[Evaluation of arterial hypertension program in a primary care center].

The rate of detection, confirmation, control and follow-up of hypertension in the Canteres Primary Care Center was evaluated two years after the beginning of the hypertension program from a sample of 1219 clinical records. It was found that 572 individuals (46.9%) had at least one blood pressure reading. Of these, 68.2% had values within the normal range, whereas 9.6% of females and 2.8% of males were known hypertensives. High BP levels were found in 69 (19.9%) females and in 34 (15%) males. Of these 103 individuals, the three BP readings were only completed in 34 (49.3%) females and 12 (35.3%) males, and only 26 and 3, respectively, were considered as new hypertensives. In 87% of hypertensives the follow-up was correct, and 59% had BP within normal limits. It is concluded that priority should be given to more deprived and higher risk groups (males of working age), a new evaluation being proposed at 5 years with the following standards; greater than 80% with BP readings (greater than 95% in individuals older than 30 years), with correct confirmation of the diagnosis in greater than 95% and greater than 80% of control and follow-up.

[The respirable fraction: measurement and characterization].

A definition is first of all made of the fractions of airborne dust that can deposit in the various regions of the respiratory apparatus, according to the aerodynamic size of the particles. Since the "respirable" fraction is the most important fraction from a medical point of view, instruments have always been developed that were designed to sample this portion of dust. The author describes the evolution of the criteria for sampling the "respirable" dust fraction and the contribution to research in this field made by the group of industrial hygienists of Milan, under the leadership of Prof. Zurlo, in particular with the development of the aeolian classifier. The results are reported of a comparison made between the four respirable dust selectors most widely used in European Community countries, that was carried out in a pyrite mine in Tuscany. The mean values obtained were practically identical, thus demonstrating that sampling techniques have now achieved proven homogeneity.

Dimethylformamide risk. An evaluation in the production of artificial organic leather.

This study aimed to evaluate the exposure to DMF during the different stages in the production of synthetic polyurethanic leathers. Environmental surveys and biological surveillances were carried out in order to classify the working areas and the tasks of the employees as regards DMF-risk. Design improvements to reduce, as far as possible, environmental pollution are discussed.

[Epidemic outbreak of hepatitis A related to a day care center].

The aim of this study was to describe an outbreak of hepatitis A in a family group which extended to a day care center and to the families of the children attending the same and to determine the risk of acquiring the disease based on exposure to one or several sources of infection. The temporary distribution of the cases and the rate of hepatitis A in the population at risk were analyzed. The risk of acquiring hepatitis by exposure to one or more sources of infection was studied by logistic regression, calculating the odds ratio and the confidence interval of 95%. Initiation of the outbreak was in May 1996, in a 25-year-old male and finalized in November, having affected 63 people. The rate of global attack was of 12% and the risk of infection 18-fold greater (CI 95% = 5.4-61.8) in those exposed to more than one source of infection than in those exposed to only one source and 3.5-fold greater (CI 95% = 1.2-9.9) in the group from 15 to 29 years of age than in those under 14. The massive administration of immunoglobulin was useful to control the hepatitis in the day care center and in the school. The size of this epidemic of hepatitis A was due to its occurrence in a population little exposed to the virus. The greatest involvement was observed in young adults, with person to person transmission and the greater risk of acquiring hepatitis A on exposure to several sources of infection characterizing the outbreak. The possible usefulness of designing prevention strategies with the vaccine should be considered.
