HrpA, a new ribosome-associated protein which appears in heat-stressed Mycobacterium bovis bacillus Calmette-Guérin.

A novel 18-kDa heat shock protein, HrpA, has been identified from Mycobacterium bovis BCG. HrpA was rapidly synthesized in membrane and ribosome fractions but not in the cytoplasmic fraction under heat shock stress. HrpA bound tightly to 70S ribosomes, mainly in 30S subunits. HrpA might be involved in the initiation step of translation at high temperature.

Five cases of measles secondary vaccine failure with confirmed seroconversion after live measles vaccination.

We report 5 patients with secondary vaccine failure (SVF) who were infected with natural measles 2, 5, 5, 7 and 12 years, respectively, after vaccination with further attenuated live measles vaccine during infancy. Their seroconversion had been confirmed after vaccination. Three of the 5 patients had mild (modified) measles, while the remaining 2 patients had typical measles. The hemagglutination inhibition antibody titers to measles virus in paired acute and convalescent sera showed a secondary response pattern in 4/5 patients, and a primary response pattern was present in the remaining patient. Measles IgM antibodies were present in all patients during the convalescent stage. The patient with the primary response pattern may have had a decrease in the B cell memory during the 5-year period between vaccination and infection. This may be the first SVF case report that confirms the existence of completely waning immunity in recipients of the further attenuated live measles vaccines.

[Tc-99m labeled leukocyte scintigraphy and CT for the evaluation of patients with inflammatory bowel disease].

Seventeen cases (11 patients) of inflammatory bowel disease (IBD) were studied to define the intensity and extent of disease by 99mTc-HMPAO-labeled leukocytes scintigraphy (TLLS), and 10 cases underwent CT examination to evaluate the bowel wall, lymph-nodes, and mesenteric surroundings. Serial TLLS were obtained up to 4 hours and CT was carried out within one week before or after TLLS. The sensitivities of early (1 hour) and delayed (4 hours) TLLS were 91% and 100%, respectively. The respective specificities were 100% and 33%. However, it appeared that mild IBD may yield false negative results in early TLLS while non-specific bowel activity and migration of white cells may cause false positive results in delayed imaging. By setting the diagnostic criteria for labeled leukocyte accumulation on visualization of the small bowel regardless of uptake or activity of the large bowel similar to or greater than lumbar bone marrow, the sensitivity and specificity of delayed TLLS changed to 91% and 83%, respectively. On CT examination, mesenteric lymph-node swelling, periintestinal blurring and dilatation of mesenteric vasa recta were observed in all five patients with active Crohn's disease, while wall thickening and enhancement were seen in four of them. None of the other three cases of inflammatory disease showed positive findings of dilatation of the mesenteric vasa recta, and they revealed relatively low uptake of labeled leukocytes in TLLS. A "scintigram score" was calculated by comparing uptake of tracer in five bowel segments with lumbar bone marrow activity, and a "CT score" was calculated by adding abnormalities of the intestine and mesenteric surroundings. The scintigram score correlated closely with CT score and clinical disease activity. Combined use of CT and TLLS for the evaluation of patients with IBD was an excellent means to obtain the findings of intensity and extent of disease of the bowel wall and mesenteric surroundings and provided useful information in determining the patient management.

[Evaluation of clinical utility of 111In-DTPA-IgG scintigraphy in the detection of inflammation/infection--a report of multicenter phase III clinical trials].

This is the report of phase III study to evaluate the safety and utility of 111In-DTPA-IgG in patients with strongly suspected inflammation/infection. One hundred and forty five patients with suspected sites of inflammation/infection were enrolled in the study. Only a few adverse experiences in one patient were reported, which were interpreted as having a possible relationship to the agent. A total of 171 suspected sites (12 in head and neck, 39 in thorax, 44 in abdomen and pelvis, 62 in musculoskeletal system, and 14 in other regions) were evaluated by investigators at each institute. Out of 171 sites, 18 were determined to be unevaluable, and 12 false negative and 5 false positive cases were observed. Overall sensitivity and specificity was 89.8% and 85.7%, respectively, and the agent proved to be effective in detecting lesions anywhere throughout the body. The analysis of data from this Phase III study indicates that 111In-DTPA-IgG is well-tolerated in patients and effective in determining focal sites of inflammation/infection although the physiological accumulation in some tissues such as the sinus or liver and possible excretion into the gastrointestinal tract may make it difficult to localize lesions.

Transposon tagging in rice.

To develop an efficient gene isolation method for rice we introduced the maize Ac/Ds system into rice. Extensive analysis of their behavior in rice for several generations indicated that Ac and Ds in the presence of Ac transposase gene actively transpose in rice. A wide spectrum of mutations affecting growth, morphogenesis, flowering time and disease resistance have been obtained in the population carrying Ac/Ds and some of them were genetically analyzed. Main efforts are currently being made to isolate genes responsible these mutations. In addition, a number of Ac/Ds were mapped on chromosomes and mapped elements will be used in the future for directed tagging of genes with known chromosomal positions.

[Can 99mTc-mercaptoacetyltriglycine (99mTc-MAG3) evaluate the renal function without blood sampling?: consensus report from multicenter study].

A multicenter study was undertaken in Japan to evaluate the correlation between the percentage of renal uptake of 99mTc-mercaptoacetyltriglycine (99mTc-MAG3) estimated by the count-based gamma camera method and the blood clearance of 99mTc-MAG3. Twenty four centers were enrolled and 172 cases were finally analyzed in this study. The renal clearance of 99mTc-MAG3 (TER) was obtained by using a single blood sample taken at 44 min after injection. Comparison of TER and renal uptake provided a coefficient of correlation of 0.874; suggesting that sufficiently accurate quantification of renal function could be obtained from the renal uptake estimate by the gamma camera method. This study also showed that the comparison of renal function might be feasible among patients under the same protocols, although precise and careful consideration is required in each center.

Specific binding of allergenic soybean protein Gly m Bd 30K with alpha'- and alpha-subunits of conglycinin in soy milk.

When defatted soy milk was ultracentrifuged, 34kDa allergenic soyabean protein Gly m Bd 30 K was more abundant in the precipitate than in the supernatant by an SDS-PAGE analysis. The addition of more than 10 mM of 2-mercaptoethanol (2-ME) to the soy milk resulted not only in further removal of the 34kDa allergenic protein to the precipitate, but also in better recovery of conglycinin in the supernatant. After a two-dimensional SDS-PAGE analysis (the first dimension, minus 2-ME; the second, plus 2-ME) of the precipitates, superimposition between the CBB-stained gel and the eletroblotted membrane stained with a monoclonal antibody specific to Gly m Bd 30 K indicated that part of Gly m Bd 30 K was preferentially bound to the alpha'- and alpha-subunits of conglycinin, and that part of them had formed the dimer through a disulfide bond.

Herpes zoster in children with bone marrow transplantation: Report from a single institution.

Herpes zoster (HZ) has been often observed after bone marrow transplantation (BMT) in childhood. The occurrence of HZ was reviewed in children who received BMT. The clinical features of HZ were reviewed in 44 children who underwent BMT at Kyushu Cancer Center. Among the 35 recipients with a history of varicella before BMT, several factors associated with BMT and the lymphocyte subsets were compared between the patients who developed HZ (HZ+ group) and those who did not (HZ- group). Twenty-two recipients (50%) developed HZ; in two-thirds of these cases (15/22: 68%), HZ occurred between 80 and 120 days after BMT (median 101 days). The recipients treated with busulfan had a higher occurrence of HZ than those treated without it. The patients with Grade II-IV acute graft-versus-host disease (GVHD) developed HZ more frequently. In the HZ+ group, the absolute number of lymphocytes, CD3+, CD4+ or CD8+ cells at 3 months was significantly lower than that observed at 12 months after BMT and the CD4/CD8 ratio was significantly lower at 1 month than after 3 months of BMT. In conclusion, recipients were susceptible to HZ at around 100 days after BMT. The development of HZ may be associated with unbalanced T lymphocytes at that time.

Long-term sequential changes of antibody to p40tax in HTLV-I carrier mothers and children.

Antibody to p40tax (anti-p40tax) in serum specimens obtained sequentially from a human T cell lymphotropic virus type I carrier population of mothers and children were assayed. The prevalences of anti-p40tax at the initial sampling were 88% (7/8) in children and 55% (16/29) in mothers. Two of the seven positive children lost their anti-p40tax during the investigation period, resulting in a final prevalence of 63% (5/8) in children. However, anti-p40tax status was constant in all the 22 mothers with multiple serum samples (15 remained positive and seven remained negative). A decline in the absorbance value of EIA for anti-p40tax was observed in seven of the 15 anti-p40tax positive mothers. This decline may result in the disappearance of anti-p40tax in some of them.

Galactosylated proteins are recognized by the liver according to the surface density of galactose moieties.

The recognition of 111In-labeled galactosylated superoxide dismutase (Gal-SOD) and galactosylated bovine serum albumin (Gal-BSA) by the liver was investigated in mice after intravenous injection. 111In-labeled galactosylated proteins were recovered in the liver by amounts that were highly dependent on the degree of galactose modification and the administered dose. The distribution patterns were analyzed based on a physiological pharmacokinetic model including an uptake process with Michaelis-Menten kinetics in the liver and hepatic plasma flow. The Michaelis constant of hepatic uptake of 111In-Gal-SOD was observed to inversely correlate with the number of galactose residues, without a significant change in maximum rate of uptake or extrahepatic clearance. This relation could be applied to 111In-Gal-BSA and other galactosylated proteins by using the surface density of galactose residues as a degree of modification, suggesting galactose density controls ligand recognition by the asialoglycoprotein receptor. The analysis also indicated that increasing galactose density higher than 1.0 x 10(-3) molecules/A2 did not affect the distribution of galactosylated proteins due to limitation by the hepatic plasma flow rate. In conclusion, efficient delivery of proteins modified with galactose to the liver will be achieved by controlling both the galactose density on the protein surface and the administered dose.

[Leukocyte scintigraphy: clinical application to inflammatory diseases and bone marrow diseases].

111In or 99mTc-HMPAO labeled leukocyte scintigraphy has been shown to be useful in detection and diagnosis of infections and inflammatory lesions. Results from multiple retrospective series indicate that leukocyte scintigraphy achieves sensitivity and specificity of near 90% in abdominal abscess detection. 99mTc-HMPAO labeled leukocyte scintigraphy is now an accepted technique in the assessment of patients with inflammatory bowel disease and the diagnostic value of early 99mTc-HMPAO labeled leukocyte images has been discussed. It is known that 99mTc-HMPAO labeled leukocytes migrate to a large extent to the bone marrow. 99mTc-HMPAO labeled leukocytes may prove to be a practical agent for bone marrow imaging. In this report, definition, classification and pathological consideration of inflammation, and history, diagnostic efficacy and consideration for clinical application of inflammation scintigraphy are reviewed.

Intra-arterial infusion of N-isopropyl-p[123I]iodoamphetamine for assessing effective blood supply to pulmonary and hepatic neoplasms.

The biodistribution and pharmacokinetics of intra-arterially administered N-isopropyl-p[123I] iodoamphetamine (123I-IMP) were prospectively evaluated in 38 patients with histologically proven pulmonary or hepatic tumors. Intra-arterially infused 123I-IMP was distributed initially in peripheral tissues in which the blood supply was maintained. Its concentration in malignant neoplasms was demonstrated to be higher than in normal tissues. In pulmonary cancer, the tumor uptake of the administered dose without a tissue attenuation correction (% uptake) of 123I-IMP at 1-2 min after injection was 14.7 +/- 5.7% (s.d.). The tumor to normal tissue ratio was 2.1 +/- 0.7 in hepatocellular carcinoma and 1.4 +/- 0.7 in metastatic tumors. The biodistribution of 123I-IMP was also compared to that of 99mTc-macroaggregated albumin (99mTc-MAA) in 9 cases of hepatic cancer. The distribution of 123I-IMP resembled that of 99mTc-MAA in 5 cases and was different in 4 cases. 123I-IMP was more concentrated in the tumor than 99mTc-MAA. Intra-arterial infusion scintigraphy with 123I-IMP seems to provide information on effective blood supply to neoplasms which are targeted in interventional radiology.

Discordant uptake of Tc-99m DTPA-galactosyl human serum albumin and Tc-99m Sn colloid in a patient with severe acute hepatitis.

A patient with recently diagnosed severe acute hepatitis underwent serial liver scintigraphy with Tc-99m Sn colloid and Tc-99m DTPA-galactosyl human serum albumin. In initial studies, radionuclide distribution on Tc-99m DTPA-galactosyl human serum albumin scintigraphy was completely discrepant to that on Tc-99m Sn colloid scintigraphy. In a follow-up study 1 month later, the distribution of both radionuclides in the liver appeared relatively homogeneous. The uptake of Tc-99m DTPA-galactosyl human serum albumin and Tc-99m Sn colloid reflects the function of hepatocytes and Kupffer cells, respectively. Both kinds of scintigraphic study may be helpful to assess histopathologic change of different hepatic tissue architectures.
