Abstract

A significant modification of the traditional radical retropubic prostatectomy is presented. A logical stepwise surgical sequence is discussed in detail. The technique described provides excellent visualization, early ligation of vascular and lymphatic channels, decreased intraoperative blood loss, and a tension-free vesicoureteral anastomosis.

The role of surgery in the management of septic shock--extra-abdominal causes of sepsis.

Septic shock is a severe inflammatory response to one or more pathogenic micro-organisms. When a person's immune response is excessively intense, a cascade of phenomena may be activated that ultimately is harmful. Appropriate management of septic shock may include surgical intervention to remove or neutralize the septic focus in an effort to treat the inflammatory response cascade. This is the first of two articles presenting current information on the role of surgery in the management of a patient with septic shock. This article describes extra-abdominal sources of sepsis.

Compatibility of electrolytically produced sodium hypochlorite solutions on long- term implanted dialysis catheters.

More than 20% of the world's population use a catheter for dialysis, despite guidelines limiting their use. Although the structure and design of the catheters differ by manufacturer, the material used in central venous catheters and peritoneal dialysis catheters are the same across manufacturers. Given the long-term use of these catheters in the dialysis population, the good compatibility of the antiseptics and disinfectants used on the catheters is imperative to prevent failure and cracking of the catheter material. Tensile strengths of commercially available catheters were measured after exposure to commonly used disinfectants. The tensile strength was then compared between the catheters by analyzing the displacement vs. force (N) curves produced during the evaluation. A total of 44 catheter lumens were evaluated. The electrolytically produced sodium hypochlorite solution, Alcavis 50/ExSept Plus, was the only solution shown to be compatible with all three catheter materials resulting in a deviation of less than 10% for each of the different catheter types. Electrolytically produced sodium hypochlorite solutions were the only solutions in this study that did not alter the physical properties of any of the catheters after long-term exposure.

Coating urinary catheters with an avirulent strain of Escherichia coli as a means to establish asymptomatic colonization.

We investigated whether insertion of urinary catheters that had been coated with Escherichia coli HU2117 could establish bladder colonization with this nonvirulent organism. Ten of 12 subjects were successfully colonized for 14 days or more. The rate of symptomatic UTI during colonization was 0.15 per 100 patient-days.
