[Clinical study on induction of labor with feedback pulsatile oxytocin system].

OBJECTIVE: To improve the safety and the effect on induction of labor with oxytocin. METHOD: 112 cases were selected for labor induction with feedback pulsatile oxytocin under the monitoring of uterine contraction controller (study group). 112 cases were selected with continuous intravenous infusion of oxytocin as a control (control group). The dose of oxytocin and the pulse time-point were adjusted. The effects of labor induction in the 2 groups were compared. RESULTS: The most suitable time-point was 1.5 minute after the uterine contraction. The best administration duration was 20 seconds. But the most suitable dose varied individually, generally 12-25 mU. The mean time to establish contraction in study group was significantly shorter (1.9 +/- 1.2 min) than that of the control group (115.5 +/- 72.3 min, P < 0.001). The average dose of oxytocin administered per hour was significantly less in the study group (312 +/- 64 mU) than that of the control group (735 +/- 125 mU, P < 0.001) the success rate was 100% in study group whereas only 90.2% in control group (P < 0.01). The total duration of labor in the study group was 6.9 +/- 4.7 hours in contrast to 20.4 +/- 10.9 hours in the control group (P < 0.001). The occurrences of dystocia, postpartum hemorrhage and neonatal asphyxia were lower in study group. CONCLUSION: This study proved that the administration of oxytocin with the feedback pulsatile oxytocin system is easier to establish the effective uterine contraction, and it is a more physiologic and effective regimen for induction of labor.

[Otoacoustic emission and auditory efferent function testing in normal subjects and patients with sensori-neural hearing loss].

OBJECTIVE: To examine the role of otoacoustic emission and function test of the auditory efferent system in evaluating of auditory functional status in both normal and diseased conditions. METHODS: Spontaneous and transient evoked otoacoustic emission and efferent function in terms of contralateral white noise induced transient evoked otoacoustic emission suppressions were tested and cross compared in 32 normal ears, 24 ears with cochlear hearing loss and 16 ears with retrocochlear hearing loss. RESULTS: Transient evoked otoacoustic emission amplitude and contralateral suppression, spontaneous otoacoustic emission incidence, peaks and frequency range were significantly reduced in cochlear hearing loss, while in retrocochlear hearing loss transient evoked otoacoustic emission and spontaneous otoacoustic emission levels were significantly higher than in cochlear hearing loss group and showed no suppression. CONCLUSION: Contralateral acoustic stimulation induced transient evoked otoacoustic emission suppression as an index of efferent function is clinically feasible. Combined otoacoustic emission and efferent tests are of great significance in evaluating cochlea status and auditory central mechanisms. Auditory efferent function is weakened in cochlear lesions and severely damaged in retrocochlear lesions.

[Chemosensitivity test for human osteosarcoma cells].

In this experiment, the sensitivity of human osteosarcoma cells to various concentration gradients of HDMTX, VCR, CBP, MMC, VP16, PMB and their time-effect relationship were examined in vitro with MTT assay among 23 cases' fresh osteosarcoma (OS) tissues. It was found that OS was sensitive to MMC and PMB when the drug concentrations were equal to the calculated in vivo drug concentrations. When the concentrations were 5 times as high as those of the calculated in vivo concentrations, OS was sensitive to HDMTX, CBP, MMC and PMB. The positive rates of sensitivity were highest in 72 hours with an average of 55.1%.

[Effect of electro-acupuncture and transcutaneous electric nerve stimulation on experimental diabetes and its neuropathy].

In this paper, the experimental diabetic rats induced by streptozotocin (STZ, i.p. 50 mg/kg) were divided into three groups, electro-acupuncture (EA group, n = 8), transcutaneous electric nerve stimulation (TENS group, n = 8), at bilateral Shenshu and Zusanli points for 20 minutes once every 2-3 days for five weeks, and without any treatment (DM group, n = 6) respectively. As compared with the DM group, the increased plasma glucose levels was lowered significantly in EA group (P < 0.05) and slightly in TENS group (P > 0.05) by the end of the sixth week. And the symtoms of polyphagia, polydipsia and polyuria were attenuated in EA group. The motor nerve conduction velocity slowing was prevented or corrected after a course of four and six weeks treatment in EA and TENS group respectively. In the DM and TENS groups the pain threshold was lowered at 6-20 days after injection of STZ, but elevated in EA group, their differences were of significance (P < 0.05). In general, the efficiency of EA treatment on experimental diabetes and its neuropathy was better than that of TENS therapy.

[The diagnosis and treatment of intracranial tuberculoma].

OBJECTIVE: To improve the level of diagnosis and treatment of intracranial tuberculoma. METHODS: Fourteen intracranial tuberculoma cases were hospitalized in the past five years, accounting for 0.9 per cent of the intracranial tumors in the same period. Five out of fourteen cases were treated conservatively while other nine were treated surgically. All the cases were received 6-month short-course anti-tuberculosis chemotherapy, among which only one case's treatment was prolonged to one year. RESULT: All the cases had good outcome, none of them dead. CONCLUSIONS: Operative indications of intracranial tuberculoma cases should be applied strictly. Good result could be gained via a short-course anti-tuberculosis chemotherapy, no matter operation was performed or not.

[Effect of fuzheng huayu recipe in treating posthepatitic cirrhosis].

This article deals with the effect of Fuzheng Huayu recipe (FZHYR), composed of Semen Persicae, Cordyceps, Radix Salviae Miltiorrhizae, Pollen pini, etc. in treating posthepatitic cirrhosis. 40 patients treated with FZHYR were observed, and another 40 patients in simila condition treated with symptomatic therapy were taken as control. Results showed that the FZHYR could (1) Increase the serum level of albumin and decrease the gamma-globulin; (2) Elevate the lowered plasma branched-chain amino acid/aromatic amino acid ratio; (3) Reduce the raised level of laminin and hyaluronic acid in serum; (4) Escalate the level of CD3+, CD4+, CD4+/CD8+ ratio, NK cells activity and complement C3, lowered the raised level of IgG and IgM in serum. In addition, it could also adjust the anormal change of hormonal secretion. These results suggested that FZHYR has beneficial effect in treating posthepatitic cirrhosis, it acts multifariously and plays important role either in controlling the development of disease or in preventing the complications.

[Morphological studies of the adrenal zona glomerulosa cells and the thyroid and pituitary glands in streptozocin-induced experimental diabetic rats].

Changes in pathological morphology in the adrenal glomerulosa, the thyroid and pituitary glands in streptozotocin-induced experimental diabetic rats were studied by histologic, ultrastructural examination and morphometry. Experimental diabetes provoked atrophy and retrograde degeneration of organelles in the adrenal zona glomerulosa cells and the thyroid follicular epithelium. In the pituitary of diabetic rats, organelles in thyrotrophs were the same as the controls in morphometry, but TSH release was impaired. These studies indicate that pathological changes in the zone glomerulsa cells and the thyroid of diabetic rats appear to be responsible for the impaired function of these two glands. Moreover, reduced TSH release in the pituitary glands is also associated with impaired function of the thyroid gland.

[Analysis of human hair lead and zinc during various physiological periods].

Hair specimens were collected from 1,477 healthy persons at various physiological periods in different areas of Henan Province and determined for their lead and zinc contents, to study systematically the trend of their changes, correlationship between them and their intrinsic connection. Results showed hair lead level was higher in persons, except neonates and the elderly, living in urban area than in rural one. Hair lead level correlated inversely with hair zinc level in logarithmic scale during various physiological periods, with a coefficient of correlation of 0.971. It suggested there possibly is antagonism between lead and zinc in human bodies. Theoretical explanation of the above results and the possibility to use zinc to prevent and treat the hazard caused by lead were provided.

Comparative study on the mechanism of formation of pulse manifestations in coronary heart disease and hematopathic patients.

Most patients suffering from coronary heart disease have string--like pulse due mainly to disorder of cardiac function, lowered arterial compliance and increased total peripheral resistance. The common type of pulse in hematopathy patients is rapid, thready, string--like and slippery, due mainly to increased compensatory pumping action of heart, shortening of ejection time of left ventricle, relatively better vessel compliance and blood rheology as well as low total peripheral resistance.

[Bromodeoxyuridine immunohistochemical observation of sensory epithelium repair after ototoxic destruction in the crista ampullaris of guinea pigs].

In this in vivo study we investigated the proliferation of crista ampullaris in guinea pigs after Gentamicin intoxication by BrdU immunohistochemical observation and semi-thin sections. The experimental animals were given a single intraperitonial injection of BrdU at a dose of 150 mg/kg body weight before subsequently killed at 1, 3, 7, 14 and 22 day post-treatment (PT). Supporting cell proliferation was evident by BrdU immunoreactivity in the chromatin material of dividing cells at 7 and 14 day PT. The results demonstrated that the crista ampullaris of guinea pigs had repaired by supporting cell mitosis.

[Otoacoustic emissions and tinnitus].

Methods of objective tinnitus testing exploration causes of tinnitus and the relationship between OAE and tinnitus are reported. In 306 ears with tinnitus (with or without hearing loss), Distortion Products Otoacoustic Emission (DPOAE), Spontaneous OAE (SOAE) and Transiently Evoked OAE (TEOAE) were tested by ILO92 Otodynamics Analyzer. Results indicated that 1. In these cases there was no correlation between the frequency of tinnitus and SOAE. 2. In 94.8% of sensorineural hearing loss with tinnitus the DPOAE-gram presented lower amplitude or was absent within the frequency range of elevated pure-tone-thresthold; In 59% of cases with normal hearing and tinnitus the amplitude of DPOAE at nearby frequencies of tinnitus was decreased and there was no SOAE detectable. At frequencies other than that of tinnitus, the amplitude of DPOAE was normal and SOAE could be recorded. So OAE could reflect cochlear lesion in early stage.

Crystal structure of (L-Arg)-B0 bovine insulin at 0.21 nm resolution.

The crystal structure of (L-Arg)-B0 bovine insulin has been determined, using data to 0.21 nm and atomic parameters of 2Zn porcine insulin as a starting model, by the difference. Fourier method, the restrained least square method and X-PLOR package, interspersed with careful review of the electron density, to a final R-factor of 0.182 and r.m.s. deviation of 0.002 2 nm for the bond lengths and 4.3 degrees for the bond angles. The electron densities of additional (L-Arg)-B0 residues to B-chain N-terminus of two monomers in each asymmetric unit are very clear. The crystallographic micro-environment of the N-terminus of the B-chain is different from that of rhombohedral 2-zinc insulin.

[Advance of gene therapy for neurological diseases by direct in vivo gene transfer].

Gene therapy for neurological diseases is a rapidly expanding field in neurosciences. It has been demonstrated that some viral vectors from HSV-1 (herpes simplex virus type 1), Ad (adenovirus) and AAV (adeno-associated virus) can transfer foreign genes into nondividing cell types (including neurons). In addition, physical/chemical methods are also used in direct in vivo gene transfer. The direct gene transfer techniques will open a new way to study neurophysiology, neuropathology and therapy for neurological diseases at molecular level. They will hopefully lead to surprising progress in gene therapy for neurological diseases.

Refined solution structure and dynamics of the DNA-binding domain of the heat shock factor from Kluyveromyces lactis.

The solution structure of the 92 residue (11 kDa) winged helix-turn-helix DNA-binding domain from the kluyveromyces lactis heat shock factor was refined using a total of 932 NOE, 35 phi, 25 chi 1, 5 chi 2 and 44 hydrogen bond restraints. The overall root-mean-square deviation for structured regions was 0.75(+/- 0.15) A. The three-helix bundle and four-stranded beta-sheet are well defined with rmsd of 0.53(+/- 0.10) A and 0.60(+/- 0.17) A, respectively. Helix H2 is underwound and bent near Pro45. The angle between helix H2 and the proposed recognition helix H3 is 96(+/- 6) degrees. Detailed comparisons are made with the X-ray structure of this protein as well as other structural studies on HSF. Overall, the results are consistent with the earlier studies. Differences are related to protein-protein interactions in the crystal and dynamics in solution. Backbone dynamics was investigated via 15N relaxation. The average R1, R2 and NOE values for residues in segments of secondary structure were 1.9(+/- 0.9) s-1, 7.8(+/- 0.9) s-1 and 0.81(+/- 0.05), respectively. The correlation time based on these data was 5.6(+/- 0.4) ns. Motional order parameters were calculated by fitting the relaxation data to one of three models. Low-order parameters were found for residues that comprise the turn between helices H2 and H3 (residues Lys49 to Phe53), and most strikingly, the 16 residue wing (residues Val68 to Arg83). These data are consistent with the lack of long-range NOEs identified in these regions. The data provide a basis for comparison with results of the protein-DNA complex. The relationship between structure and function is discussed.

Anti-B4-blocked ricin synergizes with doxorubicin and etoposide on multidrug-resistant and drug-sensitive tumors.

Anti-B4-blocked ricin (anti-B4-bR) is an immunotoxin directed against CD19-positive cells that is currently being tested in several B-cell leukemia/lymphoma clinical trials. To explore the possibility of using anti-B4-bR in combination with chemotherapy protocols, we investigated the in vitro and in vivo cytotoxic effects of combining it with doxorubicin or etoposide using the lymphoma cell line Namalwa and a P-glycoprotein-expressing cell line, Namalwa/mdr-1, obtained by retroviral infection of Namalwa cells with the mdr-1 gene. Namalwa/mdr-1 cells were slightly more sensitive to anti-B4-bR than Namalwa cells; IC37 values were approximately 4 pmol/L and 8 pmol/L, respectively. When anti-B4-bR was combined simultaneously with doxorubicin or etoposide, additive to supra-additive killing of Namalwa and Namalwa/mdr-1 cells was observed. In xenografts of Namalwa/mdr-1 cells in severe combined immunodeficiency (SCID) mice, doxorubicin and etoposide at their maximum tolerated doses (3 mg/kg x 3 or 15 mg/kg x 3) showed no therapeutic effect. However, treatment with 5 daily bolus injections of anti-B4-bR (50 micrograms/kg) followed by treatment with doxorubicin or etoposide significantly increased the life span of the mice by 129% and 115%, respectively. After treatment with anti-B4-bR, the Namalwa/mdr-1 population expressed lower levels of P-glycoprotein, and this decrease may account for the synergistic action of the drug combinations. These results suggest that anti-B4-bR could be used to good effect in combination with current treatment regimens and further hint at a promising role for this immunotoxin in treatment of disease at the minimal residual disease stage, where cells may be resistant to chemotherapy.

Egr-1 negatively regulates human tumor cell growth via the DNA-binding domain.

Human HT1080 fibrosarcoma cells, subclone H4, express little or no Egr-1 (Zif/268, Krox 24), an early growth response gene encoding a transcription factor. Phorbol ester (but not serum) treatment only can elicit a small increase in Egr-1 expression in H4, in contrast to the normally rapid, high transient expression of Egr-1 observed after the addition of a wide range of stimulating agents to normal or immortalized cell lines. Because several human tumor cell lines express little Egr-1, we tested the hypothesis that this loss was causal to transformation. We report here that the expression of exogenous mouse Egr-1 in H4 cells inhibits transformed growth in a dose-dependent manner and significantly suppresses tumorigenicity in athymic mice. By overexpression of the fragment in Egr-1 that is responsible for its DNA-binding activity, the zinc-finger domain, we show that this domain has a similar activity. Moreover, the expression of antisense mRNA encoding the DNA-binding domain increases the transformed character of the H4 cells. One possible conclusion is that endogenous Egr-1-like genes perform growth-regulatory functions. Other human tumor lines are also growth suppressed by Egr-1 overexpression including ZR-75-1 breast carcinoma, U251 glioblastoma, and to a lesser extent, SAOS-2 osteosarcoma cells. These results are surprising in light of the "early growth response" character of Egr-1 but extend our earlier report of suppression of growth in v-sis-transformed NIH3T3 cells.
