[Prospective study of HIV infection in a group of 30 young hemophiliacs].

In order to investigate the progression to AIDS, we studied 39 patients with hemophilia ranging presently in age from 8 to 20 years, who had been positive for HIV antibodies for a mean of 36 months. Every 3 to 6 months a clinical and laboratory examination was performed. The number of patients with AIDS increased from 2 to 6 and 2 patients died from opportunistic infections. The percentage of patients with less than 500 T4 cells increased from 28% to 41%. Increase in serum IgA concentrations above 2.5 gl/l ranged from 23% to 38% of patients. Seven subjects were found to be positive for HIV antigen and 11 of 15 had a loss of p24 antibody. Patients' distribution into two groups according to the results of T4 cells count at the beginning of the study have shown a significant difference on the basis of clinical and biological progression. The present investigation confirms the progression in this population and the importance of some markers.

Acquired Immunodeficiency Syndrome↗

[Nosocomial infection at a neonatal care unit].

The incidence of nosocomial infections in a neonatal intensive care unit was evaluated to be 11.5% over a 2 year-survey period (1984-1985) in 451 artificially ventilated newborns. Staphylococcus, Enterobacter and Pseudomonas were the most frequently identified germs. Low birth weight, gestational age, mode of delivery and type of the initial pathology were not found to be responsible for the superinfection. The duration of hospitalization was 3 times longer and the cost per infant largely increased in cases with infection. Antibiotic treatment at admittance seemed to delay the onset of nosocomial infection by 8 to 12 days in this population.

Anti-Bacterial Agents↗

[Initial antibacterial prophylaxis after bone marrow allograft. Pilot study with systemic vancomycin].

In 22 consecutive patients treated by allogenic bone marrow transplantation, the authors report their experience in complete gastrointestinal decontamination and prophylactic systemic vancomycin. Neither septicemia from the low digestive tract nor with Gram positive is noticed. A child developed septicemia with Capnocytophaga ochracea, resistant to vancomycin. There is no infectious death in this study and no significative toxicity is reported.

[Prevention of peritoneal adhesions by local treatments. Experimental study in the rat].

Those local treatments that are most used clinically for the prevention of adhesions in the pelvis have been evaluated experimentally using peritoneal abrasion in the rat. In a first experiment these products, namely dexamethasone, high molecular weight dextran, and noxytioline, were compared with a saline solution on the one hand which confirmed the effect of these substances in preventing adhesions, and on the other hand showed that noxytioline was about as effective as dextran 70 and better than corticosteroids. The score for adhesions that were found were; in the control series an average of 9.25, in the dexamethasone series 6, in the dextran 70 series 4.1 and in the noxytioline series 4.00. This last substance, which has been for less broadly studied than dextran, was then made the subject of a second experiment carried out in the same way as the first, and showing that the anti-adhesion effect was not something that was added on to its anti-bacterial effect because noxytioline alone was more effective than a broad spectrum antibiotic by itself, and adding the two together gave a still better result. It would seem to us that using noxytioline and dextran 70 together does not improve the results achieved by using each substance by itself.

[Haemorrhages of the posterior fossa in full-term newborns (author's transl)].

Four cases of non-traumatic haemorrhage of the posterior fossa in full-term newborns are reported and compared with the 30 cases already published. Computerized tomography is the only method that permits both a diagnostic and a therapeutic approach. Surgery is indicated in the presence of brain stem compression but should be carefully discussed in cases of intracerebellar haemorrhage with secondary extension to the posterior fossa. Early hydrocephalus is common and requires shunting in about 25% of the cases

[Prevention of peritoneal adhesions. An experimental study using noxytiolin (author's transl)].

Numerous procedures have been used to try to prevent peritoneal adhesions following operations. Noxytiolin has been used experimentally in the rat in a double blind trial on 3 series of 30 rats. A reliable procedure for making adhesions is said to be always reproducible. Comparing two control groups this study shows that statistically significantly this product lessens the number and seriousness of adhesions that can form, although it does not totally stop them developing.

[Neonatal intestinal obstruction due to congenital hypothyroidism (author's transl)].

Three children with subacute neonatal intestinal obstruction are described in whom the diagnosis of congenital megacolon was suspected. The radiological changes improved when oral feeding stopped but reappeared when it was restarted. The aetiology remained unrecognised until the results of the neonatal screening for hypothyroidism showed all three cases had congenital hypothyroidism. The severity of the disorders was unusual and remains unexplained. Systemic TSH and/or T4 determinations should lead to a better understanding of the disorders due to hyperthyroidism in neonates.
