The treatment of postmenopausal syndrome by monthly oral doses of quinestrol.

Seventy patients between the ages of 37 and 59 suffering from the menopausal syndrome were included in a clinical trial and treated for a period of 6 to 18 months. Out of the seventy, 43 were suffering from spontaneous and 27 from surgical menopause. Forty of them were given 1 mg of quinestrol and 30 received placebo. The drug was administered orally in a one-tablet dose once a month. An improvement took place in 35 (87.5%) of the women receiving quinestrol but in only 15 (50%) of those receiving placebo. Among the patients with spontaneous menopause an improvement was seen in 22 out of 25 (88%) receiving quinestrol, compared with 9 out of 18 (50%) receiving placebo. Tolerance of the drug was good and most of the laboratory tests as well as blood pressure and body weight showed statistically non-significant changes. This kind of treatment is expecially suitable when daily intake is undesirable.

[Clinico-pathological correlations in lupus nephritis with reference to therapeutic and prognostic aspects (author's transl)].

Since 1970 in 27 out of 46 patients with the diagnosis of systemic lupus erythematosus (SLE) a renal biopsy could be taken. The morphological outcome was followed in 14 patients with a total of 18 repeated biopsies. By light- and electron microscopy renal involvement was demonstrable in all patients. Four histologic subgroups could be differentiated: Mesangio-proliferative (MESLN, 14), focal proliferative (FLN, 6), diffus proliferative (DLN, 6), and membranous lupus nephritis (MLN, 1). Some biopsies demonstrated linear deposits with IgG/IgA-specificity. 2/27 patients only showed a clinical deteriorating course with progressive renal insufficiency despite steroid or steroid-azathioprine therapy. One patient with DLN died in terminal renal failure. The morphological follow-up showed an unfavourable course in 3/14 patients only. One MESLN demonstrated a transition to DLN, one DLN an increase of proliferative lesions and a second DLN focal and local sclerosis. In our experience renal involvement in SLE can adequately characterised and controlled by repeated be clinico-pathological correlations an aggressive therapeutic regimen is not indicated and can be avoided.

[Anticoagulation and immunosuppression in rapidly progressive glomerulonephritis of poststreptococcal type (author's transl)].

In the majority of cases acute poststreptococcal glomerulonephritis is characterized by a good prognosis. Only relatively rare courses of this disease with clinical symptoms of rapid progressive glomerulonephritis and morphological signs of extracapillary glomerulonephritis together with crescent formation are sometimes prone to an infavorable outcome. A widely accepted medication does not exist up to now. Our own observations including clinical and morphological follow up studies are suggesting a combination of anticoagulants and immunosuppressive drugs. Indication and usefullness of that regimen are discussed.

[Hyperergic collagenoses. Clinical aspects, differential diagnosis and therapy (author's transl)].

On the basis of inpatient and outpatient observations of the longterm course in 44 patients with erythematodes disseminatus, 8 with periarteritis nodosa, 4 with Wegener's granulomatosis, 5 with dermatomyositis, 3 with giant cell arteritis and 9 with scleroderma the pathogenesis, differential diagnosis and therapy of these diseases is discussed. The differential diagnosis and longterm supervision being dealt with particularly thoroughly. Early recognition of the disease episodes and appropriate therapeutic action in the shape of increased doses of prednisolone are important. Additional cytostatics were given if the kidney was involved.

[Renal vein thrombosis: a complication of the nephrotic syndrome? (author's transl)].

The nephrotic syndrome is complicated by an increased thromboembolic risk. This is related to changes in one or more factors of the Virchow triad (vessel wall, blood flow, coagulation). The hypercoagulability may be enhanced by iatrogenic manipulations as for instance vessel punctures, application of steroids and diuretics. The preferential location of thrombus formation are the veins of the leg, pelvis, kidney, and the cerebral sinuses. In contrast the "primary" renal vein thrombosis has long been considered not as a complication of the nephrotic syndrome but as its cause. In the meantime there have been accumulated histological, experimental and clinical facts showing the nephrotic syndrome causative for the renal vein thrombosis. This is revealed too by our own cases.

[Dose-response relationship of bumetanide in healthy subjects].

A diuretic effect is demonstrated following oral administration of 0.25 mg 3-n-butylamino-4-phenoxy-5-sulfamoylbenzoic acid (bumetanide, Burinex-R), the maximum response apparently not being reached by a dose of 0.5 mg. Within this range the dose-response curve is steep and approximately linear. The pattern of urinary volume and electrolyte excretion bears close resemblance to the "high-ceiling" diuretics known so far. Bumetanide differs from them by a markedly increased potency compared on the basis of molar dosage.

The flight experiment ANITA--a high performance air analyser for manned space cabins.

Analysing Interferometer for Ambient Air (ANITA) is a flight experiment as precursor for a permanent continuous trace gas monitoring system on the International Space Station (ISS). For over 10 years, under various ESA contracts the flight experiment was defined, designed, breadboarded and set up. For the safety of the crew, ANITA can detect and quantify quasi on-line and simultaneously 32 trace gases with ppm or sub-ppm detection limits. The self-standing measurement system is based on Fourier Transform Infrared Spectrometer (FTIR) technology. The system represents a versatile air monitor allowing for the first time the detection and monitoring of trace gas dynamics of a spacecraft atmosphere. It is envisaged to accommodate ANITA in a Destiny (US LAB) Express Rack on the ISS. The transportation to the ISS is planned with the first ATV 'Jules Verne'. The options are either the Space Shuttle or the Automated Transfer Vehicle.
