Indications on suitable scaffold as carrier of stem cells in the alveoloplasty of cleft palate.

Autologous iliac crest bone is used to close the residual alveolar bone defect in cleft palate patients during late mixed dentition. Surgery involves physical and anaesthesiologic risks, long-time hospitalization, high costs and not always good results (15% failure rate). Alternatives to iliac crest bone grafting are going to be evaluated: synthetic, xenograft and allograft matrices combined with platelet-rich plasma or recombined bone morphogenic proteins for osteoinductivity are commercially available. These alternatives have not yet been determined to be equivalent to the previous treatment. A new field of research is represented by stem cells, which have been also used to regenerate ischaemic cardiac tissue after heart attack, to treat hypophosphatasia and osteoporosis. Our aim was to use osteoblasts from stem cells to close the residual palate cleft in association with a suitable carrier. Stem cells are expanded in the Aastrom bioreactor, differentiated into osteoblasts and positioned in the bone defect by means of a Spongostan scaffold. This scaffold has the best characteristics as commercial availability, low cost, good manageability, absence of allergic reactions or other side effects on patient, biocompatibility, imbibition, radiotransparency, reabsorbability and osteoinductivity. Previous studies encourage Spongostan scaffold application.

Immunolocalization of heat shock protein 27 in developing jaw bones and tooth germs of human fetuses.

27 kDa Heat shock protein (Hsp27), which is also identified as p29 estrogen-receptor associated protein, plays a crucial role in specific growth stages. It also seems to be involved in the balance between differentiation and apoptosis. To determine whether Hsp27 is involved during craniofacial development and odontogenesis, its expression was studied through immunohistochemistry of developing jaw bone as well as the odontogenesis of heads from human fetuses. Formalin-fixed paraffin-embedded specimens of 7 human fetuses (3 female, 4 male), obtained from miscarriages occurring between the 9th and 16th weeks of pregnancy, were examined by using a monoclonal antibody against Hsp27. Staining intensity (weak, +; moderate, ++; strong, +++) was evaluated semiquantitatively. The sample slice was cut through a coronal plane, which included eyes, nasal cavities, tongue, and primitive dental lamina with tooth germs. A transient and spatially restricted expression of Hsp27 in developing human jaw bones and teeth was observed. Osteoblasts around the uncalcified bone matrix showed Hsp27 immunoreaction products (+++), whereas osteocytes were not immunolabeled. In mandibular condyle, immunolabeling was restricted to hypertrophic chondrocytes (++). In developing tooth germs, Hsp27 immunostaining was detected throughout the bud (+++). At the early cap stage, a strong immunolabeling for Hsp27 was seen in the dental lamina (+++), and a moderate staining was seen in the outer dental epithelium (++). At the late cap stage, Hsp27 expression was detected in the outer dental epithelium (++) as well as in the cells of the future stellate reticulum (++). The spatiotemporal-restricted expression of Hsp27 in craniofacial bones during development suggests that this protein could be involved in the balance between differentiation and apoptosis, by modulating the viability of osteoblasts and chondrocytes. The specific regional and temporal expression patterns of Hsp27 during tooth development sustains that this small Hsp might be related to the morphogenesis and cytodifferentiation processes of tooth germs.

Mast cells and the inflammatory response to different implanted biomaterials.

The biocompatibility of dental materials is a topic of increasing importance for dentists. We investigated the effects of alloys for ceramic crowns and removable prostheses on the skin and lymphoid organs of rats. We used three groups of Sprague Dawley rats: group I as the control, with groups II and III implanted with different biomaterials into subcutaneous pockets. After sacrifice, the rat skin around the implant and the lymph nodes was studied. The tissues of implanted rats were morphologically different from the control animals as they showed a number of mast cells that increased 7 days after the implant with both alloys. These cells decreased 14 days after the implant with alloy for ceramic crowns whereas they were still high 28 days after the implant with alloy for removable prostheses. The findings suggest that both alloys induced different and time-dependent inflammatory reactions. Our findings have importance in the future rational design of biocompatible implantable and blood-contact biomaterials.

Undergraduate orthodontic & paediatric dentistry education in Europe--the DentEd project.

As a result of a European Union funded project (DentEd), a programme of visits to dental schools throughout Europe has been underway since 1998. This report describes the philosophy behind DentEd, gives a brief description of the features of a visitation, and covers the orthodontic and paediatric dentistry teaching as reported in 26 different dental schools in 16 different countries. It is based on a report submitted to DentEd from a small working group that looked at various aspects of educational provision within the two disciplines across Europe. The value of this information to teachers within the two disciplines and to the wider dental community is briefly discussed. The report recommends the adoption of an integrated course for orthodontics and paediatric dentistry. The main objectives are that the student should be able to understand orofacial and psychosocial growth and development of the child, recognize aberrant growth and development, and manage the behaviour of the child, their straightforward preventive, restorative and occlusal needs, and to make appropriate and timely referral.

Persistent ST segment elevation in a patient with metastatic involvement of the heart.

Electrocardiographic abnormalities are commonly seen with tumor invasion of the heart, but usually these abnormalities are not specific. Pronounced and prolonged lateral ST segment elevation in the absence of myocardial infarction occurred in a patient with epidermoid carcinoma of the left lung. Computer tomography showed the presence of tumor invasion of the heart. Prolonged ST segment elevation in the absence of Q waves seems to be a pathognomonic sign for tumor invasion of the heart.

Evaluation of environmental bacterial contamination and procedures to control cross infection in a sample of Italian dental surgeries.

OBJECTIVES: To perform a pilot study on bacterial contamination in some dental surgeries (n=51) in a local health unit in Brescia (Lombardy Region, Italy) and to evaluate the procedures to control cross infection used by the personnel to reduce the risk of infection in dental practice. METHODS: A survey was carried out by interviewing 133 dental personnel with a questionnaire on the procedures used to control infection. The autoclaves, chemical baths (chemiclaves), and ovens present in the surgeries were tested for sterilisation efficiency with a spore test, and already packed and sterilised instruments were randomly sampled and tested for sterility. Microbial contamination of air, surface, and dental unit water samples were also studied. RESULTS: The dental personnel did not generally follow the principal procedures for infection control: 30% of personnel were not vaccinated against hepatitis B virus, infected instruments were often not decontaminated, periodic checks of autoclave efficiency were lacking, and the knowledge of disinfection mechanisms and procedures was incomplete. High bacteriological contamination of water at dental surgeries was often found and total bacteriological counts in air samples were high. Surface studies showed widespread bacterial contamination. CONCLUSIONS: On the basis of these results, an educational programme for the prevention of infective hazards has been prepared and carried out. The results of this pilot study will be used for planning a national survey.

A histological procedure to determine dental age.

Sclerosis of dentine has become one of the well established indicators of ageing and age determination. In this study a new technique was reported where a photomicrographic image of a cross section of sclerotic dentine was converted to a grey scale of 256 tones and then reduced to black and white and read by computer using specially developed software. A regression analysis was applied to a sample of 62 teeth (age range 17-84 years) and an age determination within an error limit of 11 years was obtained. Using a Neural Network software however the error was reduced to 8 years.

Age determination by teeth examination: a comparison between different morphologic and quantitative analyses.

Age determination by teeth examination is one of the main means of determining personal identification. Current studies have suggested different techniques for determining the age of a subject by means of the analysis of microscopic and macroscopic structural modifications of the tooth with ageing. The histological approach is useful among the various methodologies utilized for this purpose. It is still unclear as to what is the best technique, as almost all the authors suggest the use of the approach they themselves have tested. In the present study, age determination by means of microscopic techniques has been based on the quantitative analysis of three parameters, all well recognized in specialized literature: 1. dentinal tubules density/sclerosis 2. tooth translucency 3. analysis of the cementum thickness. After a description of the three methodologies (with automatic image processing of the dentinal sclerosis utilizing an appropriate computer program developed by the authors) the results obtained on cases using the three different approaches are presented, and the merits and failings of each technique are identified with the intention of identifying the one offering the least degree of error in age determination.

Induction and pharmacological treatment of oral osteopenia in rats.

BACKGROUND: Osteoporosis is a major public health problem, responsible for a great number of fractures, associated with devastating costs to society. In addition, oral bone loss has an enormous impact on the health quality of life of patients, affecting up to 90% of elderly individuals. The aim of this work was to elaborate an animal model of mandibular and maxillary osteoporosis in which to evaluate bone loss and possible prevention by pharmacological treatment. METHODS: Six Sprague-Dawley rats were gonadectomized and treated with clodronate (male) or 17 beta-estradiol (female) for two months. Six gonadectomized and six sham-operated rats of both sexes were treated with placebo. The mandible and maxilla, fixed and methacrylate embedded, were serially sectioned, microradiographed and processed for histomorphometric analysis. RESULTS: Gonadectomy did not modify the amount of compact and trabecular bone in mandibles of rats of either sex, treated or not with clodronate or estrogens, compared to sham-operated rats. Compared to sham-operated rats, a 10-25% increase of bone porosity was found in the maxilla of ovariectomized rats, either receiving estrogens or not, while in male rats no difference among groups could be evidenced. CONCLUSIONS: The conclusion is drawn that rats, due to their peculiar masticatory habits yielding huge loads on oral bones, do not represent a suitable experimental model for studying oral bone loss related to skeletal osteoporosis. In order to worsen oral osteopenia it would be mandatory to combine gonadectomy with a mechanical unloading (i.e. after molar extraction) of mandibular or maxillary bone.

[Lymphadenopathy as specific ultrasonography index of subacute thyroiditis. Preliminary data].

INTRODUCTION: We report the preliminary results of a study on the importance of juxtajugular and supraisthmian lymph node enlargement at US as a sign of occult subacute thyroiditis. MATERIAL AND METHODS: We examined patients submitted to thyroid US for noninflammatory thyroid conditions and for positive familiarity and studied the presence of supraisthmian and juxtajugular lymph node enlargement. The patients were then divided into two groups: the clinical history, antibody titer and symptoms were studied in group A (patients with enlarged lymph nodes) and the antibody titer, symptoms and US findings were studied in group B (patients with a history of thyroiditis). RESULTS: Fifty-nine of 600 patients were allocated to group A and 25 of 600 to group B. Of the former 59 asymptomatic patients (10% of the total), 29% had supraisthmian and 85% juxtajugular lymph node enlargement, with some patients positive at both sites. Forty-seven patients (80%) had subacute thyroiditis (33 of them with positive antibodies and 23 with a clinical history); we had 12 false positives. Twenty-two of 25 group B patients (91%) had supraisthmian (40%) or juxtajugular (83%) lymph node enlargement: 3 patients with no enlarged lymph nodes had an over 3-year history of low antibody titers. DISCUSSION: Forty-seven of 600 (8%) asymptomatic patients who had been never examined are or were affected with subacute thyroiditis. Ninety-one per cent of all thyroiditis patients had supraisthmian or juxtajugular lymph node enlargement, more frequently the latter than the former. CONCLUSIONS: Supraisthmian and/or juxtajugular lymph node enlargement appears to be a very useful US sign of subacute thyroiditis, even though these data need confirmation from larger series of patients.

[Pathology of taste: knowledge and prospectives].

Taste anomalies have not been systematically investigated till recently in Italy. The influence of the taste on the choice of the foods, on the nutrition and on the same psycho-physic human mindedness has largely been demonstrated. In this study, taste anomalies associated with various pathologic conditions (nervous, endocrine, metabolic, infectious, immunological, carential or neoplastic disorders) are reported, as well as those produced by drugs, or surgical interventions. The type and the degree of taste anomalies associated to immunological diseases, allogeneic bone marrow transplantation and AL amyloidosis are particularly described.

Endoscopic and histological criteria for preoperative evaluation of the depth of infiltration of gastric carcinoma.

Because the therapeutic approach to gastric carcinoma differs according to the stage of development, a study was carried out to investigate whether there are any factors which would allow the depth of infiltration of a gastric carcinoma to be evaluated preoperatively. The criteria used were endoscopic and histological. The first provide information on size and macroscopic aspect; the second reveal the relationship between bioptic specimens that are positive for carcinoma and those that are negative. On the basis of these standards, small, benign-looking neoplasias with bioptic positivity for carcinoma lower than 30% were classified as probably early. Of the 200 gastric carcinomas investigated, 55 were considered probably early and 145 probably advanced. Surgical fragment findings confirmed the diagnosis in 169 cases (37 true negatives and 132 true positives) and failed to confirm it in the other 31 cases (13 false positives and 18 false negatives). The specificity, sensitivity, negative predictive value, positive predictive value, and total diagnostic accuracy indices were 74.00%, 88.00%, 67.27%, 91.03%, and 84.50%.

[Criteria for the endoscopic assessment of hemorrhage risk in esophageal varices].

The natural history of liver cirrhosis shows one third of patients bleeding from esophageal varices. The first episode of bleeding has a 40% mortality rate and 70% of survivors will have another haemorrhage within a year. To overcome this dramatic sequence the following types of prophylactic treatment have been attempted: portocaval shunt, B-blockers, endoscopic sclerotherapy. Medical Literature shows no proven benefits from these procedures. Endoscopic sclerotherapy seems to have the best results. Thus, to be more successful we suggest a better selection of patients to undergo prophylactic sclerotherapy, also improving the prognostic criteria which could predict the bleeding. Esophagoscopy makes it possible to examine some of the predictive signs of impending haemorrhage. However, both the endoscopic and clinical criteria (Child) give a better evaluation of the risk of bleeding.

[Pharmacological support during orthodontic therapy with a topical anti-inflammatory].

On the occasion of many orthodontic procedures is often necessary to use NSAIDs in order to provide relief from the inflammatory and painful symptoms originating by removable and permanent devices. In this single blind controlled study, efficacy and tolerability of a topical antiinflammatory drug, formulated as mouth wash containing flurbiprofen 0.25%, have been evaluated in 60 patients (range 12-16 years) suffering from tissular injuries caused by permanent devices. Following a randomisation list, the patients were allocated to 3 different balanced groups, the first received flurbiprofen mouth wash 10 ml, 3 times daily for 7 days, the second received placebo mouth wash in the same way and the third group was a control one. The aim of the trial was to evaluate the pain symptom related to the injuries. The result of the study shows statistical improvement in favour of flurbiprofen in comparison to the placebo as well as to the control group. No local or systemic ADRs were reported.

[The pilonidal sinus: its surgical treatment, our experience and a review of the literature].

The authors report their experience in the surgical management of pilonidal sinus. During a 3-year period (1989-1991) 106 patients underwent primary closure. Average postoperative stay was 5.5 days, while mean healing time was 14.6 days. During the follow up, 3 patients developed a true recurrence (3.26%). After a critical review of the literature, the authors emphasize the importance of primary closure in the surgical treatment of pilonidal disease.

[Alkaline esophagitis following total gastrectomy].

Two cases of severe alkaline esophagitis following total gastrectomy, respectively for a gastric sarcoma and a gastric carcinoma, are reported. In the first case digestive continuity was assured through an Omega loop esophagojejunostomy, while in the second one an esophagojejunostomy according to the Moricca technique was performed. Both patients underwent reoperation and a conversion in Roux-en-Y esophagojejunostomy resolved the reflux related problems. The Authors, therefore, confirm Roux-en-Y esophagojejunostomy as the procedure of choice in preventing and curing esophagitis, which can seriously affect the quality of life and nutritional status of gastrectomized patients.
