Systolic time intervals in cardiac tamponade.

Systolic time intervals (STI) have been measured in three patients with cardiac tamponade. The left ventricular ejection time (LVET), the preejection period (PEP) and the ratio PEP/LVET deviated significantly from the normal values. All three parameters improved immediately after pericardiocentesis and aspiration. The total electromechanical systole changed to only a minor degree. Measurement of STI may be a valuable tool in the diagnosis and treatment of cardiac tamponade.

Fetal thymus transplantations in severe combined immunodeficiency.

Two brothers with severe combined immunodeficiency were treated with repeated transplantations of fetal thymus tissue. The first patient was not treated until he was critically ill, and the intramuscular transplants had no effect. He died at 11 months of age of overwhelming pneumonia. At postmortem examination a transplanted thymus seemed viable. In the second patient an intramuscular transplant had no effect, but three subsequent intraperitoneal transplants led to transient increase in circulating T lymphocytes with a concomitant fall in B lymphocytes. The results suggested an additive effect of each transplant. However, delayed hypersensitivity skin tests and in vitro mitogen responses were not influenced. Initially, transfer factor was given, and fetal liver was administered intraperitoneally together with the last thymic transplant. Neither of these measures had any observed effect, and this patient, similarly, died of pneumonia at nearly 12 months of age.

B-Lymphocytes

Acute disseminated phycomycosis in a patient with impaired neutrophil granulocyte function.

A 13-year-old girl with no previously known predisposing disease developed phycomycosis involving the left lung, pleura and shoulder, the left side of the neck, the left thigh, the kidneys and the brain. Prolonged amphotericin B therapy resulted in clinical improvement, but the disease was wide-spread when the patient died 5 months after debut of symptoms from a subarachnoid haemorrhage due to fungal destruction of the basilar artery. During hospitalization, a marked reduction in the bactericidal activity of circulating neutrophil granulocytes was repeatedly demonstrated and the endotoxin stimulated nitroblu tetrazolium test was negative. Together with the demonstration of granuloma formation and the accumulation of lipid-laden histiocytes in the spleen, lymph nodes, bone marrow and the thymus, these findings indicate that the patient had a less severe form of chronic granulomatous disease.

Influence of antibodies and thermolabile serum factors on the bactericidal activity of human neutrophil granulocytes.

The influence of serum natibodies and thermolabile serum factors on the intracellular killing of Staphylococcus aureus by neutrophil granulocytes has been examined using a method which facilitates a precise in vitro evaluation of the phagocytic and bactericidal activities of human polymorphonuclear leucocytes. The bactericidal activity of the granulocytes was significantly less pronounced in the presence of serum absorbed with Staph. aureus or inactivated at 56 degrees C for 30 minutes than in the presence of untreated serum. Specific antibodies seemed to stimulate the intracellular killing of bacteria more than thermolabile serum factors.

Influence of IgG, F(ab')2 and IgM on the phagocytic and bactericidal activities of human neutrophil granulocytes.

The phagocytosis and intracellular killing of Staphylococcus aureus by humans granulocytes in the presence of immunoglobulin preparations have been examined. Isolated IgG from pooled human serum induced phagocytosis and intracellular killing of bacteria. F(ab')2 fragments had no significant effect, indicating that the Fc piece of the IgG molecule is of importance not only for the phagocytosis but also for the intracellular killing of bacteria. Isolated IgM stimulated the phagocytosis to a minor extent, with no enhancement of the bactericidal activity.

Blood Bactericidal Activity

Neutrophil dysfunction and granulomatosis in the preleukemic state.

A 40-year-old male had periods of fever, sore throat and anemia for 14 months before acute myeloblastic leukemia could be diagnosed from hematological findings. During the preleukemic state, impaired bactericidal capacity of the granulocytes was repeatedly demonstrated and multiple hepatosplenic and skin granulomas occurred. Results of granulocyte function studies may prove to be of significant aid in the diagnosis of the preleukemic state of acute myeloblastic leukemia.

Cholera

Neutrophil granulocyte function in the early diagnosis of acute myelomonocytic and myeloblastic leukaemia.

The phagocytic and bactericidal activities of neutrophil granulocytes from 5 patients with early acute myelomonocytic or myeloblastic leukaemia and 5 controls have been examined. In each patient the bactericidal activity was lower than in any control and the neutrophil dysfunction was demonstrated before leukaemia could be diagnosed from clinical and haematological findings. During periods of remission, the bactericidal activity was normal. Results of neutrophil granulocyte function studies may be a significant aid in the early diagnosis of acute myelomonocytic and myeloblastic leukaemia.

Influence of therapeutic concentrations of phenylbutazone on granulocyte function.

The influence of therapeutic concentrations of phenylbutazone on human granulocyte function has been examined using a method which facilitates a precise in vitro evaluation of the phagocytic and bactericidal activities of polymorphonuclear leucocytes. Phenylbutazone caused a marked reduction in intracellular killing of bacteria by the granulocytes. Whether this inhibition of granulocyte function also takes place in vivo resulting in enhanced susceptibility to infection, remains unknown.
