[HbA1 as a parameter of the progression of diabetic retinopathy].

HbA1 was determined in 12 juvenile diabetics with diabetic retinopathy and in ten adult diabetics at four-weekly intervals. During this time the course of the retinopathy was observed. Retinopathy was constant in juvenile diabetics with HbA1 values below 8%. A moderate progression, which it was possible to slow down by laser treatment, was observed in patients with HbA1 values of between 8% and 10%. Patients with HbA1 values exceeding 10% showed a marked progression, which could not be stopped by intensive laser therapy. The behavior of the retinopathy in adult diabetics corresponded to that of the juveniles when the onset of the disease was between the ages of 30 and 50. In elderly diabetics no significant tendency was observed.

[Visual field examinations using the Octopus computerized perimeter following laser therapy of central serous chorioretinitis (Wessing type I)].

The authors examined 19 patients who were suffering from central serous chorioretinopathy were treated with argon laser beams with a special user-defined program of the Octopus computerized perimeter. Relative scotomata were found at a depth of between 4 and 10 decibels in 66 percent of the patients; they were seldom subjectively considered to be a disturbance.

[Partial canaliculus transplantation (author's transl)].

In some cases of palpebral tumors, causing epiphora by canalicular obstruction, surgical reconstruction of the lacrimal system is necessary . The defect of the nasal part of the lower lid is closed by transplantation of the nasal part of the upper lid of the other eye, including a part of the canaliculus. Three patients suffering from basaliomas in the region of the lower canaliculus were treated by this method. Good cosmetic and functional results were achieved.

[Diagnosis and differential diagnosis of hysteric amblyopia using psychophysical and electro-ophthalmological examination methods (author's transl)].

In three patients with psychogenic amblyopia the computerized visual field, the ERG, the flash light (H-EP) and pattern reversal stimulated (M-EP) cortical potentials were recorded. Differential diagnosis between psychogenic amblyopia on the one hand, and functional amblyopia and organic lesions of the retrobulbar visual pathways on the other was shown to be possible. In the cases with organic retrobulbar lesions of visual pathways, visual field defects and pathologic H-EP's and M-EP's were found. In functional amblyopia only the M-EP responses are diminished in amplitude. In psychogenic amblyopia only the computerized visual field shows pathologic changes.

[Electro-ophthalmological responses in inflammations of various sections of the orbital visual pathways (author's transl)].

In a study of electro-ophthalmological responses of the EOG, the photopic and scotopic ERG, the OP and the VECP, in combination with fluorescein angiography and computerized perimetry, in 34 patients suffering from uveitis, chorioretinitis (lesions near the macula and at the periphery), papillitis or retrobulbar neuritis, the following results were obtained: Uveitis and chorioretinitis with lesions either near the macula or at the retinal periphery produce different responses; generally, pathologic responses of the EOG and ERG and normal VECP are obtained in cases of uveitis. Peripheral chorioretinal lesions lead to a diminution of the scotopic ERG, OP and EOG, in contrast to lesions near the macula, in which pathologically diminished amplitudes of the photopic ERG and VECP are found. In papillitis and retrobulbar neuritis, however, the most important peculiarity is the increase in latency and the decrease in amplitudes of the VECP, especially in retrobulbar neuritis.

Chorioretinitis

[Electroophthalmological, Fluorescence angiographical and computer perimetrical examinations in cases of contusio bulbi (author's transl)].

In seven patients with diminished visual acuity, caused by oedema of the retina after contusio bulbi, the EOG, the photopic and the scotopic ERG as well as the VECP, elicited by luminance (H-EP) on the one hand and by pattern reversal-stimulation (M-EP) on the other hand, were registered. These registrations were made both during and after the acute phase. The results were then compared with the results of the normal fellow eyes. Additional fluorescence-angiographic and computer-perimetric examinations were performed. All EOG and ERG components studied showed a significant reduction in the acute phase and a significant increase -- with the exception of the photopic b-wave -- in the regression of the oedema of the retina. However, the values of the affected eyes never reached the values of the normal fellow eyes. The different behaviour of the H-EPs and the M-EPs is striking. While the medium values of the H-EPs showed little reduction in all examinations, the M-EPs showed a highly significant decrease or increase, with the visual acuity and the M-EP showing corresponding behaviour.
