Planning the move of patient activities at a large medical center.

The planning for the move of inpatients and outpatients to a new 845-bed, 1.5 million square foot facility that took place over a period of two and one-half years and encompassed the health care, safety, and social needs of both inpatients and outpatients and their families is described. The length of time given to the planning process allowed the hospital personnel to undertake planning the move along with their regular duties and without the need for additional staff, plus allowed for very detailed planning. The precise estimate of time requirements for the inpatient move resulted in the move being completed within two minutes of the projected 150-minute estimate. Moving patients from one health care facility to another can be accomplished safely and efficiently with the aid of a well-developed plan and sufficient practice of all needed functions.

Heteroarylalkanoic acids with possible antiinflammatory activities. Part 8: The scavenging of the oxygen-free radicals.

The scavenging activity of some isosteric antiinflammatory heteoarylalkanoic acids with respect to oxygen-free radicals has been studied. All the compounds were found to inhibit the depolymerization of hyaluronic acid due to enzymatically or chemically generated hydroxyl radicals. The reduced consumption of oxygen by xanthine oxidase indicates a cotemporaneous phenomenon of enzymatic inhibition.

How do house officers spend their nights? A time study of internal medicine house staff on call.

Recommendations to limit the working hours of house staff are forcing directors of training programs to reevaluate how house officers spend their time. We studied how 35 house officers in internal medicine spent their on-call time in three teaching hospitals: an urban county hospital, a university hospital, and a regional Veterans Administration medical center. Trained observers accompanied each member of different on-call teams for five nights and quantified how their time was spent. Teams consisting of residents and interns admitted three new patients per night at the Veterans Administration hospital, six at the university hospital, and eight at the county hospital. Each house officer received 16 to 25 calls per night. Up to 12 percent of their time was spent doing procedures (such as inserting intravenous catheters or drawing blood specimens) most of which could have been done by nonphysicians. From 87 to 175 minutes of on-call time was spent in direct patient evaluation, and the mean time spent on each new-patient evaluation ranged from 17 to 31 minutes. The mean time before the evaluation was interrupted ranged from 7 to 11 minutes. In contrast, 66 to 197 minutes per night was spent documenting new-patient evaluations in the hospital record. The average sleep time ranged from 122 to 273 minutes; however, the mean time before sleep was interrupted ranged from 40 to 86 minutes. We conclude that while on call, house officers spend relatively little time in direct patient contact, but they spend considerable time charting. They are frequently interrupted while working and trying to sleep. These data may be useful in finding administrative ways to improve patient care and the experience of the house staff while on call and in evaluating the effect of reforms.

Heteroarylalkanoic acids with possible antiinflammatoric activities. Part 6: 3-(4H-1,2,4-benzothiadiazine-1,1-dioxide-3-yl-thio)-propanoic acid derivatives.

A series of 3-(4H-1,2,4-benzothiadiazine-1,1-dioxide-3-yl-thio)-propanoic acids was synthesized in order to study their possible antiinflammatory in comparison with the corresponding lower homologues and isosters previously studied. Most of the compounds proved effective against carrageenan-induced plantar oedema. The possible influence of some chemico-physical parameters on the occurrence of this biological activity was investigated.

Heteroarylalkanoic acids with possible antiinflammatory activities, Note IX. The binding to human serum albumin.

The interaction of some isosteric and homologues series of antiiflammatory heteroarylalkanoic acids with human serum albumin (HSA) was investigated by UV difference spectroscopy and fluorescence quenching. All tested compounds were shown to bind to HSA. The interaction with surfactants and the linear relationship between binding constants and pKa suggest that electrostatic force is dominant in the binding of these compounds and that the binding site consists of a cationic charge near an hydrophobic surface.

Heteroarylalkanoic acid with possible antiinflammatory activities. Note VII.

A series of 3-(4H-1,2,4-benzothiadiazine-1,1-dioxide-3-ylamino)-propanoic acids was synthesized in order to study their possible antiinflammatory activity and to compare it with the corresponding homologous or isoster series previously studied. Of particular interest is parent compound 1, which is unsubstituted in the benzenoid moiety and exerts a very marked inhibitory effect on carrageenan-induced plantar oedema greater than that observed for any of the terms of the other series hitherto studied. The possible influence of some chemico-physical parameters on the occurrence of this antiinflammatory activity is investigated.

Cardiovascular activity of 1,2,4-benzothiadiazine-1,1-dioxide derivatives. Part 10(3): Dialkylamino-, pyrrolidyl- and morpholyl-alkyl-derivatives of 3-amino-1,2,4-benzothiadiazine-1,1-dioxide, 7- or 5,7-halogen substituted.

A series of dialkylamino, pyrrolidyl and morpholyl-alkyl-derivatives of 3-amino-1,2,4-benzothiadiazine-1,1-dioxide, 7- or 5,7-halogen substituted in the benzenoid moiety, was prepared and their cardiovascular activity studied. Most of the substances in question were capable of inducing a significant decrease in mean arterial blood pressure; some of them tended to increase pulse pressure or to cause bradycardia, an effect which was noted particularly in the case of compound 10 which reduced heart-rate by 85%. The structure/activity relationship of these substances, of the 6- and 6,7-halogen substituted isomers and of their corresponding non-substituted progenitors is examined.

[Cardiovascular action of 1,2,4-benzothiadiazine-1,1-dioxide derivatives. IX].

A series of quaternary ammonium salts of 3-alkylaminoderivatives of 1,2,4-benzothiadiazine-1,1-dioxide were prepared and tested for cardiovascular activity. Almost all the substances tested proved to have some measure of depressant activity on arterial pressure and the most interesting compounds in this respect were (XII), (XIII), (XV) and (XIX) for which hypotensive values between 60 and 70% with respect to the base level were recorded. Compound (XV) also caused increase in the differential pressure and decrease of cardiac frequency. The later however was increased by some other compounds [(XII), (XVIII)]. The structure-activity relationships are discussed taking into account also the activity of the parent tertiary amines.

[Nuclear insulin assay after a starch and maltose load in chronic diseases of the pancreas: a preliminary note (author's transl)].

In a group of 48 patients with chronic pancreatitis the authors investigated the variations of blood insulin content after stimulation with starch and maltose, and compared these changes with the correspondind glycemia values. The two curves, practically matching in normal subjects, tend to diverge considerably in patients with chronic pancreatitis. According to the authors, since the assessment of exocrine pancreas function by common laboratory methods is none too satisfactory, this method may yield better appreciation of extant damage to the pancreas.

[Cardiovascular effect of 1,2,4-benzothiadiazine-1,1-dioxide derivatives. VIII].

A series of carboxy- and carbomethoxyalkyl derivatives of 3-amino-1,2,4-benzothiaidazin-1,1-dioxide either substituted or unsubstituted in the benzene ring [compounds (I leads to XVIII)] was prepared and tested for cardiovascular activity. It was found that the introduction of a carboxy-or carbalkoxy-group in the omega position of 3-alkylamino derivatives of 1,2,4-benzothiadiazine-1,1-dioxide usually causes marked decrease or abolition of the cardiovascular activity shown by the parent compounds. The negative effect on the pressor trace (hypotension and increase in differential pressure) is more frequent and significant than that on bradycardial activity.

[Hemorrhagic duodenal ulcer].

The authors report their experience in the treatment of hemorrhagic duodenal ulcers in 19 patients (17 men, 2 women) between 1972 and 1977. They compare the results obtained with the two methods adopted, namely gastric resection and local hemostasis of the ulcer followed by troncular or selective vagotomy, or selective proximal vagotomy and pyloroplasty. The results so analyzed speak definitely in favor of the latter procedures as opposed to gastrectomy.

[Our experience in the treatment of perforated duodenal ulcer].

The Authors considers 49 cases of perforated duodenal ulcers in free peritoneum, observed in 6 years from 1972 to 1977. They are quite exclusively males (9 : 1), aged from 22 to 80 years, observed from over 4 to 72 hours from effective perforation. After the bare necessities of their hydroelectrolitics balancing, 20 cases came to a simple suture, 29 to an ulcer excision, vagotomy and pyloric plastics. No post operative complications were noted in the two groups: mortality has been of 2 patients in the first group and of no one in the second; pathological ulcerous relaps have been of 47% in the first group and 0 in the second one, with from over 15 months to 6 years controls. By clinical, radiological and andoscopical examinations the AA, concludes that the best therapy treatment in perforated duodenal ulcers--except very particular cases--consists at the same time in the treatment of ulcerous pathology and its complications, with vagotomy completed by piloric plastics sec. Judd, and ulcer's excision.
