[Case of Horton's arteritis with development of simultaneous bilateral amaurosis].

The Authors report a case of temporal arteritis which started with a sudden onset of bilateral amaurosis. They considered this case worth recording, on account of its unusual and striking way of manifestation. Moreover, the importance of an early diagnosis of the disease is underlined, since an adequate steroid therapy, carried out in time, often allows to avoid the serious complications which may arise, particularly blindness. Recent epidemiological studies which showed the elevated incidence of the disease in patients above the age of 50, underline the necessity of considering temporal arteritis as one of the diagnostical possibilities, even if the symptomatology is defective or unclear.

[Achondroplasia is also a neurologic disease].

The achondroplasics' ossification defect produce some changes of the spinal column of such a degree to cause a medullary compression. The AA. refer two cases of neurological complication hospitalized since 1960 in the Clinica delle Mlattie Nervose e Mentali - University of Padua. The clinical evolution of the two patients is considered and compared with the cases known in the Literature; the pathogenetic mechanisms of this disease and the therapeutical perspective are discussed too.

Ataxia-teleangectasia: neurophysiological studies in 8 patients.

Eight patients affected by Ataxia-Teleangectasia (AT) were studied with a set of neurophysiological tests. The study was done two times between 2 and 10 years. The most characteristic feature was a severe involvement of the posterior spinal cord: it was associated with a moderate peripheral neuropathy as well as a severe peripheral neuropathy. These data could indicate that the dying back phenomena are not the only pathogenetic basis of the spinal cord degeneration in this disease.

[Aminoacid levels in Werding-Hoffmann and Kugelberg-Welander diseases (author's transl)].

The concentration of free aminoacids in plasma and urine were estimated in 10 patients suffering from Werdnig-Hoffmann's disease of long duration. The age of the patients was between 5 and 14 years. Estimations were also made in 10 patients with Kugelberg-Welander's disease aged between 11 and 34 years. The aminoacid concentrations were estimated on samples of plasma and 24 hours samples of urine by means of chromatography on ion-exchange resins. The data obtained were compared respectively with groups of thirty and ten healthy subjects of the same age. In the group of patients with Werdnig-Hoffmann's disease a significant increase of taurine (p less than 0.001) and of glutamic acid (p less than 0.001) was found in the plasma. The urinary excretion of glutamine was increased in the same group of patients (p less than 0.001) and in the group with Kugelberg-Welander's disease (p less than 0.005). These aminoacid levels are interpreted as an expression of a reduced oxygen metabolism and increased proteolysis in the skeletal muscles in conditions of chronic denervation.

Gadolinium-enhanced nerve roots in lumbar disk herniation.

PURPOSE: To analyze contrast-enhanced features of intrathecal nerve roots in lumbar disk herniation in patients who have not had surgery and to evaluate the possible correlation with herniated disk, peridiscal enhancement, and symptomatology. METHODS: Twenty patients with a syndrome of lumbar herniated disk without surgery were studied with pre- and postcontrast MR imaging. RESULTS: In six patients (30%), ipsilateral intradural roots affected with large disk herniations showed consistent enhancement along their whole length, from the site of compression to the root-medullary junction. This enhancement persisted on delayed scanning. "Selective" enhancement differed from the minimal enhancement seen in normal intrathecal roots in 11 (55%) patients. Enhancement around herniated disks was a common finding (92% of patients) and did not correlate with selective or normal root enhancement (P > .05). No symptom differences were found among patients with and without selective enhancement. CONCLUSION: Selective intrathecal root enhancement seems to be the direct visualization of the radiculopathy resulting from disk herniation. It is probably a transient event more likely to be noted in the acute phase. Although there is no evidence of immediate clinical utility of this finding, gadolinium-enhanced MR gives insights into the basic pathophysiologic alterations involved in degenerative disk disease.

In vivo 1H MRS and in vitro multinuclear MR study of human brain tumors.

Localized in vivo 1H MR spectra of four patients with brain tumors and in vitro multinuclear (1H, 13C and 31P) MR characterization of water soluble and lipid components of the same tumors after surgical removal are reported herein. The in vivo spectra of the neoplastic regions of all patients showed N-Acetyl-Aspartate/Choline (NAA/Cho), Choline/Creatine (Cho/Cr) and N-Acetyl-Aspartate/Creatine (NAA/Cr) metabolite ratios markedly different from those of the controlateral healthy tissue. In vitro 1H MR measurements allowed for the identification of a much larger number of metabolites than those measured in vivo. Indeed, well resolved 1H, 13C and 31P MR spectra were obtained for water soluble and lipid components in all cases. Our data confirm that some metabolites observed in vitro could be specific markers of brain tumors.
