Ischemic and reperfusive release of the endogenous purines and its influence on the myocardial viability during beta-adrenergic blockade.

The aim of this study was to estimate ischemic and reperfusive release of myocardial adenosine degradation products (MADP) during beta-adrenergic blockade and its relation to infarct size (IS) and viable myocardium size (VM). In a group of 24 shepherd-mongrel dogs, randomly assigned to a metoprolol (M-) and placebo-group (P-group), occlusion of the left anterior descending coronary artery (LAD) followed by reperfusion with recombinant tissue plasminogen activator was performed. Regional myocardial blood flow (MBF) was measured by the radiolabelled microsphere technique. Blood samples from aorta and great cardiac vein were collected to evaluate the concentrations of MADP. The triphenyltetrazolium chloride perfusion and fixation technique was used for infarct size measurement. MBF in the area at risk decreased in both groups during ischemia, but it was significantly higher (p = 0.013) in M-group. Recanalization of LAD was associated with an increase in flow in postischemic vascular bed. MBF was significantly higher (p = 0.024) in P-group during late reperfusion. In M-group IS was smaller (p = 0.007) and VM was bigger (p = 0.007). The correlation between arterial adenosine concentration during early reperfusion and IS (p = 0.044, r = -0.588) or VM (p = 0.036, r = 0.607) in M-group was noted. Values of net MADP balances significantly increased during early reperfusion. The correlation between reperfusive net MADP balance and IS (p = 0.00005, r = 0.906) or VM (p = 0.016, r = -0.675) in M-group was observed. The amount of MADP released during reperfusion correlates with the IS and is inversely proportional to the area of VM. The endogenously released adenosine may have additional cardioprotective effect during beta-adrenergic blockade.

[Anxiety-depressive disorders in women after hysterectomy. Literature review].

In the common opinion amongst woman, the surgical removal of the uterus and ovaries can be a cause of limited physical and sexual activity and have an effect on their attractiveness. The majority of women after hysterectomy can be permanently depressed and can also show symptoms of mixed anxiety-depressive disorder. The review of literature allows for an assumption that the topic of anxiety and depression should not be underrated, by treating it as an exaggerated theme, as some researchers do. By considering the research quoted in this paper the collected directions of diagnostic and therapeutic procedures undertaken by the psychiatrist and psychologist can be used before the planned operation. Special consideration must be given to patients who have a history of affective disorders or sexual abuse. Pharmacotherapy and psychotherapy should applied in order to prevent post-operative affective disorders.

[Anxiety-depressive disorder in women after hysterectomy. Own study].

Hysterectomy is a surgical procedure that significantly affects the quality in which the operated person views herself, lowers self-esteem and brings about changes in the quality of life. The results of the presented study show the necessity of a more accurate and specific dealing with the problem of affective disorders and anxiety states in women who have undergone such an operation. The course of the post-operative period and the return of the patients to full health are largely affected by their psychological state and the quality of life they experience. A successful (in a medical sense) surgical procedure is not a guarantee of the bringing back health in a holistic sense. The results of studies and clinical observations show that half of the group of women operated suffer from anxiety-depressive disorders as a cause of the operation, and a quarter of all those operated require specialist help. No psychological preparation for the operation, absence of closest people in the decision making before the operation, lack of knowledge on the surgical operational-span, lacking psychological aid after the operation--all these can significantly affect the rehabilitation and the process of regaining the social functions. It appears vital to introduce a psychological programme and special care taking of the women who declare having symptoms which appear to lead to the development of depressive disorders. The appearance of depressive symptoms post-operatively as well as the earlier presence of affective disorder symptoms can be prognostic in the further development of the disorder. The early diagnosis of the affective disorder and the higher level of anxiety in women post-hysterectomy and the fast application of appropriate treatment can inhibit further symptom elevation and persistence. Catamnestic studies on women post-hysterectomy due to non-oncologic causes will allow seeing the dynamics of the changes in the affective disorders at different time intervals from the operation.

[An episode of depression in a patient with coronary artery disease].

An episode of depression in a patient with Ischaemic Heart Disease is presented. The patient had undergone coronary angioplasty with optimal results. The clinical picture of the depressive disorder consisted mainly of chest pain complaints in the cardiac area, which made the diagnosis all the more difficult. The application of antidepressive treatment caused the depressive symptoms together with the chest pain to disappear. A therapeutic success was achieved owing to the good co-operation between the invasive cardiologist and the psychiatrist.

[Marital problems in depressive patients: a review of the literature and own clinical investigations].

The study presents a review of the literature and our own findings on marital and social problems of patients with major depression, and their spouses. The impact of illness on family and social life is marked and includes restrictions in social and leisure activities, considerable strain on marital relationships, lack of support. The functional impairment in families of depressed patients is an important factor worsening the course of the illness. These data suggest that in the complex treatment of depression a focus on intervention and prevention in areas of family and marital life is desirable.

Influence of oxy-LDL and interleukin-8 on the platelet/PMNs adhesion and on chemotaxis-mediated induction of iNOS in neutrophils.

Selectin-P expression on platelets stimulated with thrombin was measured in terms of formation of "rosettes" according to Jungi et al. [9]. Selectin-P-mediated platelet/PMNs adhesion was inhibited by iloprost (IC50 = 5.0 nM), sodium nitroprusside (NaNP, IC50 = 0.93 microM) and interleukin-8 (IL-8, IC50 = 88 ng/ml), but activated dose-dependently by oxy-LDL (3-15 micrograms/ml). Glycogen-induced peritonitis in rats up-regulated the iNOS activity measured by the 2,3-[3H]-citrulline formation by the abdominal cavity PMNs up to 6 h after insult. Pretreatment with IL-8 (3 micrograms/300 g iv) decreased the amount of PMNs in ascites as well as its iNOS activity. Chemotaxis mediated iNOS gene expression of PMNs were measured by Northern blot hybridization. IL-8 (100 ng/ml) did not influence the PMNs iNOS gene expression induced by chemotaxis. We conclude that the decrease in iNOS activity by IL-8 involves postranslational modification of the enzyme activity.

[Relationships in families of depressed patients].

There is evidence that many depressed adults come from dysfunctional families. What dominates in our sample of patients treated for endogenous depression is adverse early family environment, especially relationship with father. Mother is described as hopeless and overprotective. Such description of a patient's family may cause--according to the cognitive model of depression--dysfunctional thinking style.

[Sexual dysfunctions in depressed patients].

Sexual dysfunction in depressed patients is an important clinical problem. Its complex etiology involves depressive symptoms, comorbid medical disorders, medication, difficulties in interpersonal relationships with partner, level of sexual functioning prior to the onset of illness. Also, mechanisms of action of psychotropic medication on sexual function are complex. Sexual dysfunction is a source of additional suffering for depressed patients and may contribute to poor compliance with treatment. It is an important challenge for the therapy of depression.
