The dissemination of smoking cessation methods for pregnant women: achieving the year 2000 objectives.

The smoking prevalence rate among adult women and pregnant women has decreased only 0.3 to 0.5% per year since 1969. Without a nationwide dissemination of efficacious smoking cessation methods based on these trends, by the year 2000 the smoking prevalence among pregnant women will be approximately 18%. This estimate is well above the US Department of Health and Human Services Year 2000 Objective of 10%. The US dissemination of tested smoking cessation methods could help an additional 12,900 to 155,000 pregnant smokers annually and 600,000 to 1,481,000 cumulatively to quit smoking during the 1990s. Dissemination could help achieve 31 to 78% of the Year 2000 Objectives for pregnancy smoking prevalence. (With dissemination, at best a 15% smoking prevalence during pregnancy, rather than the 10% objective, is likely to be observed.) Our results confirm a well-documented need for a national campaign to disseminate smoking cessation methods.

[Experiences with laparoscopic cholecystectomy].

From January 1991 through October 1992, we performed our first 148 laparoscopic cholecystectomies with a morbidity rate of 4.22% (6 of 142 patients), and without mortality. There were 98 women and 50 men, with a mean age of 46 years. Six patients were converted to open surgery. There were no bile duct injury, no hemorrhage, and no bile leak. The median hospital stay was 5 days (range: 3 to 12 days) and the median time to return to full activity was 10 days. Laparoscopic cholecystectomy is safe and effective with minimal risk. Its advantages include shortened hospital stay. Early return to normal activity, diminished postoperative pain and decreased postoperative morbidity.

Efficient plasma membrane expression of a functional platelet glycoprotein Ib-IX complex requires the presence of its three subunits.

The glycoprotein (GP) Ib-IX complex of the platelet plasma membrane mediates the adhesion of platelets to damaged blood vessel wall. The complex is composed of three membrane-spanning polypeptides, GP Ib alpha, GP Ib beta, and GP IX, all of which are absent from the platelets of patients with the hereditary bleeding disorder Bernard-Soulier syndrome. In this study we report stable expression of the recombinant receptor in three cell lines and demonstrate that the three subunits of the complex are necessary for its efficient expression on the plasma membrane. The expressed complex associates with the cytoskeleton of the transfected cells through an interaction with actin-binding protein and binds its ligand, von Willebrand factor. These data suggest that the lack of plasma membrane GP Ib-IX complex in the Bernard-Soulier syndrome could potentially arise from mutations affecting any one of its three subunits.

Effects of cysteamine on flexion reflex facilitation by C-primary afferents in cats.

Brief (20 s) conditioning stimuli at C-fiber intensity applied to the sural or gastrocnemius nerve evoked a facilitation of the flexor alpha-motoneurone discharges from small filaments of the posterior biceps semitendinosus nerve produced by subcutaneous stimulation of the plantar region in anesthetized spinal cats. Such C afferent-induced facilitation was blocked reversibly by cysteamine (50 mg.kg-1), a somatostatin depletor, without affecting the basic mechanisms of flexor reflexes, suggesting an important role played by somatostatin in transmission and/or modulation of nociceptive information in the spinal cord.
