An improved motorcycle helmet design for tropical climates.

Approximately 20% of all fatal traffic crashes in India involve riders of motorized two-wheelers. Head protection of two-wheeler riders is therefore a very important issue. This paper reports the results of a survey of patterns of motorcycle helmet use in Delhi, and the design details of a more comfortable helmet. The survey results were used as guidelines for designing a safer helmet, with a better ventilating system, to promote wider use. In the new design additional protection is provided on the sides of the helmet, since our earlier studies showed that more head injuries occur on the side of the head than on the top. A low-cost locking device has also been developed with which the helmet can be secured to the vehicle when not in use. All these design features are incorporated into a stylized product form. The new design is being marketed by a Delhi-based manufacturer and has received a very favourable response in the market.

Unidimensionality of alcohol dependence syndrome?

To evaluate the coherence of Alcohol Dependence Syndrome (ADS), as per DSM-III-R criteria, 72 and 223 subjects with alcohol dependence disorder from Hospital and Community settings, respectively, were interviewed. Nine DSM-III-R criteria assessing the dependence syndrome were factor-analysed. A single factor model provides an adequate description of the interrelationship among the nine criteria and supports the coherence of the dependence syndrome for the hospital sample but not for the community. A two factor model, namely 'withdrawal' and 'social', fits the community data. When the criteria were combined into cumulative scales, they formed good approximations of unidimensional Guttman scales for both the hospital and community settings. For hospital data, preoccupation was the highly central criterion (lambda = 0.95) in defining the dependence syndrome while 'can't stop' (v = 0.11) measured the most severe level of dependence. In the community the 'socially dysfunctional use' identified as a separate secondary factor measured the most severe level of dependence.

Mathematical modelling and field trials of an inexpensive endoskeletal above-knee prosthesis.

The swing-phase motion of the shank of an above-knee prosthesis has been modelled mathematically. An inexpensive endoskeletal prosthesis was designed using the Jaipur foot and conduit pipes with a hinge joint for the knee. Results of field trials and the modelling indicate that a very simple above-knee prosthesis can give near normal gait at "normal" walking speeds on flat surfaces. The swing of the shank is most sensitive to the timing of toe-off.

A rapid survey on substance abuse disorders in the urban slums of New Delhi.

The results are presented of a rapid survey screening method of general population for identifying dependent and non dependent drug abusers, especially in vulnerable groups such as slum dwellers. The results showed that with the help of instrument with modified DSM III and interviewing only the heads of the households it was possible to get reliable estimates of dependence disorder in the community. This method can be of assistance to health planners for a quick assessment of the magnitude of the problem leading to better allocation of funds and developing services for the affected population. The method is an improvement on key informant technique and a full fledged survey.

Seasonal variation of persistent organochlorine insecticide residues in Vellar River waters in Tamil Nadu, South India.

Water samples collected from Vellar river and Pichavaran mangroves at Porto Novo (11 degrees 29' N, 79 degrees 46' E), Tamil Nadu State, South India, from December 1987 to January 1989 were analyzed to determine the seasonal variation of the levels of organochlorine insecticides such as HCH (BHC) and DDT. Both these insecticides showed higher levels from October to February, although this trend was more pronounced in HCH than DDT, reflecting the application of technical HCH largely and probably small quantities of DDT during the flowering season of rice. The alpha-HCH was detected as a dominant isomer for all seasons monitored followed by beta-HCH. Among DDT compounds, p,p'-DDT was the highest in river water except in the dry season when p,p'-DDD showed a higher percentage. On the other hand, in mangroves p,p'-DDE was highest during the wet season and p,p'-DDD during the dry season. Air-water partitioning data of HCH isomers and DDT compounds in Vellar river revealed that these chemicals tend to be in the water phase. These observations may aid in understanding the role of a tropical paddy area on the behavior and fate of man-made chemicals in view of worldwide contamination.

Fireworks cast a shadow on India's festival of lights.

This article describes a campaign for the safe use of fireworks and the prompt application of cold water to skin burned by them. Some success was achieved in the latter regard as a result of frequently repeated television announcements.

Seasonal variations of organochlorine insecticide residues in air from Porto Novo, South India.

The levels and percentage composition of these insecticides recorded in the present study may aid in interpreting the role of a 'point source' area since India is one of the countries still using the persistent organochlorine pesticides in large quantities.

Alcohol-related problems in the emergency room of an Indian general hospital.

Alcohol-related problems made up 17.6% of the case load of psychiatric emergencies in an Indian general hospital. The police brought three-quarters of them, 45% for quarrels, street-fights and under influence of alcohol and 20% for minor offences like abusing in public. A psychiatric illness was definitely present in 40% of the cases. Only 10% of the patients with alcohol-related problems were referred for outpatient treatment. Eighty-five percent were not given any follow-up advice because the patients said they needed no help.

Arachnoid cysts of the middle fossa predispose to subdural haematoma formation fact or fiction?

An association between arachnoid cysts of the middle fossa (ACMF) and complicating subdural haematoma (SDH) has been previously noted. More recently it has been hypothesized that ACMF may predispose to SDH formation. The Plymouth Neurosurgical Unit has treated twenty patients with ACMF between 1976-1985, seven of these being complicated by SDH. There was an age range of 11-56 years in those with SDH. Six of the seven patients with ACMF and SDH gave no significant trauma history. Four of these were males aged 11 to 20 years. The presenting histories, clinical findings and subsequent management of these patients were compared with the age-matched males with SDH alone (twelve patients). In the SDH alone group 100% suffered major skull trauma and 80% had demonstrable skull fractures. In addition the patients with ACMF were compared with patients presenting with other supratentorial arachnoid cysts in Plymouth. Only ACMF were associated with the development of SDH in our study. Three patients demonstrated total masking of the ACMF by isodense intracystic haematoma on computed tomography. In two of these patients the presence of an ACMF was suspected due to plain radiographic and CT enlargement of the middle fossa. It is advocated that with special reference to young males, in the absence of major skull trauma an ACMF should be suspected as a predisposing factor to SDH. Postoperative CT scans for at least one year are advisable in young patients with SDH as demonstration of the presence of an ACMF and SDH affects future management.

Turcot's syndrome. A review.

Turcot's syndrome represents the association between familial multiple polyposis coli and neural tumors. The syndrome is reviewed with reference to genetics, colonic and central nervous system manifestations. This evidence suggests that Turcot's syndrome represents one manifestation of the pleiotropic autosomal dominant gene responsible for familial polyposis coli and the associated extracolonic manifestations of Gardner's syndrome. A diagnosis of Turcot's syndrome should be restricted to those patients with familial polyposis coli associated with gliomas or medulloblastomas. A further case is presented that is believed to be the first report of such a case in the United Kingdom.

Determinants of emergency room visits for psychological problems in a general hospital.

Determinants of emergency room visits for psychological reasons were studied prospectively for a four month period in an Indian General Hospital. Psychiatric emergencies constituted only 2% of all emergency visits. Most of the patients were new except for 7.4% who were already registered with the outpatient services of the psychiatry department. Males outnumbered females in a ratio of 2:1. Self-referrals constituted 77% of the samples; 21% of patients were brought by police. Two-thirds of the patients were brought owing to the severity of their clinical condition and the rest, one-third, for medico-legal and social reasons. Approximately 80% of the patients sought consultation within one month of the onset of illness episode. First episode of mental illness was within last one year of the emergency room visit in 60% patients. Past history of hospitalization for mental illness was obtained only in 10% of cases. The pattern suggested that there was no misuse of emergency services by psychiatric patients although 20% of the patients presented with social problems only which required social rather than psychiatric intervention.

Unwelcome guests: bugbears of the emergency room physician.

In a prospective study on psychiatric emergencies in the setting of a general hospital, the phenomenon of repeat-visits was studied. Results confirm that repeaters represent a sizeable load on emergency services--about 18% of all psychiatric emergencies. Females and neurotics are over-represented among the repeaters. Chronic repeaters tend to evoke strong feelings among the emergency physicians thereby decreasing their chances of referral to the psychiatry resident on call.

Hypopituitarism presenting as delirium.

A case of hypopituitarism following childbirth (Sheehan's Syndrome) presenting with delirium is described. Diagnosis was confirmed with pituitary stimulation tests and the patient remains symptom-free for the last three years on maintenance hormonal therapy.

MRI diagnosis of vertebral arteriovenous malformations in neurofibromatosis.

We describe two patients with neurofibromatosis, each of whom was found to have an arteriovenous malformation (AVM), an uncommon manifestation of the disease, presenting as an extradural space-occupying lesion in the cervical spine. In one patient the finding of an unsuspected AVM at operation led to massive blood loss; in the other magnetic resonance imaging (MRI) of the neck allowed the diagnosis to be made and the hazard of an operation to be avoided. Solid tumours that may occur in the disease of neurofibromatosis are known to cause neurological deficits as the result of space occupation. We wish to draw attention to another pathological entity which may cause neurological deficits: that of AVMs. We also propose that in the investigation of such a patient MRI be borne in mind as a non-invasive radiological tool, particularly as, hopefully, its cost reduces and its availability becomes more widespread.
