Bekesy test results in patients with eighth-nerve lesions. Forward reverse- and fixed-frequency tracings.

Sweep-frequency Bekesy tracings, both forward (low to high frequency) and reverse (high to low frequency), as well as fixed-frequency tracings, were obtained from 18 patients who had eighth-nerve tumors (subsequently confirmed surgically). In general, reverse-sweep frequency tracings revealed greater adaptation for continuous tone stimuli than did forward-sweep frequency tracings. However, if a search for excess adaptation (greater than 20 dB) is the sole purpose for administering Bekesy audiometry to a given patient, then fixed-frequency tracings yield adequate differential information.

Combined central and peripheral sympathetic blockade: absence of additive antihypertensive effects.

Two antihypertensive agents that inhibits sympathetic activity by differing pharmacologic mechanisms, the centrally-acting alpha-adrenergic agonist, clonidine, and the peripheral alpha-adrenergic antagonist, prazosin, were compared and also given in combination in 24 patients with essential hypertension. The patients were randomized into two groups; 11 received chlorthalidone (50 mg) daily throughout the protocol, whereas 13 received no diuretic. In the 13 patients not receiving the diuretic, supine mean blood pressure fell by 11.0 +/- 3.5 mmHg (p less than 0.01) with clonidine (0.3 mg daily), 5.5 +/- 2.9 mm Hg (p less than 0.05) with prazosin (15 mg daily), and 12.7 +/- 5.2 mmHg (p less than 0.02) with the combination. In the 11 patients concurrently receiving chlorthalidone, supine mean blood pressure fell by 9.5 +/- 2.0 mmHg (p less than 0.01) when clonidine was added, 2.3 +/- 1.9 mmHg (ns) when prazosin was added, and 7.9 +/- 2.1 mmHg (p less than 0.01) with the combination. In both the presence and absence of concurrent diuretic treatment, the blood pressure-lowering effects of clonidine were greater than those of prazosin, but were not different from the antihypertensive effects of the combination of clonidine and prazosin. Therefore, the simultaneous administration of clonidine and prazosin in the doses used in this study does not appear to be more effective than clonidine alone in the treatment of patients with hypertension.

Tinnitus maskers: a follow-up.

A follow-up study was conducted on the 31 individuals who rented tinnitus maskers after otologic and audiologic examination and consultation. Ten of 31 purchased instruments after a 30-day rental period. Eight of 10 who purchased instruments have been contacted either by letter or by phone. Two of eight are presently using their instruments on a regular basis. One is using his masker sporadically and is not positive about the help received.

Brainstem responses to tone pip and click stimuli.

Brainstem auditory-evoked responses to tone pips at 1000, 2000, and 4000 Hz and clicks were evaluated for 17 normal-hearing adults. Wave V consistently emerged as the best indicator of auditory sensitivity, whereas waves II, III, and IV generally were not identifiable at lower intensities. An inverse relationship existed between wave V latency and intensity and between wave V latency and stimulus frequency. Large ranges were noted for the levels at which the various components became identifiable. Even though some frequency specificity may have occurred, the range of wave V latencies for 1000- and 4000-Hz tone pips demonstrated considerable overlap.

Auditory brain stem response results from 255 patients with suspected retrocochlear involvement.

Auditory brain stem response (ABR) evaluations were conducted on 255 patients with suspected retrocochlear involvement. Twenty-six patients (10%) had surgically confirmed tumors, and ABRs were abnormal in 25 (96%) of these cases. The remaining 229 patients had nontumor medical diagnoses, but 25% of these were found to have abnormalities in ABR recordings. The most common ABR abnormality in the nontumor patients was large wave V interaural latency difference, followed by no response or poor waveform resolution and delayed absolute wave V latency. Peripheral hearing loss, including decreased sensitivity at 2000 and 4000 Hz, appears to have influenced waveform morphology and component latency.

Paget's disease and hearing loss.

Hearing loss, tinnitus, and vertigo have long been associated with Paget's disease of the bone. We reviewed the records of 463 patients with Paget's disease who were seen in the otolaryngology department. Hearing loss was common. It was our observation that mixed hearing losses were part of the disease process. Sensorineural hearing loss was the most frequent loss, but it usually was not part of the disease process. Tinnitus, vertigo, or both were seen in about 20% of these patients.
