Cost-effective uroflowmetry in men.

A simple method of timed urine flow measurement performed at home by the patient is compared to instrumental measurements of peak flow rate. The timed method correlates well with the peak flow rate. Timed uroflowmetry is free, can be done in the privacy of the home and provides multiple measurements. This is a valid technique to document a weak stream and is a useful screening test for patients with lower urinary tract obstruction.

Bone marrow acid phosphatase by radioimmunoassay.

A double-antibody radioimmunoassay was developed and utilized to measure prostatic acid phosphatase in bone marrow aspirates. One hundred-eighteen patients with carcinoma of the prostate in various clinical stages, and fifty with benign prostatic hyperplasia were studied. In patients with carcinoma, levels of prostatic acid phosphatase in bone marrow aspirates were found to correlate well with increasing clinical stage of the disease. Determination of bone marrow prostatic acid phosphatase by radioimmunoassay may be a valuable adjunct to clinicopathologic staging of prostatic carcinoma.

Modified radical retropubic prostatectomy.

A significant modification of the traditional radical retropubic prostatectomy is presented. A logical stepwise surgical sequence is discussed in detail. The technique described provides excellent visualization, early ligation of vascular and lymphatic channels, decreased intraoperative blood loss, and a tension-free vesicoureteral anastomosis.

Ataralgesia in outpatient urology. Clinical evaluation.

The combination of fentanyl citrate (Sublimaze) and diazepam (Valium) was evaluated for efficacy of analgesia, sedation, and safety in 1,008 predominately outpatient urologic procedures. These procedures included prostate biopsies, basket extractions of ureteral calculi, internal urethrotomies, and cystourethroscopies. Ninety-two per cent were judged to be successful with regard to adequate tranquilization and relief of pain. No detrimental effects were seen with the recommmended method and dosage. This drug combination provides the clinician with an effective and safe alternative to local, general, or spinal anesthesia for many routine urologic procedures and allows them to becom true office procedures.

Experience in staging testis tumors with bleomycin 57cobalt and present role of 67gallium scan.

A technique was developed using bleomycin and 57cobalt to study nodal metastases in testis tumors. Comparative studies were made on 15 cases with 67gallium, lymphangiography, supraclavicular node biopsy, liver and spleen scans, chest x-ray, excretory urogram, bone survey and pathological study of surgical specimens when possible. The results with the bleomycin-57cobalt complex and 67gallium were discouraging. The bleomycin-57cobalt study was discontinued. Pathological staging is still the most accurate of all modalities available for staging testicular malignancies.

Diagnostic value of 99mTc in scrotal scan.

The radioisotope 99mTc scans reflect the vascular flow to the testis. As a result the scan shows a distinct cold pattern when arterial profusion is compromised and a hot pattern when blood supply to the organ is increased. This is the basis for use of the technetium scrotal scan to differentiate between testicular torsion and other conditions simulating it. It can also be used to follow vascular flow in the testis after orchiopexy. To date there has been no erroneous result in this study. Although this is a new diagnostic technique and experience is still limited, it does show promise as an adjunctive tool when the differential diagnosis of testicular torsion and conditions simulating torsion is not readily apparent. Findings in other lesions of the scrotum are discussed.

The persistent nephrogram: A sentinel sign of contrast reaction.

Two cases are presented in which a serious contrast reaction was accompanied by a dense persistent nephrogram. This radiologic sign results from the patient's systemic hypotension and provides the physician with an opportunity for earlier, more effective therapy.
