Elective versus therapeutic radical neck dissection in epidermoid carcinoma of the oral cavity: results of a randomized clinical trial.

This study concerns 75 patients with squamous cell carcinoma of the oral cavity who were patients at the Gustave-Roussy Institute between December 1966 and July 1973. These patients were divided into two groups. The first group was comprised of 39 patients who underwent elective radical neck dissection; nodal involvement was present in 49% with capsular rupture in 13%. In the second group of 36, neck disease appeared during follow up in 19 cases. Therapeutic radical neck dissection was carried out in 17; the nodes were histologically positive in 15, 9 of which had a capsular rupture. In 2 cases, local or general conditions did not permit operative intervention. In this group, the involvement rate was 47% with a 25% capsular rupture rate. However, the comparison of the survival curves by the log-rank test did not reveal any differences, even though histologic prognostic factors were taken into account. These findings led to the conclusion that in squamous cell carcinoma of the oral cavity staged T1N0, T2N0, or T3N0 (from AJC's and UICC's clinical staging system), it seems possible, without risk, to delay neck dissection until a node is detectable, although it is reasonable to perform elective neck dissection in those cases in which the patient is unavailable for regular followup.

[Radiographic signs of radiolesions of the pelvis (excluding the femur) after irradiation for epithelioma of the cervix uteri (author's transl)].

Radiolesions of the pelvic bones (excluding the femur) were observed in 29 patients after irradiation therapy for cervix uteri cancer. Three regions can be affected corresponding to the areas irradiated: the sacrum and internal part of the iliac wings, the pubis, and the cotyles. In most cases the lesions appeared after 1 to 4 years. Their radiological appearances are characteristic, the principal sign being irregular bone condensation in a demineralized bone, without true cavitation. The only lytic lesions observed were in the pubis. Calcification of soft tissues may occur and fractures are frequent. There is a slow progression of the lesions over long periods. Clinical, radiological, and progression signs differentiate radiolesions from other affections: metastases, invasion by contiguity, infections, and radio-induced sarcoma.

[Post-radiation injury of the ileal small bowel: clinical and pathological report of 12 cases (author's transl)].

At the Institut Gustave-Roussy we have collected a series of 12 cases studied both on clinical and patological grounds. These patients underwent ileal resection for acute abdominal symptoms occuring after radiation therapy for genital cancer. Ileal lesions after radiation of gynecological cancer are rare: one per cent at the Institut Gustave-Roussy. These complications occur when certain technical types of radiation therapy are employed, and especially when dose exceeds 50 Gy or when a particular clinical condition is present. Gross examination of the bowel segment shows a stiffness of the digestive wall with frequent stenosis, sometimes in association with perforation or fistula. Histological lesions of radiated bowel are essentially located in the submucosa. They are nonspecific, mainly represented by obliterative angiopathy and fibrosis of the ileal wall.

[Diagnostic value of vaginal mouldings in uterine cancer (author's transl)].

Vaginal mouldings were originally introduced as applicators for uterovaginal radium therapy. After 13 years of experience with this method in the Gustave-Roussy Institute we have found that it is also of great value for diagnostic purposes: it enables the site of the uterine cervical and vaginal lesions to be determined in a more precise manner; areas of invasion of the vagina can be detected which would normally not have been found, either because of the difficulty of performing previous investigations, or because of the extremely superficial nature of certain vaginal lesions: the extent of invasion and the subsequent therapeutic approach can then be radically modified; objective records can be kept in the form of photographs which can be compared with any future mouldings. The vaginal moulding can, in fact, be used as a diagnostic tool during supervision of the patients, when there is the least suspicion of an abnormal finding.

[A projector of iridium 192 wires: motivations and preliminary studies (author's transl)].

Though the majority of procedures involved in curietherapy with Iridium 192 wires cause very few problems concerning radiation protection, this is not true in all cases: in elderly of debilitated patients, young children, or when using special techniques (curietherapy for prostate of bladder cancers for example). In these cases, the need for frequent treatments exposes the therapists to increased doses. The projector of Iridium 192 wires was conceived in order to reduce this irradiation hazard. A simple manipulation places the radioactive wires in a lead container during treatment, and then replaces them in the right position after therapy. This apparatus appears to offer the possibility of: an almost total protection of the therapists, and thus an opportunity for enlarging the indications for curietherapy; an improved quality of treatment to the patient during the application.

The Paris system in interstitial radiation therapy.

As a result of almost 20 years' experience using 192Ir wires in interstitial radiation therapy, a new method of dose calculation has been evolved, which is especially suitable for the techniques employed. Because this system was developed and brought into routine use in Paris, it was called the Paris System. Its basic principles are outlined in this report and the manner in which it is used in clinical practise is explained.

[Present day development of the Paris System. First part: coplanar patterns and those called "in squares" (author's transl)].

The Paris System is a previsional dosimetric system which facilitates the work of curietherapists working with iridium 192 wires. Introduced about twelve years ago for radio-active implantations in one linear plane, this system has now become more generalized for implantations in two or more linear planes. After a brief review of the principles on which the Paris System is based, and introducing the new definitions arising from its development, the authors present the results of a systematic computerized study. The results obtained were used to establish, among others, the simple relationships between the geometric implantation data (disposition, length, number, separation... of the radioactive lines) and the dimensions of the volume treated, for the coplanar patterns and those called "in squares",

Endocurietherapy of penis cancer.

Forty-five carcinomas of the penis have been treated with curietherapy using 192Ir to a dose of 6500 rad. With a minimum follow-up of 4 years, it appears that small lesions (less than 30 mm) have excellent local results (2 recurrences in 14 cases). With only a slight potential for lympatic extension, these patients are alive without evidence of disease after 4 years. The lesions of moderate size (30-40 mm) have a poorer prognosis because of more frequent lymphatic extention and disseminated metastases; in contrast, the local results are still quite satisfactory (4 recurrences in 24 cases). The large lesions (more than 40 mm) recur or necrotize (5 recurrences and 2 necroses in 7 cases). In conclusion, curietherapy of epidermoid carcinomas of the penis is an excellent primary treatment for small or moderate lesions (T1, T2, and T3a) if one wishes to avoid surgical mutilation.

[Proposals for common definitions of reference points in gynecological brachytherapy (author's transl)].

In May 1975 the report of European Curietherapy Group recommended in gynecological Dosimetry by computer. Use of reference points = lymphatic trapezoid figure with 6 points, Pelvic wall, all points are refering to bony structures. Use of critical organ reference points = maximum rectum dose, bladder dose mean rectal dose. Use of 6,000 rads reference isodose described by height, width, and thickness dimensions. These proposals are the basis of a common language in gynecological brachytherapy.
