[Analysis of psychological condition of parents with cleft lip and/or palate children].

OBJECTIVE: To evaluate the psychological situations of cleft lip and/or palate children's parents, to discuss the relationship between psychological situations and cleft types, and to provide clinical treatment to improve the psychological situations of these parents. METHODS: 100 parents whose children were proceeded the primary surgery treatment were selected as the study group, and other 34 parents whose children were normal as the control group. All parents were inquired and tested by the life event scale (LES) and self-rating anxiety scale (SAS). RESULTS: The LES total scores of the cleft lip and/or palate patients' parents were significantly high compared with the normal (P < 0.05). The SAS total scores of the cleft lip and/or palate patients' parents had no significant difference with the control group (P > 0.05). The scores of cleft lip and/or palate patients' parents were the highest and the scores of cleft palate patients' parents were the lowest. CONCLUSION: Most of the cleft lip and/or palate patients' parents were disturbed by a negative psychological situation. Some kind of negative psychological situation may manifest some body symptoms. Meanwhile, the three subgroups' data had the significant difference, which indicated that parents may much more care about the appearance abnormality.

[Use of relative energy in low frequency area to describe the severity of hypernasality in cleft palate patients].

OBJECTIVE: To find an objective index to describe the severity of hypernasality in cleft palate patients through speech spectroanalysis. METHODS: 60 audio records of cleft palate patients were analyzed by spectroanalysis, the relative energy in low frequency area (REL) was calculated in the LPC spectrogram. One experienced speech therapist and two nonprofessionals made perceptual judgments of the same records. Spearman correlation analysis was conducted to test if the 3 groups of data have significant correlations. RESULTS: REL had significant correlation with professional judgments (correlation coefficient was 0.272, P < 0.05) and nonprofessional judgments (correlation coefficient was 0.274, P < 0.05). CONCLUSION: REL can be used as one objective reference in the judgment of the severity of hypernasality.

Measuring the in vivo binding parameters of [18F]-fallypride in monkeys using a PET multiple-injection protocol.

The goal of this work was to quantify the in vivo transport and binding parameters of [F-18]fallypride and the D2/D3 receptor density (B'max) in both the striatal (putamen, caudate, ventral striatum) and extrastriatal regions (thalamus, amygdala, cerebellum, temporal and frontal cortices) of the rhesus monkey brain. Multiple-injection PET experimental protocols with injections of radiolabeled and unlabeled doses of fallypride were used to estimate the K1, k2, kon/VR, koff and B'max kinetic parameters. The experimental design was chosen using the D-optimal criterion to maximize the precision of the estimated binding parameters for the various brain regions. There was a significant range in B'max for the putamen (27 pmol/mL), caudate (23 pmol/mL), ventral striatum (14 pmol/mL), thalamus (1.8 pmol/mL) and amygdala (0.9 pmol/mL). Significant receptor binding was also found in the cortical regions. Knowledge of these in vivo rate constants serves as a necessary step in using [F-18]fallypride PET to measure D2/D3 receptor density and drug occupancy in clinical research applications. We believe the precise parameter estimates derived from these complicated experimental protocols are necessary for proper application of drug occupancy and clinical research studies with [F-18]fallypride, which often rely on the validity of assumptions regarding the model parameters.

[The influence to maxillary growth by exposed bone wound in hard palate of rat].

OBJECTIVE: To confirm if denuded bone on lateral aspects of the hard palate of surgical repairs of the cleft palate is the main reason of causing disturbance of facial growth and to investigate the mechanism. METHODS: 48 SD rats were employed in the study. Among them 40 animals were divided into normal control and experimental group randomly. Normal control was not operated. Hard palate mucoperiosteum on the left side in experimental group were excised. Left hard palate mucoperiosteum in another 8 female three-week-old SD rats were excised with same method and were sacrificed at 2nd, 5th, 8th, and 12th day, respectively. The 8 rats were used for histological research. RESULTS: The width of all left sides hard palates in experimental group was significantly narrower than right side, and demonstrated statistical difference. Chronic osteomyelitis on the denuded hard palate was observed. CONCLUSION: The denuded bone wound might be a principal factor for the following maxillary growth deformity in cleft palate patients who received palatoplasty in childhood. The mechanism of causing the deformity might be wound and scar contraction, decreased blood flow and nutrition plus chronic osteomyelitis.

[Research on relationships of gastric cancer with serum trace elements, Helicobacter pylori and COX-2 in gastric tissue].

The relationships of gastric cancer with serum trace elements, helicobacter pylori (HP) and COX-2 in gastric tissue were investigated. We took 50 blood samples from the gastric cancer patients in Hexi region (the gastric cancer group), and 50 blood samples from healthy volunteers (control group) and detected the level of trace elements, the rate of HP infection, and the expression of COX-2 in gastric tissue. The results showed the levels of Cu/Zn, Fe in the serum of the gastric cancer group were higher than those of the control group respectively(P < 0.05, 0.01), and the levels of Zn, Mn were lower than those of the control group respectively (P < 0.05, 0.01). The data on Zn were submitted to multi-variate non-conditional logistic analysis. The rate of HP infection and the positive expression of COX-2 were 88% and 78% respectively in the gastric cancer group, 42.0% and 0 in the control group (P < 0.01). These findings suggest that the decrease of Zn in serum may be a precancerous factor of gastric cancer development which induces HP infection and the higher expression of COX-2 and hence may lead to the development of gastric cancer. Detecting the trace elements could conduce to the diagnosis of gastric cancer. Regulating the level of trace element in the patient can be an effective chemoprophylaxis for gastric cancer.

[500 cases of external ultrasound-assisted liposuction].

OBJECTIVE: To discuss the therapeutic effects and safety of external ultrasound-assisted liposuction and to find out the rules of circumference variation in different body areas after liposuction. METHODS: External ultrasound liposuction was performed on 500 patients (595 sites). We recorded the volume of anesthetic drugs and the aspirated pure fat as well as the body contour variations of every area at one to three months postoperatively. The decreased dimension at different body sites after liposuction were compared and analyzed. RESULTS: The volumes of the anesthetic liquid and the aspirated pure fat were the most in the waist-abdomen and the thigh. After liposuction, the circumstances in different body areas were all decreased to certain degrees. Along with the severity of obesity, changes in the waist-abdomen became more obvious and the decreases of the umbilical circumference, the minimum and the maximum waist circumferences were prone to be less. But when the, minimum waist circumference was over 111 cm, obvious changes occurred in the umbilical. Meanwhile, the upper-middle part of the thigh and the upper arm showed larger variations than the lower part. The ultrasound-assisted liposuction had less blood loss and pain. It resulted in smooth skin without severe complications. CONCLUSIONS: External ultrasound-assisted liposuction is a safe, effective and easily acceptable operation for body contour remodeling. The body circumference variation may have its intrinsic rule, which, to some extent, is meaningful for conducting clinical inquiry and forecasting the operation results.

Synthesis and characterization of iodine-123 labeled 2beta-carbomethoxy-3beta-(4'-((Z)-2-iodoethenyl)phenyl)nortropane. A ligand for in vivo imaging of serotonin transporters by single-photon-emission tomography.

2beta-Carbomethoxy-3beta-(4'-((Z)-2-iodoethenyl)phenyl)nortropane (ZIENT) (6) and 2beta-carbomethoxy-3beta-(4'-((E)-2-iodoethenyl)phenyl)nortropane (EIENT) (10) were prepared and evaluated in vitro and in vivo for serotonin transporter (SERT) selectivity and specificity. High specific activity [(123)I]ZIENT and [(123)I]EIENT were synthesized in 45% (n = 5) and 42% (n = 4) radiochemical yield (decay-corrected to end of bombardment (EOB)), respectively, by preparation of the precursor carbomethoxy-3beta-(4'-((Z)-2-trimethylstannylethenyl)phenyl)nortropane (7) and 2beta-carbomethoxy-3beta-(4'-((E)-2-tributylstannylethenyl)phenyl)nortropane (9), respectively, followed by treatment with no carrier-added sodium [(123)I]iodide and hydrogen peroxide in ethanolic HCl. Competition binding in cells stably expressing the transfected human SERT, dopamine transporter (DAT), and norepinephrine transporter (NET) using [(3)H]citalopram, [(3)H]WIN 35,428, and [(3)H]nisoxetine, respectively, demonstrated the following order of SERT affinity (K(i) in nM): ZIENT (0.05) > nor-CIT (0.12) >> EIENT (1.15) > fluvoxamine (1.46). The affinity of ZIENT and EIENT for DAT was 69 and 1.6-fold lower, respectively, than for SERT. In vivo biodistribution and blocking studies were performed in male rats and demonstrated that the brain uptake of [(123)I]ZIENT was selective and specific for SERT-rich regions (hypothalamus, striatum, pons, and prefrontal cortex). SPECT brain imaging studies in monkeys demonstrated high [(123)I]ZIENT uptake in the diencephalon, which resulted in diencephalon-to-cerebellum ratios of 2.12 at 190 min. [(123)I]ZIENT uptake in the diencephalon achieved transient equilibrium at 157 min. In a displacement experiment of [(123)I]ZIENT in a cynomolgus monkey, radioactivity was reduced by 39% in the diencephalon at 101 min following injection of citalopram. The high specific activity one-step radiolabeling preparation and high selectivity of [(123)I]ZIENT for SERT support its candidacy as a radioligand for mapping brain SERT sites.

[A preliminary study of the two models treated by presurgical orthopedics compared with early soft palate adhesion method].

OBJECTIVE: To compare results of the growth and development of the upper dental arch and the velopharyngeal closure of the cleft patients treated by two methods. METHODS: The dental cast of patient and X-ray films were measured and the statistical medical records were analyzed. RESULTS: The transverse distance of upper dental arch was found to be wider in group A than in group B. The anterior-posterior distance of the dental arch in bilateral cleft group was shorter in group A than in group B. The difference of the two groups were gradually lessened as age increases. Bony bridge in alveolar gap was 63% and 83.3% in unilateral and bilateral cleft group respectively. 15% of cases in group A and 35.2% in group B needed pharyngeal flap. CONCLUSIONS: The stable upper dental arch in group A can opposes the pressure from the lip muscles, this maintains the width of the arch. But A-P distance of upper dental arch in BCLP in group A should be followed up after the age of 9 years. Pharyngeal flap is needed less in group A than in group B.

[Study on the expression level of Annexin I, cPLA(2) and PCNA in cleft-palate mice induced by dexamethasone].

OBJECTIVE: To test the changing expression level of Annexin I, cPLA(2) and PCNA in the palatine process of cleft-palate mice A/J and C57B/6j induced by dexamethasone. To discuss the developing mechanism of cleft-palate and the corresponding preventive methods. METHOD: s Pregnant mice A/J and C57BL/6j were randomly divided into normal group as blank control, group with deformity induced by dexamethasone, group given VitB(12) as antagonist to deforming factor, group given only VitB(12). The relative quantity of Annexin I, Cpla2 and PCNA were detected by immunoblotting. RESULTS: Among mice A/J, with the development of palatine process, Annexin I's expression level was increased in the normal group, and other groups showed the similar changes. Annexin I's expression level was significantly higher in group DEX and group DEX + VitB(12) than in normal group and VitB(12) group of, while there was no significant difference between normal group and VitB(12) group. But the changes of cPLA(2) and PCNA expression level was in an opposite direction, with development it decreased in the normal mice's palatine process. In mice C57B/6j there was no significant difference between normal group and group DEX on the measured quantity of Annexin I, cPLA(2) and CONCLUSIONS: Annexin I and cPLA(2) introduce glucocorticoid to induce cleft-palate. VitB(12) can not inhibit DEX's enhancing effect on the expression level of Annexin I, but it can antagonize DEX's inhibiting effect on expression level of cPLA(2), which is probably one of the mechanisms why VitB(12) antagonize glucocorticoid's deforming effect.

[Maxillary growth following tissue engineered oral mucosal implantation on mucoperiosteal denudated palate process in young rat].

OBJECTIVE: To observe the effectiveness of prevention and cure for maxillary growth deformity following tissue engineered oral mucosa implantation on mucoperiosteal denuded palate process in young rat. METHODS: Hard palate mucoperiosteum of a SD baby rat were excised and oral keratinocytes were isolated and cultured. Tissue engineered oral mucosa was fabricated with the cultured oral keratinocytes and the membrane made of sodium alginate (SA). 80 female three-week-old SD rats were used as subjects in this study. The animals were divided randomly into a normal control group and 3 experimental groups, each group included 20 rats. Normal control group (NG) were not operated. Hard palate mucoperiosteum on left side in all experimental groups were excised, exposed bone were not treated in denuded group (DG), but repaired with membrane in material group (MG) and repaired with the tissue engineered oral mucosa in mucosal group (MUG). All the animals were sacrificed at 9th week postoperatively (12 weeks old), and the clean widths of right and left hard palatal were measured under a dissection microscope. The difference between palatal widths of two sides and the asymmetry ratio between the different groups were compared and analyzed. RESULTS: No significant difference in asymmetry was discovered between the DG and the MG, but the asymmetry in MUG was less than DG or MG. CONCLUSION: Tissue engineered oral mucosal implantation in palatoplasty is an effective method in preventing and curing secondary maxilla deformity by repairing denuded bone wound.

[Preliminary research for constructing tissue-engineered oral mucosa].

OBJECTIVE: To search for a method of constructing tissue-engineered oral mucosa. METHODS: Hard palate mucoperiosteum were excised and extracted from raw SD milk rat. Tissue-engineered oral mucosa was made with the cultured oral keratinocytes that had been digested by Dispase and cultured in the serum-free keratinocytes medium, the supportive membrane being made from sodium alginate. RESULTS: Rat oral mucosal epithelial cells could be obtained with Dispase digestion. It was found that the best time for oral mucosa membrane to be digested to independent cells by Dispase again is ten minutes after the mucosa membrane has been obtained by Dispase; the best density of rat oral mucosa cells cultured is 1.5 x 10(5)/cm2, and the mucosa cells will be difficult to form the colon if the density of cells is too low. The keratinocytes can be cultured in serum-free keratinocytes medium without fibroblast contamination; the mucosa cells grow well on the sodium alginate membrane. CONCLUSION: Tissue-engineered oral mucosa can be constructed with the cultured oral keratinocytes in the serum-free keratinocytes medium and the self-made sodium alginate membrane.

[Effects of vacuum-assisted closure (VAC) on the expressions of MMP-1, 2, 13 in human granulation wound].

OBJECTIVE: To study the changes of metalloproteinases 1, 2, 13 in granulation wound after the treatment of vacuum-assisted closure (VAC). METHODS: The chronic wounds in 5 patients were treated with VAC. The expressions of MMP-1, 2, 13 in the granulation tissues of the chronic wounds were determined and quantified using RT-PCR technique before and at 1, 4, 7 days after the treatment. RESULTS: The MMP-1, 13 mRNA showed obvious decrease, with the steepest variation of MMP-13. The MMP-2 mRNA also showed a decreased tendency, though in an undulatory fashion. CONCLUSION: VAC can promote healing of chronic wounds through depressing the expressions of MMP-1, 2, 13 mRNA and protein synthesis, depressing the degradations of collagen and gelatin.

[Influence to maxillary development by repairing denudated palates with buccal mucosa and palatal mucosa].

OBJECTIVE: To provide experimental data of different oral mucosa suitable to repair denudated palate. METHODS: Thirty Japanese white rabbits (5 weeks old) were divided into three groups: I group (left hard palate was denudated with right side intact), II group and III group (both sides of the hard palate were denudated with right side repaired by palatal mucosa or buccal mucosa correspondingly). Then the effect of different transplanted mucosa on preventing maxillary deformity was investigated and compared. RESULTS: Maxillary development in the three groups was same and no significant differences among them were discovered. CONCLUSION: Buccal mucosa is effective in serving as reparative tissue for preventing maxillary deformity.

[Effects of dexamethasone and vitamin B12 on EGF, TGFalpha, and TGFbeta1 gene expressions of A/J mouse palatal cells in vitro].

OBJECTIVE: To investigate the molecular mechanism of congenital cleft palate induced by Dexamethasone (DEX) and the counteractive effect of Vitamin B12. METHODS: The RT-PCR was used to detect the independent and joint effects of DEX and Vitamin B12 on the gene expressions of A/J embryonic mouse palatal cells. RESULTS: DEX stimulated the gene expressions of EGF and TGFbeta1, while the expression of TGFalpha left unchanged in the palatal cells. Vitamin B12 had no direct effects on the gene expressions of EGF, TGFalpha, and TGFbeta1; however, it counteracted the effects of DEX on the palate cells. CONCLUSION: DEX induces cleft palate through increasing some gene expressions of growth factors, which can be inhibited by Vitamin B12.

A comparative study on the uptake and incorporation of radiolabeled methionine, choline and fluorodeoxyglucose in human astrocytoma.

PURPOSE: The goal of this investigation was to evaluate uptake and incorporation of 2-deoxy-2-[18F]fluoro-D-glucose (FDG), 11C-methionine, and 11C-choline in 17 patients suspected of grade-II and grade-III tumors using positron emission tomography (PET) and use in vitro astrocytoma cell lines in order to support in vivo findings. METHODS: Seventeen patients with suspected astrocytomas (9 grade-II and 8 grade-III) were studied by PET with FDG and 11C-methionine; and one patient (grade-III) with FDG, 11C-methionine and 11C-choline. Uptake of PET molecular imaging probe was quantitative based on tumor to corresponding contralateral-region uptake ratio, tumor to mean-cortical-uptake ratio, and tumor to white matter uptake ratio. This was correlated with World Health Organization histology grading system and clinical follow-up. Uptake and incorporation of 3H-methionine, 3H-choline and FDG into lipid, RNA, DNA, and protein were investigated in a grade-III human tumor brain-14 astrocytoma cell line. RESULTS: A time-dependent increase in the total uptake of 3H-methionine, 3H-choline and FDG was observed in human tumor brain-14 astrocytoma-III cell line. 3H-methionine was incorporated predominantly into proteins (in excess of 40% at 1 h) while 3H-choline incorporated primarily into lipids (in excess of 60% at 1 hr). Total uptake of FDG was accounted for in the free-pool supernatant fraction. In all patients, PET images of 11C-methionine and FDG provided higher tumor to white matter ratios than tumor to corresponding contra-lateral region ratios and tumor to mean cortical uptake ratios. In grade II patients, FDG did not exhibit significant increase in tumor uptake, while 11C-methionine was a good predictor with ratios of approximately 1.50 +/- 0.48. In grade III patients, both FDG and 11C-methionine exhibited higher ratios than for grade II, with 11C-methionine being the greatest (ratios of 2.50 +/- 0.85), possibly suggesting enhanced protein synthesis. With respect to tumor delineating potential, 11C-choline may be equal to or slightly better than 11C-methionine in the subject evaluated with all three probes. CONCLUSIONS: Results suggest that a combination of FDG and 11C-methionine is useful in the prediction of histological grade of astrocytomas. In addition, 11C-methionine is better than FDG in delineating tumor boundary for low-grade gliomas. In vitro results suggest that 3H-methionine is significantly incorporated into proteins and provides the major driving force in the uptake of 11C-methionine observed in PET images.

[Randomized controlled trial of sodium hyaluronate for degenerative disorders of the temporomandibular joint].

OBJECTIVE: To assess the effect of sodium hyaluronate (HA) for degenerative disorders of the temporomandibular joint (TMJ). METHODS: A prospective randomized controlled clinical trial was conducted. The experimental group received injections in the upper compartments of the involved TMJs with 1% HA 6 mg, whereas the control group received prednisolone (PS) 12.5 mg once a week. Three to four injections were as one course. Before and one week after the treatment courses, clinical symptoms, amount of interleukin-6 (IL-6) and total protein of synovial fluid were measured and compared. RESULTS: Sixty-seven patients were included and 4 out of them were dropped out. There were 12 males and 51 females, among them, 14 cases with synovitis, 21 with anterior disc displacement without reduction and 28 with osteoarthritis of the TMJ. Thirty-five patients allocated in HA group and 28 in PS group. Both drugs could relieve the clinical symptoms of TMJ degenerative disorders. In HA group, marked improvement rate was 51.43% and failure rate was 2.86%, whereas marked improvement rate 39.29% and failure rate 17.86% in PS group. The declined levels of IL-6 in synovial fluid was notably greater in HA group than those in PS group. CONCLUSION: Intra-articular injection of HA is effective and safe to treat TMJ degenerative disorders with mild adverse reactions, better in terms of effective rate and declined level of IL-6 than PS.

[Comparative study on effects of sodium hyaluronate and prednisolone injections on experimental temporomandibular joint osteoarthritis of rabbits].

OBJECTIVE: To assess the treatment effect of sodium hyaluronate (HA) on experimental temporomandibular joint (TMJ) osteoarthritis of rabbits in comparison with prednisolone (PS). METHODS: The upper compartments of both TMJs of 12 Japanese White Ear Rabbits were injected with 0.2 ml of 1.6% papain, 3 days after the right TMJs were injected again with same amount of papain to induce osteoarthritis with different severity levels. Except 1 rabbit was died accidentally. After one week from final injection of papain, the upper compartments of both TMJs of 6 rabbits were injected with HA 1.3 mg, 5 rabbits with PS 1.6 mg weekly for 4 times. At 3, 5 and 7 weeks after the final injection, the rabbits were sacrificed and the TMJs were pathologically examined. RESULTS: The TMJs receiving PS showed predominant structural disorganization, and the right TMJs had much severe pathology. The manifestations were fibrillation, thinner or flaking of the articular cartilage of the temporal part of the joint, and the articular surface was covered with fibrous tissue. Whereas the TMJs receiving HA injections demonstrated limited changes of cartilage, less fibrillation, only local loss of cartilage on outside layer of the surface. In vicinity of the defect area, cluster of the chondrocytes appeared. Pathological scores of the TMJs receiving HA were significantly less than those of the TMJs revieving PS. CONCLUSION: The results suggest that hyaluronate have effect of cartilaginous reparation and protection for the osteoarthritis of rabbit. While prednisolone has no help or worsened for articular cartilage reparation.
