[Comparison of total N-nitrosamides in fasting gastric juice from subjects in high and low risk areas for gastric cancer].

OBJECTIVE: The status of human exposure to N-nitrosamides (NAD) is almost unknown, although they are probably carcinogenic to human stomach. This study is to elucidate the relationship between NAD exposure and causes of gastric carcinomas (GC). METHODS: The total amount of natural NAD was detected with a photohydrolysis-TEA method (184 nmole/l, detection limit) in fasting gastric juice samples from subjects aged 35-68 by fibroendoscope both in a high and low risk area for GC with the same geographical and socioeconomic conditions in Shangdon Province, China. RESULTS: A) More NAD-positive samples in the high risk area (Linqu County, n = 176) were observed than in the low risk area (Chongshan County, n = 99), the difference was significantly after age-adjustment (40.9% vs. 30.3%, P = 0.0324). B)84% of samples was pH < or = 3 in Chongshan, whereas only 48% in Linqu (P < 0.001). In the acidic samples more NAD-positive ones in Linqu were found than in Chongshan (46.2% vs 27.4%, P < 0.01). NAD-positive rate was higher in samples with pH > or = 5 than those with pH < or = 3 only in the low but not in the high risk area. C)NAD-positive rate was also higher in samples from subjects with chronic atrophic gastritis (CAG) with or without dysplasia than those from normal subjects and from those with superficial gastritis or CAG with intestinal metaplasia both in the high and low risk areas. CONCLUSION: The results suggest that human endogenous exposure to NAD in the high risk area for GC be higher than that in the low risk area; CAG and dysplasia may play a role in human gastric carcinogenesis by N-nitrosamides.

[An experimental study of the growth pattern and mechanisms of surgically induced cleft palate and palatoplasty on maxillary growth in dogs].

The authors observed the changing process of length, width and height of maxillary growth in experimental animals, the experimental animals were divided into the unoperated control group, the surgical induced cleft palate group, the repaired cleft palate group and the group of implanted autograft bone in total hard palate. The method of a serial dorsoventral and lateral cephalograms were used in the study. The experimental result showed: the rate of maxillary growth began to change from two to four weeks and finished in the eighteenth week after operation. There was a remarkable different rate of maxillary growth among the four groups. The track of premaxillary growth had a difference between unoperated control group and the others. The experimental result suggested that the direction of growth in nasal septal cartilage and the conduction of force from premaxillary to maxillary were inhibited. It is a main reason for maxillary growth disturbance of the cleft palate who had or had not surgery.

[An experimental study of the influence and mechanism of surgically induced cleft palate and palatoplasty on teeth and dental arch in dogs].

The dogs were selected as the experimental subjects in this study. The authors continuously observed the maxillary teeth and dental arch form in preoperation and 2, 4, 6, 10, 14, 18, 22, 26, 30, 34 weeks of post operation to all the animals. 40 puppies were divided into four groups, which were unoperated control group, surgically induced cleft palate group, cleft palate repaired group and cleft palate implanted bone group. The experimental result showed: there were remarkable differences at rate of itself growth between deciduous dentition and permanent dentition in the four groups. The result suggested that the cleft palate was a reason for disturbances of dental arch development, an exposed bone wound in hard palate of maxillary after palatoplasty was an important reason that it made maxillary dental arch growth problem heavier, authors could prevent the disturbs the transverse development of the maxillary dental arch from cleft palate who had or had not repaired by implanted bone in total cleft palate.

[A study of human papillomavirus infection during pregnancy and transmission of virus to fetus].

To study the human papillomavirus (HPV) infection during pregnancy and transmission of virus to fetus, samples such as, cervical secretion, peripheral blood, amniotic fluid, neonatal umbilical blood from 96 women with normal pregnancy were screened for HPV DNA by polymerase chain reaction (PCR), and HPV DNA in condyloma acuminata tissues, amniotic fluid of 31 pregnant women were detected by PCR as well. The results showed HPV DNA positive rate of condyloma acuminata tissues was 92.32%, of cervical secretion in normal pregnant group, was 35.71%, of peripheral blood was 57.69%. The HPV DNA maternal-fetus transmission rate was 44.44%, the transmission rate via placenta was 60%. It is concluded that HPV DNA can be found not only in condyloma acuminata tissues, but also in genital tract, peripheral blood of pregnant women, Besides the transmission route of genital tract, there is also the blood-placenta route.

[A study on human behavior and socioeconomic factors affecting malaria transmission and control in Qiongzhong, Hainan].

This study was conducted in Heping District of Quiongzhong County, a hyperedemic mountainous area, in August-September 1992. The comparative surveys between the village and state-run farm, Li and Miao nationalities and Han nationality were carried out by using the sociological method together with the epidemiological methods. Gray relational analysis was conducted between the aforementioned 7 socioeconomic human behavioral factors and IFA rates. The result showed that their degrees of relation (r) were in the following order: (1) percentage of persons who had stayed in the mountain overnight (r = 0.8690); (2) percentage of bed net users (r = 0.7990); (3) percentage of households seeking medical service (r = 0.7990); (4) number of mosquito nets per person (r = 0.7867); (5) percentage of householders knowing malaria transmission route (r = 0.7798); (6) percentage of households with tile-roofed houses (r = 0.6767) and (7) income per capita (r = 0.6636). It indicates that staying in the mountain, using bed net and seeking medical service were three discriminating factors affecting local malaria transmission and control. Therefore, it is suggested that carrying out health education, changing the stay-in-mountain behavior, increasing the utilization of mosquito nets and reinforcing the primary health care should be taken as the fundamental measures for malaria control programme.

[Preliminary studies on microbe-mediated N-nitrosamide synthesis].

Chemical synthesis of N-nitrosamide and influences on it by bacteria and fungi isolated from human gastric juice were studied with trace analysis method to explore the pathway of its endogenous formation. Results showed in synthesis of N-methyl-N-nitrosourea (MNU) with precursors of methylurea (0.42 mmol/L) and sodium nitrite (25 mmol/L) in a condition simulating human stomach, pH of the reaction system played an important role in the synthesis, the amount of synthetic chemical maximized at a pH of 1-3 and almost no synthesis was found at pH of 5-7. A study on the ability to synthesize MNU catalyzed by bacteria isolated from the patients with stomach cancer living in a high-risk area showed its synthesis could be accelerated by Pseudomonas aeruginosa at pH of 6-7 with its catalyzing activity existed in heat liable component of living bacteria. This is the first report showing microorganisms to catalyze the formation of N-nitrosamide from its precursors.

[A study on behavioural characteristics of staying on the mountain and its relationship with malaria infection in Li and Miao minorities in Hainan Province].

A study of sociology combined with epidemiology was conducted in Li Minority's two villages and Miao Minority's two villages nearby the foot of mountain in a historically malaria hyperendemic area, Nanqiao Township of Wanning County in August, 1993. The results showed that malaria infection was closely correlated to the behaviour of staying overnight on the mountain of Li and Miao Minorities during farming seasons. Most of Li people stayed overnight on the mountain for planting areca were adults, accounting for 21.6% of the whole population, the duration of stay was about half a year; Miao people stayed overnight on the mountain planting and harvesting upland rice, always with their family member, so the percentage of people who had stayed on the mountain was as high as 82.1%, the duration of stay was about one and half a month. Blood smear examination showed that the malaria parasite rates in both Li and Miao villages were 11.1% and 24.1%, respectively, the positive rate of P. falciparum was 1.0% and 9.1%, respectively, the positive rate of IFA (titer >or= 1 : 40) was 31.2% and 46.5%, respectively. It is indicated that both the rate of parasitaemia and the P. falciparum infection were much higher in Miao Minority than those in Li Minority (P < 0.001). A cohort analysis showed that the malaria parasite rate of population who having not stayed, having ever stayed and being stayed on the mountain was 6.5%, 27.4% and 42.1%, respectively, of them the rate of P. falciparum infection was 0.5%, 8.8% and 18.4%, respectively. The results suggest that malaria acquired from mountainous forest referring to "up-mountain infection" is a major source of malaria infection and a significant risk factor in determining the prevalence of malaria (especially falciparum malaria) in Hainan Province. This behavioural risk factor has become the main obstacle for malaria control in Hainan Province. Therefore, controlling "up-mountain infection" and adopting appropriate anti-malaria measures to protect the risk population who have to work and stay on the mountain should be strengthened in the malaria control program.

[Observation of compensatory head posture in superior oblique muscle paralysis].

PURPOSE: Superior oblique muscle paralysis is the most common disease in clinic. We analysed the cause, machanism and regularity of compensatory head posture in superior oblique muscle paralysis. METHODS: All of 27 cases with superior oblique muscle paralysis aere examined regularly, and special muscle, binocular single vision function with synoptophore examined. Bielschowsky test was made. RESULTS: In our cases, the incidence rate of compensatory head posture was 40.7%, which had relations with age when a patient had disease, period and function of binocular single vision. CONCLUSION: The existance of binocular single vision function is the physiological bases leading to compensatory head posture, and the patients whose ages are older than 3 years and the course is less than 10 years may present the compensatory head posture. So we should pay attention to early diagnosis and training to restore the binocular single vision function.

[A study on the degradation kinetics of nicotinamide powder].

The effect of temperature and humidity on the degradation of nicotinamide powder was studied below the critical relative humidity at temperatures 50-80 degrees C and relative humidities 50-80%. The rate equation of the degradation of nicotinamide under isothermal and constant humidity conditions is -dC/dt=k(Co-C)-1, where k is the apparent rate constant of degradation, Co and C are initial concentration and concentration at time t of nicotinamide, respectively. The relationship between the apparent rate constant of degradation and relative humidity RH is in K=a + m (RH), where a and m are constants. When RH are 50.35, 66.85, 75.33 and 80.35%, the apparent activation energies of degradation from Arrhenius equation are 219.94, 181.86, 168.99 and 153.89 kJ/mol, respectively. The rate equation the photodegradation of nicotinamide under room temperature is -dR/dt=K' (Ro-R)-1, where K' is the apparent rate constant of photodegradation, Ro and R are initial reflectance and reflectance at time t of nicotinamide, respectively.

[Analysis of relative degree of the grey system on socioeconomic factors contributing to malaria transmission].

This paper reports relative degree of the grey system analysis on socioeconomic factors contributing to malaria transmission in Hainan province. The results indicated that the prior factors related to malaria transmission are economic conditions, primary health care, human behavior and culture. It is suggested that rebuilding primary health care and strengthening health propaganda would be advantageous to malaria control in these areas.

[Studies on distribution and behavior of Anopheles minimus and its role of malaria transmission in Hainan Province at present].

Anopheles minimus was once a main malaria vector in Hainan Island and had been eliminated basically through the campaign of indoor residual spraying launched in 1959. It again became an incriminated vector of some focal malaria outbreaks in recent years. The present study was conducted in a selected county-Danxian and a typical hilly area-Feibar in the west part of Danxian county in 1989-1990. An. minimus was found in 50% and 62.5% of the surveyed sites at mountainous and hilly area of Danxian county, but not found in coastal region. An. minimus was found in all 18 sites surveyed in Feibar district, constituting 52% of anopheline composition. Man-biting rate made by human-baited collection was 3.2 before midnight and 38.2 when collected through whole night in some sites. However, the behaviour characteristics of An. minimus has changed. It has become exophilic, exophagic, and has an equal preference for man and cattle. The vectorial capacity of An. minimus estimated by quantitative data was in accord with malaria infection rate in Feibar district, and the malaria infection rate among the inhabitants in three types of residential quarter with different socioeconomic conditions. Malaria infection rates of residential quarter of land-reclamation outcomers, villagers and state farm residents were 10%, 2.9% and 0.5%, respectively during 40 days from July to August, 1990.(ABSTRACT TRUNCATED AT 250 WORDS)

[Survey and treatment of the blind in Xinhui, Guangdong, China].

Co-operating with Helen Keller International (HKI) in 1987 and 1988, Zhongshan Ophthalmic Center made a large scale prevalent survey of the blind in Xinhui, Guangdong, China. Three screening methods were used in the project. The first was a house-to-house visit by ophthalmologists. The second was performed initially by trained country health workers and then checked by ophthalmologists. In the third one, the blind people were asked to come together to be examined by ophthalmologists. The second method was found to be the most effective. The survey revealed that the prevalent rate of blindness was 0.24% in the county. Of all the blind, 89% were over 50 years old, 73% were females. Cataract accounted for 47% of the blindness. Thirty percent of the blind from cataract received surgical treatment. The postoperative re-examination half a year later showed that the corrected visual acuity in 86% of the patients was more than 0.05.

[Interval division, forecasting and decline tendency estimation model of malaria incidence in Xuzhou City].

This paper uses Grey Model to divide the malaria incidence of 1956-1986 into interval in Xuzhou City. The Grey forecasting GM (1,1) model is used to calculate the malaria incidence in 1987 and 1988. The accuracy of forecasting is 94.83% and 82.44% respectively. Furthermore, the Grey Verhulst model is used to fit the malaria incidence for the study of the declining tendency of vivax malaria incidence. Based on the fitting models, the decline tendency estimation models of vivax malaria are worked out.

[An epidemiological study of the residual vivax malaria cases in Huanghuai plain].

In order to get more knowledge of the epidemiological characteristics of the residual cases of vivax malaria in Huanghuai Plain at the present time, a case-control study was carried out from March to May, 1986 in Zhoukou Prefecture, Henan Province and in Xuzhou City, Jiangsu Province. In both areas, the incidence of vivax malaria in 1985 was lower than 1%. The data were analyzed with the conditional logistic multi-regression model and calculated by using Apple II microcomputer. All the malaria cases selected were within an interval less than 20 days (N.B. The incubation interval of vivax malaria is 20 days) during the transmission season because the cases should be independent of each other and fit with logistic multi-regression model. Three controls were matched for each case at random, including a neighbour person in the same village and a person outside the village. The questionnaire contained a total of 31 relevant items of the subjects, families and villages. Six risk factors (personal history of malaria, family member's history of malaria, number of cases in the village in 1984, direction of breeding place, using mosquito-net and number of livestock) related to malaria transmission were obtained through the analysis of the model. The results were in accord with actual epidemiological characteristics in the region, indicating that the conditional logistic multi-regression model can be applied to epidemiological analysis (study) under controlled conditions and provided quantitative method for analysis of socio-economic effects on malaria. The distribution of the cases and their affecting factors were discussed.
