[Infectious corneal ulcers--once with endophthalmitis--after photorefractive keratotomy with disposable contact lens].

BACKGROUND: Infections of the cornea with or without involvement of intraocular tissues are severe complications after photorefractive keratectomy. We report on three cases with ulcerative keratitis and one case with severe endophthalmitis, which developed some days after laser treatment. PATIENTS AND METHODS: After 39 photo-therapeutic and 118 photo-refractive treatments without complications four young treated persons suffered from corneal infiltrations. Three of them developed severe corneal ulcerations, one a severe endophthalmitis. We used the laser machine of the company Schwind (193 nm wavelength, repetition rate 10 Hz, fluence 180-200 mJ/cm2). One part of the treatment which all patients had in common was the application of a disposable contact lens postoperatively, which was worn overnight. THERAPY AND FOLLOW-UP: Because all three patients with ulcerations revealed hyphen of fungi in their corneal-scratch material they were treated with antifungal agents locally and systemically. Due to the local and temporal connection the patient with the endophthalmitis was treated with antifungal agents, too. All corneal ulcerations healed with scars similar to a haze graded 3 to 4, the eye with the endophthalmitis healed almost completely. CONCLUSION: Probably the use of disposable contact lenses postoperatively, especially the overnight wear, during an extremely hot and humid summer lead to the described severe infections of the eyes after the "traumatisation" of the cornea by the excimer laser. A known higher concentration of Aspergillus in the air due to renovations of buildings in the clinical area might have been an additional negative effect. The avoidance of the use of disposable contact lenses postoperatively, especially the overnight wear, is emphasized.

Fetal development of the human orbit.

The development of the orbits in 70 human fetal skulls was investigated by measuring the width and height of the orbital entrance, as well as the volume and depth of the orbital cavity and the interorbital width. For determination of the orbital volume, we used the imprint method; the remaining parameters were estimated. Our measurements showed a linear growth rate for the orbital width, height, and depth. After transformation to the cubic root, the values of the orbital volume also demonstrated a linear increase. The orbital index (height/width x 100) expressed the change in the oval outline of the orbital entrance during fetal development from a flat, wide form to a nearly round form at birth. No statistically significant difference between the right and left orbit was found.

[Proliferation markers, enzyme markers and oncogene expression profile of intraocular melanoma].

In this study 9 uveal melanomas, 1 iris melanoma and 1 conjunctival melanoma were evaluated for their proliferation activity with antibodies to KI67 protein. In addition, the distribution of glutathion-S transferase (alkaline and acid isoforms) and lysosomal cathepsin D protease was demonstrated immunohistochemically. The expression of the oncoproteins c-neu (internal and external domaine) and ras (mutated and non-mutated isoform) were also analyzed with specific monoclonal antibodies. In the case of the metastasing melanoma significant Ki67 protein expression and marked expression of the oncoproteins ras p21 and pan ras were obvious. All other melanomas showed less proliferation and enzymatic activity with a moderate expression pattern for oncoproteins. Regarding the results of the proliferation and enzymatic markers, the tumors were heterogeneous; single cells or clusters may play a role in the prognosis of the tumor if there is an intense immunohistochemical reaction. The influence of histomorphological criteria, e.g., cell subtype, seems to be minor compared to immunohistochemical criteria.

[Ultrastructural findings in the cornea in xeroderma pigmentosum].

Xeroderma pigmentosum is a very rare disease caused by an inherited defect in the DNA repair system. The cells are less able to repair defects caused by UV radiation, which results in early aging of the skin and the development of malignant skin tumors. Robins assumes that the eye is involved in 60% of the cases. In our patient we found a dense opacity of both corneas in addition to the typical lentiginous alteration of the skin. Therefore, a corneal transplantation was carried out on both eyes. The corneal buttons obtained by keratoplasty were examined by light, and transmission electron microscopy. We found massive alterations in the tissues of all corneal layers.

[Cytology of the normal ciliary ganglion].

In view of the increasing importance of tissue cytology in the diagnosis of different diseases the authors studied 12 ciliary ganglions of patients without eye disease who had died at ages ranging from seven months to 76 years. After preparation of the ganglions from the retrobulbar tissue, two different methods were used, i.e., the abklatsch and sedimentation methods. Only a small number of cells could be obtained by abklatsch preparation because the ganglion cells were difficult to separate. With sedimentation, however, excellent results were obtained relating to the morphology of the ganglion cells and their satellite cells. Three types of ganglion cells were distinguished, which differed in size and in their reaction to May-Grünwald-Giemsa staining.

[Initial description of a retinal ganglioglioma].

Gangliogliomas are very rare neuronal tumors, predominantly localized in the thalamus, third ventricle, and temporal lobe. They usually occur in younger patients, grow slowly, and their glial components may develop to malignancy. A case is reported in which a tumor was detected on the upper rim of the optic disk. The patient was then four years old. Five years later the eye hat to be enucleated because of increasing growth and vascularization of the tumor. Histologically, it consisted of mature ganglion cells and neoplastic glial cells. Six years later, in addition to the glial hamartoma, a vascular hamartoma, i.e., a hemangioblastoma of the cerebellum, was found, this tumor was extirpated.

[Limbus-based flap versus fornix-based flap in goniotrepanation].

In a prospective, randomized study 61 eyes in which goniotrepanation had been performed using fornix-based or limbus-based conjunctival flaps were followed up for 4 to 18 months. No differences were found in the results of filtration, nor in the form and frequency of filtering blebs or the frequency of complications. In view of the easier technique and better visualization, the present authors prefer the fornix-based flap.

[Visual fixation tonometry as an aid in the early diagnosis of endocrine ophthalmopathy].

In some cases of autoimmunogenic hyperthyroidism an endocrine ophthalmopathy is found, but it is difficult to diagnose Graves' disease in early stages. Even before the occurrence of measurable morphological changes (measured by sonography and computed tomography of the orbit and eye muscles) short positional intraocular pressure changes are found in 64.5% of cases with autoimmunogenic dysthyreosis. These pressure changes are detected by positional applanation tonometry. These findings admit the conclusion that with the method described not only early detection of ocular signs associated with thyroid disease is possible, but also early differentiation between autoimmunogenic and nonimmunogenic thyroid disorders.

[Follow-up of children after toxoplasmosis infection in pregnancy].

A vival toxoplasmosis infection was diagnosed in 188 cases (3,8%) out, of 49,582 pregnant women routinely screened. 4 children live with congenital toxoplasmosis. Follow-up studies showed that 33 children have specific antibodies, 5 are with suspect clinical findings. Therefore is the conclusion that more late signs of connatal toxoplasmosis are to be detected by follow-up investigations.
