[Paraquat poisoning. Clinical and anatomo-pathological aspects].

Two cases of Paraquat poisoning of anticonservative origin are described. A brief account of the chemical composition and mechanism of action of Paraquat is followed by a discussion of the clinical and therapeutic features of the two cases. The anatomopathological findings obtained from lung specimens examined during necropsy are also described. Personal experience and the literature data are cited in an explanation of what is currently regarded as the most effective treatment protocol.

Biological responsiveness to cholinesterase inhibition: a test for exploring the developmental maturity of the cholinergic system.

The acute mortality caused by two irreversible inhibitors of cholinesterases [diisopropylfluorophosphate (DFP) and diethoxyphosphorylthiocholine, 217 MI-phospholine iodide] has been investigated on chick embryos at different stages of development. The results demonstrate that the above compounds do not show any acute lethal action when administered before the 9th day of incubation; on the other hand, the administration is regularly followed by death after the 9th day of incubation. The doses are comparable to those causing death in hatched chicks. It has also been observed that no appreciable difference exists in DFP uptake from the yolk before and after the 9th day of incubation and that drug-induced cholinesterase inhibition is of the same order of magnitude at any developmental stage; the compound pyridine-2-aldoxime methanesulfonate (2-PAM) was a good antidote against DFP acute lethality. It seems likely that between the 8th and the 9th day of incubation the target system of organophosphorus inhibitors, that is, the cholinesterase enzymatic system, reaches a new point of maturation.

Bringing decision support to nurse managers.

Modern hospital nursing management requires timely and accurate information to allow nurse managers to adjust resources to patient requirements. We report an experience using production theory to provide the framework for the development of Decision Support Objects: graphic displays of nursing hours budgeted, scheduled, and worked within institution-specific control limits. Every month, nurse managers follow an analytic algorithm to understand nursing resources and trends on their units. Exception reporting closes the accountability loop. The essentials of education for nurse managers include skill training in the use of the decision-support tools and supportive lecture/seminars for understanding the managerial implications of using them.

Injection-corrosion cast study of the microcirculation of the brain in haemorrhagic and ischaemic disorders. Preliminary investigation.

The results of studies with the injection-corrosion cast technique in cases of haemorrhagic and ischaemic complications of saccular cerebral aneurysm in man are presented. This technique in conjunction with SEM makes possible a detailed qualitative analysis of the microvascular network. The morphological features of the pathologic areas are analysed. The pathophysiological significance of the observed abnormalities of the cerebral microcirculation are discussed.

Fine needle aspiration cytology of soft-tissue lesions.

The use of fine needle aspiration (FNA) cytology was reviewed in 117 cases of soft-tissue lesions: 23 non-neoplastic lesions, 34 benign mesenchymal tumors and 60 histologically proven soft-tissue sarcomas. The soft-tissue sarcoma aspirates were classified according to their cytomorphology into five groups of possible histologic diagnoses. Difficulties were experienced in the correct diagnoses. Difficulties were experienced in the correct assessment of aspirates from low-grade malignancies. On the other hand, in high-grade malignant sarcomas and in recurrent or metastatic soft-tissue sarcoma, FNA cytology was useful in both the initial diagnosis of a new lesion (22 patients) and in the confirmation or exclusion of a suspected treatment failure (38 patients with recurrence or metastases). In the latter, FNA cytology supported the clinical data and reduced the number of repeat open biopsies. However, the final diagnosis of soft-tissue sarcomas should be based upon the histologic study of tissue sections.

[Lumbosacral extraspinal ependymomas. Review of literature and report of an operated case].

The extradural lumbosacral localization represents for ependymomas an exceptional event full of interest both from an etiopathogenetic point of view and from a clinical one. Extradural ependymomas develop in two characteristic sites: with an anterior or presacral expression in the retro-rectal pelvic area; posterior in the intergluteal space below the lumbo-sacral fascia. The possibilities of a correct diagnosis are based essentially on radiographical examinations and CAT which supply useful factors, but not peculiar to the lesion. The Authors describe a case of posterior "extradural lumbo-sacral ependymomas" which is added to the 36 already reported.
