Influence of type of feeding on lymphocyte function and development of infantile allergy.

Symptom scores indicating infantile allergy were recorded longitudinally in a group of infants during the first year of life. The symptom scores were related to duration of breast-feeding. Lymphocyte function, measured as the proliferative responsiveness to PHA-stimulation, was related to feeding with breast-milk or cow's milk. Babies with a short duration of breast-feeding; i.e. 1 month or less had significantly higher symptom scores at the ages of 1 month (P less than 0.05), 3 months (P less than 0.05), 6 months (P less than 0.001) and 12 months (P less than 0.05), than babies who were breast-fed for more than 1 month. The lymphocyte responsiveness to PHA was significantly higher at 3 and 6 months in babies who were fed cow's milk compared with those who were solely or partially breast-fed (P less than 0.05 and P less than 0.01 respectively). There was no correlation between symptom scores and lymphocyte responsiveness. It is concluded that infantile allergy is more common in babies with a short duration of breast-feeding and that feeding with cow's milk, compared to breast-feeding, causes a changed lymphocyte reactivity to PHA, which latter finding indicates an altered T-cell function between the two feeding groups.

Implantational growth of adenocarcinoma after Kraske operation for rectal tubular adenoma.

A 55-year-old man underwent an operation for a rectal tubular adenoma 1.5 cm in diameter by posterior proctotomy (Kraske operation). Later two rectal tubular adenomas were excised by electrocoagulation. Eight years after the primary operation, a 12 x 8 x 5 cm large mucinous adenocarcinoma was discovered inside the gluteus maximus muscle extending under the subcutaneous region of the primary incisional scar. No other metastases could be detected. It is probable that implantation of malignant or potentially malignant cells during the first operation was responsible for the adenocarcinoma.

Oxatomide in seasonal rhinoconjunctivitis.

The new antiallergic drug oxatomide was evaluated in 40 schoolchildren suffering from severe allergic seasonal rhinoconjunctivitis. Antihistamine drugs had earlier given insufficient symptomatic relief. Double-blind technique was used. There was no significant difference in effect between oxatomide, the antihistamine cinnarizine in combination with phenylpropanolamine and placebo. The results do not support the hypothesis that oxatomide is superior to antihistamine drugs in the treatment of severe seasonal rhinoconjunctivitis.

Immunohistochemical identification of lysozyme in intestinal lesions in ulcerative colitis and Crohn's disease.

Lysozyme (LZM) was identified in ulcerative colitis in granulocytes, monocytes, and macrophages of the intestinal lamina propria. In contrast with findings in normal colon or rectum, in ulcerative colitis LZM was also detected in some mucosal crypt cells and metaplastic Paneth cells. In both ulcerative colitis and Crohn's disease LZM was present in inflammatory cells of crypt abscesses. In Crohn's disease intense LZM staining was seen in epitheloid cell granulomas. The present observations permit one explanation for the raised concentration of serum-LZM in patients with ulcerative colitis and Crohn's disease.

Rubber band ligation of haemorrhoids. Analysis of 281 patients.

The problems and discomfort resulting from haemorrhoidectomy have led to the development of other forms of therapy. Rubber band ligation has become a popular method of treatment for internal piles. The results are comparable with those achieved by excisional surgery. The material consists of 281 consecutive patients with internal haemorrhoids treated with rubber band ligation. The average follow-up was 12 months. 239 of the 262 patients re-examined were entirely free of discomfort (91%). In all the eight cases with mucosal prolapse the ligation was not successful. In these as well as in patients with rubber allergy, other forms of treatment are suggested.
