Phase II study of 5-fluorouracil, methyl-CCNU, and daunorubicin in colorectal cancer: a Cancer and Leukemia Group B study.

The three-drug combination of 5-fluorouracil, methyl-CCNU, and daunorubicin was evaluated in 38 patients with unresectable or metastatic carcinoma of the colon. There were five partial responses and one complete response for an overall response rate of 16%. Although toxicity was tolerable, daunorubicin failed to add to the 5-fluorouracil and methyl-CCNU combination.

Thymidylate synthetase and dihydrofolic acid reductase in the stimulated human lymphocyte.

The phytohemagglutinin (PHA)-stimulated human lymphocytes demonstrated trace or no activity (dihydro) folate acid reductase using three methods including a radioassay, but demonstrated ample activity of thymidylate synthetase. This was true regardless of the day of harvest, (first through seventh) of the stimulated lymphocyte. The lymphocyte extracts revealed no inhibitor to the reductase enzyme when these extracts were added before the liver extracts to the assay system. When methotrexate (MTX) was added to the culture media of the lymphocytes, there was, as expected, an increase in the synthetase activity, but the expected rise in the reductase activity did not occur, it remained nil. On the other hand, MTX did influence the incorporation of nucleosides by the stimulated lymphocytes in a fashion similar to its action on the incorporation of the same nucleosides by other cells.

Adaptive coping mechanisms in adult acute leukemia patients in remission.

The adult leukemia patient in a drug-induced remission possesses a unique set of emotional responses as he adjusts to an altered life-style and a life-threatening disease. Six patients, ranging in age from 24 to 62 years and being treated with monthly maintenance chemotherapy, were interviewed over a six-month period. The adaptive coping mechanisms most frequently identified were denial of being sick, identification with fellow patients to form a "hospital family," and anticipatory grief of one's own losses by participation in grieving another patient's death. The means of adjustment was to adapt to the "hospital family" and benefit from the therapeutic milieu established on the ward. The patient's total response to remission in acute leukemia can be influenced positively by appropriate intervention based on an assessment of his previous and present patterns of coping.

Evaluation of a hexamethylmelamine and 5-fluorouracil combination in the treatment of advanced ovarian carcinoma.

The combination of two nonalkylating agents, hexamethylmelamine (HMM) and 5-fluorouracil (5-FU), was evaluated in 14 patients with advanced ovarian carcinoma. Nine patients had been treated previously with alkylating agents. The overall response rate was 36% which is not greater than could be expected with either HMM or 5-FU alone. However, the median survival of the five responders was greater than 24 months. Treatment had to be discontinued in five of 14 patients due to excessive toxicity. Although the toxic effects were predominately gastrointestinal, unpredictable hematopoietic and neurotoxic effects were observed.
