Paraprotein interference causing pseudohyperphosphataemia: evaluation of an improved methodology.

The measurement of serum inorganic phosphorus by direct acid/molybdate assay is known to be subject to interference by some serum paraproteins. Serum samples from 45 paraproteinaemic patients were used for the measurement of inorganic phosphorus using a manufacturer's kit reagent and a detergent-modified reagent from the same manufacturer. In addition, the same samples were analysed with both reagents following precipitation of serum protein with trichloroacetic acid. Using the original reagent pseudohyperphosphataemia was found in 27% (12/45) of the samples. The modified reagent was free from interference with no significant difference between results from serum and protein-free supernatants.

Reference ranges for serum creatinine in infants, children and adolescents.

Reference ranges for serum creatinine, derived from 2110 cases aged between 1 day and 20 years, are presented. Levels fall during the neonatal period and then increase with age in both sexes. Levels fall more rapidly in full term infants than in premature infants and are significantly higher in males than in females aged 15 years and above.

Quality control of measurements of pH, carbon dioxide tension, and total carbon dioxide in plasma.

We have measured total carbon dioxide in plasma with a new carbon dioxide analyzer, and compared the results with total carbon dioxide data derived from measurements of carbon dioxide tension and pH. The results agree sufficiently well to demonstrate that the new instrument provides a simple, efficient procedure for monitoring the precision and accuracy of pH, carbon dioxide tension, or total carbon dioxide in plasma.
