovenous prosthetics (6), carotid-subclavian autovenous shunt (2), carotid endarterectomy (1). Reconstructive operations on the brachiocephalic trunks were preceded by reconstruction of the arteries in 5 (7.6%) cases and were conducted after correction of the blood flow in the thoracoabdominal segment in 2 (3%) cases. In 9 (13.6%) patients only the brachiocephalic trunks were reconstructed. A total of 42 reconstructive interventions on the thoracoabdominal segment were carried out on 40 (60.6%) patients, in 35 (87.5%) correction of the blood flow in the visceral branches and renal arteries was performed simultaneously. The surgical tactics suggested by the authors and the results produced by it are discussed in detail.

[Indications for and the methods of revascularization of visceral branches in aorto-femoral reconstruction].

The work presents data of 774 patients with occlusing injuries of the abdominal aorta and arteries of lower extremities subjected to aorto-femoral reconstructions. In 546 of these patients (70.5%) there were different variants of injuries of visceral branches. A detailed analysis of the clinical picture of injuries of the visceral branches is presented related with the injury volume, etiology of the occlusing process and coexistent arterial hypertension. The authors have an experience with 169 observations of revascularizations of visceral branches during aorto-femoral reconstructions. Reconstruction of the inferior mesenterial artery was made in 134 cases, that of the celiac trunk of the superior mesenterial artery-in 35 cases. A detailed description of indications to correction of the visceral branches blood flow in aorto-femoral reconstructions is presented as well as the methods of revascularization of the visceral arteries.

[The initial experience of using pulse therapy with cyclophosphane and 6-methylprednisolone in patients with acute and subacute stages of nonspecific aortoarteritis].

The authors summarize the first experience gained with the use of pulse therapy with cyclophosphamide and 6-methylprednisolone in 18 patients with acute and subacute nonspecific aortoarteritis. Remission attained in 16 patients was attended by a decrease in objective criteria of the process acuity. It has been noted that duration of the remission was inversely proportional to the time of the inflammatory anamnesis. Provided the patients manifested exacerbation of the process, repeated courses of pulse therapy were conducted.

Acute Disease↗

[Clinical aspects of the progression of nonspecific brachiocephalic aortoarteritis].

Based on the features of the clinical course of nonspecific aorto-arteritis, three types of the course were identified: slowly progressive, rapidly progressive, and recurrent. The progression of the disease and involvement of new arterial beds into the pathological process were found to primarily depend on persistent inflammation. It is suggested that the types of the clinical disease course are directly related to immunological responsiveness.

Morphogenesis of intimal thickening in nonspecific aortoarteritis.

Hyperplastic intima in nonspecific aortoarteritis was investigated immunomorphologically and by means of electronmicroscopic autoradiography. The cellular composition of the involved vascular wall was studied by the method of alcohol-alkaline dissociation. The prevalence of elongated cells with side processes and stellate cells in thickened intima was shown. These cells are characterized by essentially intensive RNA synthesis and, according to the electron microscopic data, can be regarded as pericytes. A significant amount of capillaries and precapillaries in thickened intima was visualized. Distribution of laminin and type IV collagen coincided with localization of the basal membrane of the vasa vasorum and with that of microvessels growing in the intima. Considered together, these data suggest that pericytes that surround a capillary/precapillary network may play a significant role in the morphogenesis of vascular transformation in nonspecific aortoarteritis intimal thickening.

[T8-positive lymphocytes in inflammatory infiltrates of the vascular wall in nonspecific aortoarteritis].

Artery specimens obtained during surgical manipulations from 11 patients suffering from nonspecific aortoarteritis (NAA) were studied immunohistochemically for T4, T8-positive lymphocytes, LFA-1-positive lymphocytes, basal membrane (BM) proteins: laminin and type IV collagen. T8-positive and LFA-1-positive cells were shown to be predominant in the sites of injury. The results obtained point out to the presence of cytotoxic cells in the affected arteries. The targets for such cytotoxic cells are medial smooth muscle cells rather then intimal cells, whereas intimal cells become hyperplastic, expressing BM proteins, which proves their smooth muscular origin. The above data support the suggestion on the autoimmune etiology of NAA.

[Various problems in the morphogenesis of nonspecific aortoarteritis].

Hyperplastic aortic intima in non-specific aorta-arteritis (NAA) was studied histologically and by means of electron-microscopic autoradiography and immunomorphology. Main cell elements of focal intimal growth in NAA were shown to be fibroblast-like cells producing type I collagen and the cells ultrastructurally corresponding to smooth-muscle cells and producing type III collagen. There were many small vessels of capillary and precapillary type located between these cells and in the intercellular substance. It is discussed whether the hyperplasia of vascular systems of the vessel wall is a decisive factor in the morphogenesis of local thickening in the intima.
