Continuous passive motion after knee-joint arthrolysis under catheter peridural anesthesia.

The results of arthrolysis of a stiff knee are often poor because postoperative pain prevents the early active mobilization that is so essential. Adequate analgesia may be ensured by the use of continuous anesthesia via a peridural catheter; in combination with continuous passive motion, such analgesia is able to maintain, and often improve, the range of movement obtained at surgery. Twenty-two patients treated in this way showed improvement in the range of movement between 39 degrees and 120 degrees. Patients with post-traumatic knee stiffness achieved an average improvement in the range of movement of 93%, while those with stiffness following infection improved by only 55% on the average. The preoperative loss of movement does not appear to determine the end result; the etiology of the stiffness is more important.

[Intrathoracic dislocation of the head of the humerus].

Intrathoracic displacement of the proximal humerus following axillary luxation is a rare injury; but with ipsilateral serial costal fracture and/or single costal fracture with contused lung it has to be included in the considerations concerning differential diagnosis and the tactics of surgical approach. The primary measures are determined by the extent of pulmonary damage; the further course is marked by aseptic necrosis of the head of the humerus. On the basis of our study case we are describing a typical course, and discussing its problems taking into consideration the individual publications available so far.

[Simultaneous autologous spongioplasty and muscle flap transposition to restore the continuity of the leg with an osteitis defect].

About 12% of compound fractures of the lower leg give rise to posttraumatic osteitis. Loss of bone by sequestration causes many problems particularly if soft tissue quality is poor. Cancellous and corticocancellous bone from the iliac crest is used for bony reconstruction. To obtain bony union, a well vascularised recipient for the graft is essential. Local muscle flaps and distant microvascular flaps and distant microvascular flaps improve vascularity by their separate connection to the vascular system. We treated 54 soft tissue defects by local muscle flaps in 41 and by microvascular flaps in 13 cases. In 21 cases a cancellous or a corticocancellous bone graft was used. Necrosis or partial necrosis of the flap occurred in 12 cases. Skin closure and healing was obtained by secondary procedures. Combined muscle flap and bone grafting was found to allow full weight bearing within reasonable period of time.

Deficient autoregulation and lengthening of the diffusion distance in the anterior optic nerve circulation in glaucoma: an electro-encephalo-dynamographic investigation.

Electro-encephalo-dynamographic investigations taken from 36 glaucoma patients showed that the autoregulative mechanism in the region of the anterior part of the optic nerve (apon) is disturbed and the ciliary perfusion pressure is altered in a characteristic manner. The results lead to the conclusion that glaucomatous ischemia occurring in the apon may mainly be due to the following causes and their combinations: high IOP; low blood pressure in the ciliary arterial circulation; insufficiency of autoregulation, and changes of the local conditions of diffusion of oxygen and nutrients.

Relationship between ciliary perfusion pressure and pattern-reversal visual evoked cortical potentials: an electro-encephalo-dynamographic investigation.

Pattern-reversal visual evoked cortical potentials (VECP) taken from 25 healthy volunteers before, during and after short-lasting elevation of intraocular pressure proved to be dependent on ciliary perfusion pressure (pp cil) in a characteristic way. Reductions of pp cil to ca. 30 mm Hg did not result in VECP changes. These results point to efficient autoregulation in the retinal circulation and in the circulatory region of the anterior part of the optic nerve. When pp cil was reduced to below 30 mm Hg a marked reduction of VECP amplitudes and prolongation of latencies were observed. An autoregulatory capacity of about 15 mm Hg was found.

Spontaneous thrombolysis of a large caval thrombus after insertion of an inferior vena cava filter: diagnosis and follow-up by ultrasound.

In the ultrasonic evaluation of a patient with a right-sided subphrenic abscess, coincidentally, a large inferior vena cava thrombus was found and subsequently proved by cavography. Considering the clinical condition of the patient, abdominal surgery was not performed, and instead a percutaneously inserted caval filter was successfully placed above the thrombus. The patient received oral anticoagulant therapy and after 6 weeks, ultrasound demonstrated complete thrombolysis.

[Oculo-oscillodynamography, a new procedure for the determination of ophthalmic artery blood pressure and ocular pulse curve analysis].

OODG is a diagnostic method permitting assessment of ocular pulse curves and objective determination of blood pressures in the ophthalmic circulatory system (retinal and ciliary arterial blood pressures) simultaneously for both eyes, without dilatation of the pupils. Calibrated scleral suction cups pneumatically connected to a sensitive transducer are used to provide well reproducible ocular pulse recordings. The systolic retinal and ciliary arterial blood pressures and the diastolic ocular blood pressure are determined at artificially elevated IOPs from typical changes in the ocular pulse curves.

Oculo-oscillo-dynamography: a diagnostic procedure for recording ocular pulses and measuring retinal and ciliary arterial blood pressures.

An objective noninvasive diagnostic procedure permitting continuous registration of ocular arterial pulsation and determination of retinal and ciliary arterial blood pressures is presented. Arterial pulsation of both eyes is recorded using two infrasound transducers which are connected pneumatically with suction cups placed temporally on the sclerae. By means of a suction pump the intraocular pressure (IOP) of both eyes can be raised artificially to desired values. It is thus possible to study the ocular pulse oscillogram at different IOP values and to determine retinal and ciliary arterial blood pressures without having to perform ophthalmoscopy. Results from first tests of the oculo-oscillo-dynamograph device and data obtained from 30 healthy persons are discussed.

Patients with post-traumatic knee stiffness achieved an average improvement in the range of movement of 93%, while those with stiffness following infection only improved by 55% on the average. The pre-operative loss of movement does not appear to determine the end result: the aetiology of the stiffness is more important.

[Noninvasive diagnosis of occlusive processes of the brachiocephalic arteries: Doppler sonography compared to ophthalmodynamometry and ophthalmodynamography].

97 patients with suspected cerebrovascular disease were examined using ophthalmodynamometry/ophthalmodynamography and dopplersonography. In 49 of the patients we found haemodynamic changes typical of brachio-cephalic artery occlusion processes. In 36 patients angiography was performed either as a preparation for vascular surgery or for further clarification. Ophthalmodynamometry/ophthalmodynamography examination yielded diagnosis in 87%, dopplersonography and combined use of ophthalmodynamometry/ophthalmodynamography and dopplersonography in 94% of the brachio-cephalic artery occlusion processes.
