Stressful life events and fibrinogen level in middle-aged teachers.

BACKGROUND: Strong evidence has been reported that a high plasmatic level of fibrinogen plays a central role in the development of coronary heart disease. Many researches showed that stressful life events lead to an increase in plasmatic fibrinogen. The present investigation examined the relationship between plasmatic fibrinogen level and stressful life events. SAMPLING AND METHODS: A sample of 40 middle-aged women employed as teachers underwent a general objective examination, a venous puncture for plasma fibrinogen level evaluation, and completed a questionnaire regarding health behaviours, at baseline and 1-year follow-up. The stressful life events were recorded using the Paykel's interview for recent life events. RESULTS: At the baseline assessment, the distribution of teachers in the two stress exposure groups was 16 women for the non-exposed group of teachers, and 24 for the exposed teachers, while at the 1-year follow-up assessment, it was 10 for the non-exposed and 30 for the exposed group. The number of teachers exposed to stress was significantly higher at the 1-year follow-up than at the baseline assessment (30 and 24, respectively; p < 0.05). Average levels of plasmatic fibrinogen were significantly higher in stress-exposed women than in non-exposed women both at baseline assessment (p < 0.01) and at 1-year follow-up (p < 0.002). Moreover, the mean value of fibrinogen at the follow-up assessment showed a higher increase in stressed women compared with non-stressed women with an annual increase of 10.71 and 5.40%, respectively. CONCLUSIONS: Stress may trigger the hypercoagulable state evidenced by an increased plasmatic fibrinogen level. This mechanism may help explain the adverse impact of stress on the cardiovascular system.

Ocular discomfort and conjunctival alterations in operating room workers. A single-institution pilot study.

OBJECTIVES: The aim of this pilot study was to relate the eye symptoms complained of by subjects working in the operating rooms of a hospital in southern Italy, with the observations of alterations of the ocular surface. METHODS: An epidemiological study was carried out by a questionnaire aimed at investigating the prevalence of ocular discomfort symptoms among 213 subjects working in operating rooms and 40 subjects working in the wards. The investigated symptoms were the following: tiredness, heaviness, burning, redness, tearing, itching, blinking, foreign body sensation, and photophobia. A randomised comparative study of the ocular surface and conjunctival cytology was also carried out, comparing two groups of age- and gender-matched subjects. Group 1 included 24 subjects randomly chosen from the operating room workers with ocular discomfort symptoms; group 2 included ten subjects randomly enrolled from hospital personnel working in the wards. Ophthalmological examination of the ocular surface was performed on each subject in the following order: slit-lamp examination, break-up time (BUT) of the pre-corneal tear film, corneal fluorescein stain, lachrymal basal secretion test, conjunctival impression cytology. RESULTS: A high prevalence (72.3%) of ocular discomfort symptoms was reported by operating room workers, while in ward personnel the prevalence was 55% (P = 0.04). The ocular tests showed that the conjunctival features and BUT were statistically significantly altered in subjects in group 1. Also, the conjunctival impression cytology study showed statistically significant alterations of all the investigated parameters: specimen cellularity, cell-to-cell contacts, nucleus/cytoplasm ratio, chromatin pattern, goblet cell distribution, keratinisation and the total cytological score. CONCLUSIONS: Our results show that self-reported eye complaints and ocular surface alterations have a high prevalence in subjects working in the operating rooms. This seems to indicate that the operating room environment could play a role in the onset of the eye disturbances.

Head posture and cephalometric analyses: an integrated photographic/radiographic technique.

A photographic technique for natural head posture (NHP) recording that can be associated to standard radiographic exposures is presented. It allows the evaluation of radiographs, according to both the standard intracranial references and NHP. On the NHP photograph, the angle between the soft tissue nasion-pogonion line and the true vertical is calculated, and this value is used to rotate the standard radiograph around the Bolton point. The technique has been applied to 40 private orthodontic patients (24 females and 16 males, aged 7 to 20 years, mean 12.9 years). The hard tissue Frankfurt plane in NHP showed a wide variation: 80% of the patients had orbitale lower than porion (mean angle -6 degrees relative to the ground), 20% had orbitale higher than porion (mean angle 4 degrees). The position of this plane in NHP seemed to be different in the two sexes, with more males having the Frankfurt plane going upwards than females. The soft tissue Frankfurt plane (tragus-orbitale) in NHP was directed upwards (head extended) in 53% of patients. The two Frankfurt planes were never coincident in all subjects; the tragus was always lower and more anterior than porion. On average, the angle tragus-orbitale-porion was about 6 degrees. In young orthodontic patients NHP is therefore highly variable, gender dependent, and cannot be deduced from mean population values. Nevertheless, the evaluation of head position should be performed in each young patient before and during the treatment, to verify how the combined effects of therapy and growth act.

Lack of effects of thallium on GABAergic mechanisms in discrete areas of the rat brain.

The effects of intramuscular treatment for 3 and 5 consecutive days with thallium sulphate (2.5, 5 and 10 mg/Kg) on GABA content, glutamate - decarboxylase (GAD) and GABA-transaminase (GABA-T) activity in different areas of the rat brain were studied. Thallium at the dose levels used did not produce significant changes in GABA content, GAD and GABA-T activity in the brain hemispheres, brainstem, hypothalamus, diencephalon and caudate nucleus.

[Erythrocyte protective synergism of ascorbic acid and alpha-mercaptopropionylglycine].

The antitoxic action of 1-ascorbic acid and alpha-mercaptopropionyl glycine was examined via the formation of Ehrlich-Heinz bodies in human venous red cells following in vitro intoxication with 100 mg/ml acetylphenylhydrazine. Marked synergism was noted, especially when small amounts of glucose were added. Both substances displayed a marked affinity for red cells and penetrated them on contact without an incubation period. Optimum amounts for maximum synergy were alpha-mercaptopropionyl glycine 0.01-0.02, 1-ascorbic acid 0.01-0.02 ml and glucose 40 mg per ml of blood. At this dose, appearance of Ehrlich bodies was appreciably retarded. In addition, changes in the size and quality of such bodies pointed to enhanced cell resistance to the poison used.

Affective correlates of occupational exposure to whole-body vibration. A case-control study.

BACKGROUND: Exposure to whole-body vibrations (WBV) represents a highly diffuse occupational risk. Some studies report that chronic exposure to WBV induces a significant increment in the modifications of the cognitive and emotional area. The aim of this study was to investigate the long-term effects of WBV exposure on the neuropsychic and behavioural system. METHODS: The study population consisted of 137 male subjects, mean age 45.7 years, working as drivers of mechanical trolleys and occupationally exposed to WBV of a frequency below 2 m/s2. The Profile of Mood States was administered to all subjects in a maximum time of 10 min and in the same operative conditions. The results were compared with those of a homogeneous control group by means of an innovative statistical technique. RESULTS: The results show a significant difference in the response to the neurobehavioural test. The exposed group showed, in fact, a higher score compared with the controls for all 6 factors. In particular, the scores for factors F (fatigue-inertia), D (depression-dejection) and T (tension-anxiety) were more elevated, expressing a significant alteration of the psycho-affective status. Moreover, statistical analysis showed a correlation between exposure time and the score obtained for all factors in the exposed group. CONCLUSION: This study indicates that occupational exposure to WBV is associated with alterations of mood status and that these modifications are directly proportional to exposure time. Furthermore, our results show that psychological tests commonly used in psychopathologic diagnostics can be useful in monitoring alterations of psycho-emotional status determined by physical stressors.

[Symptoms of ocular discomfort and microclimate: epidemiologic and environmental survey in operating rooms].

An epidemiological investigation regarding the ocular symptoms of 213 subjects working in the operating theatres of a hospital of the eastern Sicily was conducted. Information was gathered via a questionnaire designed to identify the ocular symptoms that occurred most frequently during working hours in the operating theatre. The questionnaire was administered by a specialist in Occupational Health, before any decision was made to investigate the indoor air quality. The microclimate parameters (temperature, relative humidity, number of air changes/hour, air velocity, illumination levels) of the operating theatres were also studied. The results of the epidemiological investigation showed that 154 subjects out of 213 (72.3%) reported a lack of eye comfort. The most frequently reported symptoms were fatigue (35.21%) and heaviness (34.27%), followed by burning (25.82%), redness (25.35%), lacrimation (17.84%), itching (2%), blinking (20%), foreign body sensation (19%) and photophobia (10%). The study of the indoor climate of the operating theatres showed that the number of air changes/hour and illumination level were not consistent with those recommended by the Italian Regulatory Authority. Others parameters were considered to be satisfactory. The Authors conclude that the low illumination level may be responsible for the high percentage of subjects suffering from eye fatigue and heaviness. The ineffective rate of room air exchange, possibly interacting with chemicals and aerosols, is thought to be responsible for the other ocular symptoms experienced by the workers. The present study suggests that the operating theatre environment might be a cause of the ocular discomfort often complained of by health care workers. Further studies are needed in this area.

[Cutaneous histopathologic changes in laboratories working in the processing of petroleum].

Petroleum and its derivatives are a complex mixture of many constituents in varying proportions, such as polycyclic aromatic hydrocarbons, alcohols, ethers and additives. It is well established that petroleum derivatives are absorbed through the skin but exposure has not been quantified. Manifestations of cutaneous exposure vary from squamous cell carcinoma to irritative contact dermatitis such as occurs in the workplace. The aim of the investigation was to evaluate cutaneous histomorphological alterations in a group of 10 healthy workers occupationally exposed to repeated and low amounts of gasoline for a prolonged period. Histological examination and immunocytochemical techniques were performed after taking punch biopsies. The results showed the presence of perivascular lympho-plasma cell infiltration and leucocyte cells scattered in the dermal layer. A mild increase in epidermal Langerhans cells was also observed while the detection of the Ki-67 antigen in keratinocytes gave a normal appearance. In conclusion, the overall histomorphological and immunological features was not specific, but resembled skin reactions due to several irritative agents. Such skin alterations can lead to contact dermatitis.

A cross-sectional study of workers with occupational exposure to petroleum derivatives.

Expression of human involucrin (hINV) a protein of the cornified cell envelope, was studied in the skin of gasoline storage workers, in order to evaluate the effects of the exposure to petroleum derivatives. A total of 25 forearm skin punch biopsies were carried out. Twenty of which were performed on exposed subjects and five on controls. The specimens were processed for immunohistochemistry and hINV expression was evaluated using an anti-hINV monoclonal antibody and the ABC technique. Percentage of immunolabeled keratinocytes was significantly higher in subjects exposed to gasoline with respect to the control sample. A premature hINV expression was detected both in suprapapillary and interpapillary keratinocytes. Such overexpression of hINV seems to be related to an attempt of increasing skin defence mechanism. Therefore it was concluded that also in absence of clinical skin manifestation the exposure to gasoline determines an involvement of keratinocytes on molecular basis.
