The Scandinavian multicenter photopatch study. Preliminary results.

The Scandinavian photopatch test procedure has been applied to 745 patients with suspected photodermatoses during the years 1980-1981. Our experience has been encouraging with the recording of several relevant reactions. A total of 132 positive photocontact reactions and 120 ordinary contact reactions were seen. Photocontact reactions to musk ambrette (19 cases) and PABA (19 cases) were surprisingly frequent. The next most common photocontact reactions were to promethazine (24), chlorpromazine (22 cases) and fentichlor (12). Ordinary contact reactions were observed to balsam of Peru (30), PABA (23), lichen mix (21), wood mix (14) and to perfume mix (10).

Etomidate suppresses adrenocortical function by inhibition of 11 beta-hydroxylation.

We studied the effect of short term infusion of the imidazole-derived anesthetic agent etomidate on plasma concentrations of ACTH, cortisol, and the cortisol-precursors 11-desoxycortisol and 17-hydroxyprogesterone. During the infusion of etomidate, a significant increase in the peripheral concentration of ACTH occurred, while plasma cortisol concentrations decreased. After the end of the infusion, cortisol concentrations further decreased, while the concentrations of desoxycortisol and 17-hydroxyprogesterone increased. Furthermore, in in vitro experiments with isolated rat pituitary and adrenal cells, etomidate did not affect corticotropin-releasing hormone-induced ACTH secretion from pituitary cells, whereas ACTH-stimulated corticosterone secretion from adrenal cells was inhibited by addition of etomidate in concentrations which occur in plasma during and after infusion of the drug. These results lead to the conclusion that etomidate inhibits adrenal 11 beta-hydroxylation.

Serum cholesterol and triglycerides and hyperlipoproteinemia in elderly women.

Calculated energy, fat and carbohydrate intakes, and serum cholesterol and lipid concentrations for 70 elderly women 62 to 99 years of age were related to age, income, education, and type of residence. Serum lipoproteins were determined by electrophoresis. Total fat and saturated fatty acid intakes decreased with age. As percentage of energy from fat decreased, percentage of energy from carbohydrate increased. Approximately one-fourth of the women had cholesterol levels in excess of 250 mg/dl, and 41% had triglyceride levels above 170 mg/dl. Mean triglyceride and cholesterol levels were higher in younger subjects than in older subjects, in higher income groups compared to lower income level, and cholesterol levels of women with education beyond high school level exceeded those of women with less education. Hyperlipoproteinemia, diagnosed for 14% of the subjects, occurred most often in the 70- to 79-year group.

Particulate myocardial perfusion scintigraphy: its clinical usefulness in evaluation of coronary artery disease.

The determination of myocardial viability is a crucial parameter in the evaluation of a patient with coronary artery disease. Particulate myocardial perfusion scintigraphy (PMPS) is one of several methods available for this determination. A brief comparison between PMPS and two other commonly used methods is described in this review. The technique and analysis are described and applied to 194 patients with 366 surgically bypassed regions. The patients are analyzed for postoperative regional wall function and for total ventricular function as expressed by ejection fraction. PMPS is a highly accurate and unique study for clinically predicting myocardial viability.

Intakes and serum levels of protein and iron for 70 elderly women.

Calculated protein and iron intakes, serum protein levels, and measurements of iron nutriture for elderly women were related to age, income, education, and type type of residence. Low or deficient levels of total serum protein and albumin were observed for 36 and 20% of the subjects and low hemoglobin and elevated total iron-binding capacity values were recorded for 19 and 40% of the subjects. Values below the acceptable standards for albumin and hemoglobin and above the acceptable range for total iron-binding capacity were greater for nursing home patients than for private home residents. Nutrition intervention appears to be a crucial part of correction of the deficient biochemical measurements which may result from malnutrition and/or pathological conditions.

[Proceedings: Studies on myocardial perfusion by means of marked particles, injected selectively into the coronary system: an aid in preoperative evaluation of patients with coronary sclerosis].

Myocardial color scintigraphy using macroaggregates labeled with 99mTc and 131I were performed in patients with coronary heart disease. The particles were injected selectively into the coronary arteries. The scintigram was obtained using a color coded display system. The channel with the maximum counts was assigned the color red and each 10% change resulted in a different color. A count rate below 40% of the maximum was considered inadequate. 80 patients undergoing coronary surgery were evaluated postoperatively. Postoperative ventriculography documented improved contractions in 34 of 40 ventricular segments (85%) in which preoperatively an adequate perfusion was observed, whereas in only 13 out of 42 segments with preoperatively inadequate perfusion was an improvement in ventricular contractility observed.

The abnormal bone scan in intracranial lesions.

In 43 patients with abnormal brain scans restudied within 2-7 days with 99mTc-labeled ethane-1, hydroxy-1, diphosphonate (EHDP), cerebral infarctions, primary and metastatic neoplasms, chronic subdural hematoma, arteriovenous malformations and inflammatory lesions were visualized. The localization of EHDP in primary and metastatic neoplasms is usually less apparent than pertechnetate. Conversely, the localization of EHDP in cerebral infarctions is usually more apparent than pertechnetate. 99mTc-EHDP, in conjunction with pertechnetate, may become useful in differentiating cerebral infarctions from neoplasms. Further, skull scans must be interpreted with the appreciation that bone-seeking radiopharmaceuticals may localize in a variety of intracranial lesions.

Report of the Inter-Society Commission for Heart Disease Resources. Optimal resource guidelines for radioactive tracer studies of the heart and circulation.

The status of radioactive tracer studies in cardiovascular diagnosis is summarized. Planning guidelines are suggested for a hospital nuclear medical service including a central nuclear medical facility and satellite units, and resource criteria are given for professional personnel including training and duties, equipment, space, and support system. The administrative organization of a nuclear medical service and case loads are discussed. Guidelines for equipment maintenance and quality controls are described.

A tool for individualized management of fat-controlled diets.

Presently recommended fat-controlled dietary plans are rigid in pattern and require omission of some foods and substitution of others with little regard for the individual's food preferences. A tool for individualized management of fat-controlled diets has been developed. Commonly eaten foods have been grouped according to their fat content. After the patient's habitual dietary intake is analyzed by computer or food composition table, changes in the diet are recommended, using the food groups, so that the guidelines of a fat-controlled diet may be met with a minimum deviation from individual food preferences.
