Operative treatment of displaced talar neck fractures.

Twenty-two displaced talar neck fractures were treated by open reduction and internal fixation. Four open fractures were operated within 8 hours and 18 closed injuries were treated at an average interval of 13.8 hours after injury. Fractures were classified according to Hawkins' classification into 14 type II, 7 type III and 1 type IV. At an average follow up of 4.4 years, 18 cases obtained excellent or good results. Result was fair in 2 cases of delayed union and 1 case of avascular necrosis. Another case of avascular necrosis developed osteoarthritis of the ankle and had poor result. The overall incidence of avascular necrosis of the body of the talus was 9%.

The effect of platyspondyly and pubertal growth spurt on the stature of patients with beta-thalassaemia major.

OBJECTIVE: To study the effect of the body proportion and pubertal growth spurt on the stature of children with beta-thalassaemia major. METHODS: The height, sitting height, upper to lower segment (U:L) ratio and pubertal development were determined in 71 Chinese children (38 girls and 33 boys) with beta-thalassaemia. The growth patterns of 20 patients with complete growth data between 3 years and final height, were analyzed according to whether they underwent a pubertal growth spurt or not. RESULTS: 27% of the boys and 32% of the girls had a height below the 3rd percentile. About 60% of all the children had a U:L ratio below the 10th percentile for age. Abnormal body proportion was found in patients with or without growth retardation. 34% of the 41 children over the age of 14 years underwent spontaneous puberty. In 28 patients over the age of 16 years, a growth spurt was observed in 46% of the children during spontaneous or induced puberty. The retrospective analysis showed that the height deviation from the mean in adulthood was significantly higher in patients without pubertal growth acceleration than in those with a growth spurt (x = -11.8 cm, s = 7.6 cm vs x = -4.4 cm, s = 4.4 cm; P = 0.02). CONCLUSIONS: An abnormal U:L ratio was commonly observed in patients with beta-thalassaemia major and may be one factor contributing to the short stature of these patients. Abnormal puberty was present in a significant proportion of children and the lack of a pubertal growth spurt was found to be detrimental to adult height.

Cross cultural adaptation and validation of the Chinese Health Assessment Questionnaire for use in rheumatoid arthritis.

OBJECTIVE: The Health Assessment Questionnaire - Disability Index (HAQ), used as a disability and outcome measurement in rheumatoid arthritis (RA), has been validated in several languages, but not in Chinese. Our aim was to validate the Chinese version of HAQ (Chinese-HAQ) to suit the needs of Chinese speaking patients with RA in an Asian setting. METHODS: The original HAQ was modified in the context of Chinese culture and translated into Chinese by 2 translators aware of the objective of the questionnaire. The Chinese HAQ was self-administered by 42 patients with RA during their routine followup visit and one week later. RESULTS: The test-retest reliability assessed using Spearman's correlation coefficient was 0.84. Between dimensions measured in the HAQ, the highest test-retest reliability was observed for walking (Spearman correlation coefficient rs=0.80) and the lowest was for eating (rs=0.54). The internal consistency of the scale using Cronbach's alpha was high at 0.86. In terms of criterion validity, the Chinese-HAQ score was found to correlate well with American College of Rheumatology functional status (rs=0.501, p=0.01). The Chinese-HAQ scores also correlated well with markers of disease activity such as patient's perception of pain measured on a visual analog scale (rs=0.55, p < 0.001), grip strength in mm Hg (rs=-0.55. p < 0.001 ), and physician's assessment of disease activity (rs=0.59, p < 0.001). CONCLUSION: The Chinese HAQ is a reliable and valid instrument for studies measuring disability of patients with RA in Singapore.

Comparison between single injection transthecal and subcutaneous digital blocks.

A randomized double blinded study was performed on 142 patients to evaluate two different techniques of single injection digital anaesthesia. In group A, 86 digits in 71 patients were anaesthetized by a single injection transthecal technique using 3 cc of lignocaine and bupivacaine mixture. Anaesthesia of the whole digit was achieved in 83 (97%) digits. In group B, 80 digits in 71 patients were anaesthetized with a single injection subcutaneous technique using the same amount of anaesthetic mixture. Total anaesthesia of the digit was achieved in 75 (94%) digits. These two techniques were found to have no differences in effectiveness, distribution, onset and duration of anaesthesia.

Comparison of transthecal and subcutaneous single-injection digital block techniques in cadaver hands.

A controlled, randomized single-blinded study was performed on the thumbs and little fingers of 20 cadaver hands. The digits were randomly divided into 2 groups. In the transthecal group, 2 mL of 0.5% methylene blue was injected into the tendon sheath at the A1 pulley. In the subcutaneous group, the same amount of dye was injected into the subcutaneous tissue superficial to the A1 pulley. The injections were performed by 2 investigators. They exchanged specimens and performed dissections on the injected digits without knowledge of which technique had been used. The distributions of dye along the digit and the color intensity of the dye on the digital nerves were studied. There was no significant difference (p > .05) between results for the 2 techniques. It was expected that both techniques would result in similar anesthetic distribution in the clinical setting. In the transthecal group, intra-articular staining of the metacarpophalangeal joint was noted in 3 little fingers and 1 thumb. This complication did not occur in the subcutaneous group. This difference was significant (p < .05).

Comparison of transthecal and subcutaneous single-injection digital block techniques.

A randomized double-blinded study was performed on 20 normal volunteers to evaluate 2 different techniques of single-injection digital anesthesia. Single-injection transthecal digital block technique was used to anesthetize 1 index finger and single-injection subcutaneous technique to block the other index finger. Pain and light touch were evaluated and sensory nerve-conduction studies were performed on both index fingers. These data were obtained prior to the nerve blocks and then at 10-minute intervals until recovery from the anesthesia. The method of anesthesia was found to have no effect on the distribution, onset, and duration of anesthesia. Median and radial nerve sensory nerve action potential amplitude reductions following digital anesthesia were also not influenced by the technique of anesthesia. Single-injection subcutaneous block was found to be easier to administer and to produce less pain during and 24 hours after injection than did the single-injection transthecal technique.

A retrospective study on elbow function after internal fixation of intercondylar fracture of adult humerus.

Between 1989 and 1993, 20 intercondylar fractures of the distal humerus were treated by open reduction, internal fixation and early postoperative mobilisation. One patient died on the third postoperative day as a result of multiple injuries, leaving 19 patients for evaluation. The mean follow-up period was 4.1 years (range 2.0 to 6.7 years). According to the Muller system, there were 7 type C1 and 12 type C2 fractures. Using the Jupiter criteria, 6 elbows were rated as excellent, 9 good and 4 fair. Complications included late ulnar neuritis in one patient and wound infection in another patient.

A retrospective evaluation of operative treatment of ankle fractures.

Between 1990 and 1992, 89 patients with ankle fracture were treated by open reduction and internal fixation in accordance with the Arbeitsgemeinschaft für Osteosynthesefragen (AO) principles and were available for evaluation with a mean follow-up period of 3.6 years (range 3 to 5 years). According to the AO classification, there were 21 type A, 37 type B and 31 type C fractures. The results were rated by the Olerud and Molander score. Excellent or good results were seen in 20 type A, 35 type B and 25 type C fractures. Fair or poor results in 1 type A, 2 type B and 6 type C fractures. Complications included screws in the joint in 2 cases and infection in 3 cases.

Infection of vascularized fibular grafts.

Between June 1978 and July 1991, 64 limb reconstructions were done using vascularized fibular grafts. Of these, 10 became infected. Five grafts were proven viable, whereas the other 5 were proven nonviable based on the survival of the skin in the composite osteocutaneous graft and from bone scans, angiograms, and biopsies. A distinct difference in the radiologic manifestation and clinical course of the infection was noted between the viable grafts and nonviable grafts. The viable grafts showed radiologic changes of osteomyelitis that were localized, and the graft incorporated, healed with antibiotics, and exhibited graft hypertrophy. In the nonviable grafts, the radiologic changes were extensive, evidence that resorption of the grafts had resulted. This suggests that, because of the poor prognosis associated with infection of the nonviable vascularized fibular grafts, the infected grafts should be removed early to minimize the morbidity and to shorten the protracted course associated with infection. With the infected viable grafts, efforts at salvaging the graft with multiple debridements and systemic antibiotics were rewarding.

Comparison of various interpositional materials in the prevention of transphyseal bone bridge formation.

Various interpositional materials, except muscle, have been used to prevent transphyseal bone bridge formation after resection of the damaged physeal plate. In this animal model, muscle was used as an interpositional material, and its effectiveness was compared with that of 3 known materials (fat, physeal allograft, and iliac apophyseal autograft). Five experiments were done on the distal femoral physis of 40 skeletally immature 3-month-old New Zealand white rabbits. The rabbits were divided into 5 groups, each containing 8 rabbits. A standard defect was created in the lateral distal physis of the left femur in all the rabbits. In Group A, there was no interpositional material. Vastus lateralis muscle, groin fat, physeal allograft, and iliac apophyseal autograft were inserted into the femoral defect in Groups B, C, D, and E, respectively. The right femur served as a sham control for the animals. The animals were sacrificed at 12 weeks after surgery. The results of limb length discrepancy and angular deformity of the groups with interpositional material were compared with those of Group A (experimental control). Muscle, fat, and iliac apophyseal autografts had less severe limb length discrepancy and angular deformity. These differences were statistically significant, whereas the differences between allograft and experimental control were statistically insignificant.

Comparing staple fixation to buttress plate fixation in high tibial osteotomy.

A retrospective study was made between July 1988 and March 1992, of 171 patients (223 knees) who had high tibial osteotomy at the Singapore General Hospital. Seventy-four cases (97 knees) had buttress plate fixation and 97 cases (126 knees) had staple fixation. The mean postoperative Hospital for Special Surgery Knee Score was 87.0 for the buttress plate group and 87.4 for the staple group. The difference was statistically not significant (P > 0.05). The hospitalisation stay was shorter in the staple fixation group as compared to the buttress plate group (P < 0.05). However, the mean rehabilitation time was shorter in the buttress plate group as compared to the staple fixation group (P < 0.05). The incidence of wound infection was higher among those fixed with buttress plate (9 cases) as compared to those fixed with staple (1 case). The difference was statistically significant (P < 0.05). Buttress plate fixation did not seem to eliminate the risk of non-union of the osteotomy (4 out of 126 osteotomies compared to 3 out of 97 in the staple group), neither did it prevent recurrence of varus deformity (2 out of 126 osteotomies). Overall it would appear that buttress plating did not have any significant advantage over staple fixation for high tibial osteotomy.

Contributions of various major causes of death to life expectancy in Singapore, 1980-1990.

This paper considers the contributions by age of the various major groups of deaths to the increase in life expectancy at birth between 1980 and 1990 for both sexes in Singapore. Sixteen cause groups were used in the study. The data were analysed using LIFETIME, a personal computer package with a wide variety of methods for mortality investigations. Respiratory diseases made the largest contribution to the increase in life expectancy for both sexes. In contrast, ischaemic heart disease made a negative contribution of 1% in the gain in female life expectancy but contributed 12% improvement for males. Life tables for Singaporean males and females in the year 2000 were projected by extrapolating the mortality trends observed in earlier periods. The calculations show that the life expectancy at birth in the year 2000 to be 74.72 years for males and 79.48 years for females.

Factors affecting healing of below knee amputation.

Between January and December 1992, 54 diabetic patients with 60 below knee amputations performed in the Department of Orthopaedic Surgery O, Singapore General Hospital were available for evaluation. The average follow-up period was 16.3 months with a range of 13 to 24 months. Five patients had stump necrosis and above knee amputation was performed. Eight patients had partial stump necrosis and required debridement with or without secondary procedures. The other patients had uneventful stump healing which was defined as healing of the stump wound without complications and remained intact for at least 6 months after surgery. Absence of popliteal pulse, presence of central cardiovascular disease and absence of intraoperative skin flap bleeding were found to be associated with higher incidence of stump necrosis.

A cadaver study of the effects of dorsal angulation and shortening of the metacarpal shaft on the extension and flexion force ratios of the index and little fingers.

Two experiments were performed on the second and fifth metacarpals of five normal cadaver hands. The forces obtained on full extension and flexion of the digits were measured. An oblique osteotomy was performed on the shaft of the metacarpal and fixed with dorsal angulation. The forces obtained on extension and flexion of the digits were measured. The relationships between the changes in force and the angle were analyzed. Flexion force decreased and extension force increased as the dorsal angulation increased, and these were significant beyond 30 degrees of dorsal angulation. The differences between index and little fingers were not significant. In the second experiment, the metacarpal bone was shortened at the osteotomy site, and the same measurements made. Flexion and extension forces both decreased, and were significant beyond 3 mm of shortening. The differences between index and little fingers were not significant.

Infection in diabetic patients with ankle fractures.

Between January 1992 to June 1993, 93 ankle fractures underwent surgical treatment, of which, 10 patients were diabetic and 83 were non-diabetic. Infection occurred in 5 patients and all belonged to the diabetic group. The average follow-up period of the infected cases was 16.2 months. Wound infection occurred in 4 patients treated with open reduction and internal fixation. An infected pressure sore developed in the other patient treated initially in a below-knee cast for which arthrodesis of the ankle was performed. Infection was resistant to treatment in 2 patients and they ended up with below-knee amputation. Infection resolved in 3 patients with treatment. Of these, 1 developed neuropathic ankle joint and the other 2 achieved fracture union.

A case report on bilateral avascular necrosis of the medical cuneiforms.

Avascular necrosis rarely affects the cuneiform bones. The treatment has been mainly conservative and histological confirmation of osteonecrosis is therefore not available. The authors report a case of bilateral avascular necrosis of both medial cuneiforms with histological confirmation. The patient was treated operatively with decompression of intra-osseous pressure through multiple drill holes. Six weeks postoperatively, the symptoms subsided and radiographs showed reconstitution of both medial cuneiforms.

Exostosis presenting as solitary loose body in the ankle of two children.

Two children, aged 5 and 11 years, presented clinically with ankle pain and a mobile bony hard lump in the ankle. Radiological examination revealed a large radio-opaque shadow in relation to the talus. Intraoperatively, a loose body was found. It resembled a detached exostosis with cartilaginous cap and was confirmed histologically as an osteochondroma. In the older child, there was a remnant stalk attached to the lateral surface of the talus. In the younger child, the exostosis probably came from the talus as the osteochondroma could not originate from the epiphyses that form the ankle mortise.
