Crohn's disease in a defined population course and results of surgical treatment. I. Small bowel disease.

In a total, unselected series of 207 patients with Crohn's disease diagnosed between 1958 and 1974, 87 of the 182 patients operated upon during the investigated period were, at a comprehensive reevaluation, found to have the disease predominantly or exclusively confined to the small bowel. In five of these cases wide-spread lesions were present. The frequency of preoperative sequelae or complications was low. Postoperative complications were comparatively few with only one death. The calculated recurrence rate at 15 years was 43%. In spite of the fact that some patients needed repeated resections, the outcome of surgery as assessed at the end of the observation time was good in most cases.

Crohn's disease in a defined population course and results of surgical treatment. Part II. Large bowel disease.

In a total consecutive series of 182 patients operated upon for Crohn's disease, 95 (52%) had the lesions predominantly or exclusively confined to the colon. A "radical" operation was performed in 88 of these 95 patients. The general policy was to spare the rectum, if possible. It was found that when the rectum was primarily involved the lesions did not disappear after subtotal colectomy. Ileorectal anastomosis was only successful in 9 of 22 cases. In all these nine cases the rectum was uninvolved preoperatively. The overall mortality rate related to Crohn's disease was 6% and the overall recurrence rate 62%. All patients with an anastomosis between a disease-free small intestine and a diseased colon developed recurrence in the ileum. Therefore, a thorough preoperative examination of the whole colon is essential.

The double contrast examination in inflammatory large bowel disease. A prospective clinical, radiographic, and pathologic study.

In a prospective clinical series of 2371 consecutive patients referred for double contrast examination (DCE) of the large bowel, 154 had clinical and/or radiographic signs of inflammatory disease. The patients were followed up from May 1976 until May 1981. At the time of interpretation of the roentgenograms, the radiologist was unaware of clinical and laboratory findings. In 31 patients, organ specimens were studied histologically and in 101 other multiple endoscopic biopsies were available. Basing on the clinical observations and the pathology reports, the accuracy of the DCE was calculated at 99%; the predictive value of a positive DCE at 92, and that of a negative DCE at 99%.

Carcinoma of the anal canal.

A 20-year retrospective study of all anal canal carcinomas in the city of Malmö has been performed. The series consists of 29 patients, 23 women and 6 men, with a mean age of 66 years. Eight patients had non-keratinizing squamous cell carcinoma, 7 had keratinizing squamous cell carcinoma, 12 had basaloid squamous cell carcinoma, and 2 patients had mucoepidermoid carcinoma. Two of four patients operated on for curative purposes with local excision survived 5 years, as did seven of nine patients operated on curatively with abdominoperineal resection. In discussing the results, an overview of the literature is presented. It is concluded that adjuvant combined chemotherapy and radiotherapy appears to result in a higher survival rate for patients with tumours growing at or above the dentate line.

Malignant hemangioendothelioma of metacarpal bones.

Malignant hemangioendothelioma is a tumour of vascular origin with wide distribution in various organs and tissues of the body. However, it is seldom localized primarily in bone. In this report our experience with a case of primary multifocal malignant hemangioendothelioma of the metacarpal bones is reported. Because of the focal pattern of malignancy excisional biopsy failed to give the diagnosis which was not established until the tumour was radically removed.

Angiography in angiomatous lesions of the gastrointestinal tract.

Twenty patients with various angiomatous lesions of the gastrointestinal tract, such as right-colon angiodysplasia and manifestations of Rendu-Osler-Weber disease and Klippel-Trenaunay syndrome are reported. During the past 15 years an increasing number of such lesions have been reported, diagnosed mainly by angiography. The major complication from these vascular lesions is bleeding. Angiography is the primary method for their detection because of their often small size and is a prerequisite before surgery when reason exists for anticipating an angiomatous lesion as the source of bleeding. However, in certain cases angiography will fail to demonstrate the vascular abnormality or its full extent.

Radiographic appearances in Crohn's disease. III. Colonic lesions following surgery.

Posteroperative films of 114 patients subjected to a primary operation for Crohn's disease were reviewed with special regard to postoperative lesions in the colon. Recurrence in the colon was in 13 patients of 16 due to an exacerbation of the disease in segments left behind at operation. Recurrence in the new distal ileum in addition to persistent colonic disease occurred in 11 patients of 12. This was probably due to the operative connection of the ileum to a diseased colon.

Colitis↗

Radiographic appearances in Crohn's disease. IV. The new distal ileum after surgery.

Postoperative films of 150 patients operated upon for Crohn's disease were reviewed with regard to postoperative abnormalities in the new distal ileum. The lesions were always located in the most aboral part of the ileum. In patients with persistent colonic disease the ileal lesions were apparently the result of a spread of colonic lesions. In many of the remaining patients there was strong evidence that recurrent disease was representative of persistent disease.

The yield of endoscopy and double contrast radiography for the detection of early gastric cancer in Malmö.

To evaluate the efficacy of endoscopy and double contrast radiography for the diagnosis of early gastric cancer we compared two three-year materials of all gastric cancers detected in Malmö before and after these diagnostic tools were introduced. Despite a high diagnostic potential of these methods they have not significantly increased the detection of early gastric cancer in Malmö. Evidently the selection of the patients is unsatisfactory and modifications of the indications for these examinations are discussed.

Superficial lesions in Crohn's disease of the small bowel.

Double-contrast examinations of the small bowel were compared with operative specimens obtained within 4 weeks after X-ray evaluation of 20 patients with Crohn's disease. Objective evidence of superficial lesions was submucosal edema and superficial ulcerations observed in the operative specimens. Edema was seen radiographically in 18 of 20 patients. Superficial ulcerations were seen radiographically in 8 of 14 patients. These ulcerations were either single with edematous edges or smaller, multiple, and more uniformly spread. The diagnosis demands a high-quality technique and even then the diagnostic accuracy is not always optimal.

Crohn Disease↗

Radiographic appearances in Crohn's disease. I. Accuracy of radiographic methods.

Preoperative films of 176 patients operated upon for Crohn's disease were reviewed and compared with microscopic findings in the resected specimens in order to assess the accuracy of the radiographic techniques used. It was found that a diagnosis of the presence and the extent of the disease was often dependent on the demonstration of minor mucosal ulcers and less marked submucosal edema. These lesions were not adequately demonstrated by any of the techniques used for small bowel examination, but were consistently revealed by the double-contrast technique used for examination of the colon.

Radiographic appearances in Crohn's disease. II. The course as reflected at repeat radiography.

The course and distribution of the lesions in 174 patients with Crohn's disease were evaluated from preoperative films. In the small intestine the course was locally progressive. Colonic disease involved either the right hemicolon or the entire colon. Fluctuations of mucosal lesions were common and could explain the radiographic finding of segmental and left-sided colitis. Variations in the anatomy of the lymphatic system could explain the varying distribution of the lesions.

Intermittent treatment of psoriasis with clobetasol propionate.

Clobetasol propionate cream, applied intermittently for 2 weeks, induced a rapid clinical and histological clearing of psoriatic infiltrates. The serum cortisol level was transiently lowered. By maintenance therapy, with the corticosteroid given only one day a week, 8 of 12 patients were kept in remission for an average period of 5 months.
