Anti-nidatory effect of vaginally administered (Ala8,13, 18)-magainin II amide in the rhesus monkey.

The hypothesis that timed application of a potent anti-microbial, anti-tumor agent like magainin peptides can inhibit blastocyst implantation was examined in the present study using the rhesus monkey as the primate model. Incidence of pregnancy, vaginal bleeding patterns, serum levels of progesterone, estrogen and monkey chorionic gonadotropin were examined following vaginal administration of (Ala8,13,18)-magainin II amide, a synthetic analogue of magainin 2, via tampon during days 20 to 26 of mated cycles. Implantation occurred in two out of three animals following administration of 0.25 mg magainin, while administration of 0.5 mg (Ala8,13,18)-magainin II amide resulted in inhibition of implantation in all females with no change in lengths of treatment cycles, and subsequent cycles. It appears from the present study that, besides being a local microbicidal agent, intravaginal administration of (Ala(8,13,18))-magainin II amide is a potential anti-implantation strategy for intercepting pregnancy.

Anti-nidatory effect of vaginally administered fumagillin in the rhesus monkey.

In the present study, fumagillin, which is an antibiotic with potent angiostatic activity secreted from Aspergillus fumigatus was administered intravaginally during peri-implantation stage in the rhesus monkey and its effects on ovarian function, blastocyst implantation and pregnancy outcome in the rhesus monkey were investigated. Female monkeys (n = 18) showing normal menstrual cycles were vaginally inserted with tampons containing fumagillin (0 mg/animal in group 1; 1 mg/animal in group 2; 2 mg/animal in group 3; 4 mg/animal in group 4) on cycle day 20 of the mated treatment cycle, and these were removed on day 26 of the treatment cycle. Pregnancy was found to occur in animals treated with 1 mg and 2 mg fumagillin. However, animals treated with 4 mg fumagillin remained non-pregnant along with decreased (p <0.001) concentration of progesterone in circulation during the luteal period compared with that in normal, non-mated, ovulatory cycle.

Evaluation of efficacy, safety, and reversibility of combination regimen of cyproterone acetate and testosterone buciclate in bonnet monkey.

A combination regimen of cyproterone acetate (CPA) and testosterone buciclate (TB) was evaluated for its contraceptive efficacy, safety, and reversibility in bonnet monkeys. Cyproterone acetate (5 mg in 0.2 mL of olive oil) injected daily for 180 days, in combination with 40 mg testosterone buciclate given i.m. on days 0, 60, and 120 in the monkeys of group II (n = 6) induced azoospermia in all animals by 120 days, which was maintained until day 210. By day 240 sperm concentration increased gradually and reached baseline values by day 330. When 5mg of cyproterone acetate was injected daily for a similar duration in combination with a higher dose (80 mg) of testosterone buciclate in the monkeys of group III (n = 6) on days 0, 60, and 120, uniform and consistent azoospermia could not be achieved and two animals remained oligozoospermic even after 180 days of treatment. Mean sperm concentration did not return to baseline values until the day that the study ended, i.e. day 330. In groups II and III serum testosterone levels were elevated (p <0.05) from days 9-120 except on day 150 and returned to near baseline values by day 330. Serum testosterone levels were higher in group III compared to group II. The sperm concentration and testosterone levels in control animals (group I; n = 6) showed fluctuations. Lipid profile and liver function parameters did not show significant changes in any group. The present data clearly indicate that administration of CPA and TB in proper dosage combination can provide an effective, safe, and reversible method of male contraception.

Cervical carcinoma recurring in an abdominal wall incision.

Incisional recurrence of carcinoma of the uterine cervix is very rare. We report two patients who developed recurrence in the abdominal wall incision after hysterectomy. The first patient was aged 60 years and had a FIGO Stage IB1 cervical carcinoma treated initially by Wertheim's hysterectomy and postoperative radiotherapy. Nine months after surgery she developed recurrence on the abdominal scar, which was outside the radiation portal. She was treated with electron beam radiation therapy. The second patient developed an incisional recurrence 23 months after surgery and was treated by wide excision of the mass followed by radiotherapy. A survey of the literature identified 12 reported cases to date (eight squamous cell carcinoma and four adenocarcinoma).

Palliative treatment of advanced non small cell lung cancer with weekly fraction radiotherapy.

Various hypofractionated radiotherapy schedules of radiotherapy have been tried in patients of advanced NSCLC with no conclusive superiority of either a more protracted low dose per fraction schedule or a rapid course of large fraction size. Present study evaluates the role of weekly high dose radiotherapy for the palliation of various intrathoracic symptoms like cough, chest pain, hemoptysis, dyspnoea. A total of 47 patients of advanced non-small cell lung cancer were treated with four weekly fraction of 5 Gy to a total dose of 20 Gy. Mean age at presentation was 53 years. Majority of patients had stage III disease. Pain relief was seen in 77% (23/30) patients with complete relief in 57%; haemoptysis was completely relieved in 77% patients (17/22); and cough was relieved in 68% (19/28) patients. Complete relief from dyspnoea was seen in only 5/12 (42%) patients whereas 2/12 (17%) patients had partial response. The present regime of 20 Gy in four weekly fractions was well tolerated by the patients without any identifiable acute toxicity and was simple and convenient to the patients, who traverse a long distance for radiotherapy in India. To conclude, weekly schedule af EBRT provide effective palliation of various intrathoracic symptoms in the patients with advanced NSCLC.

Paclitaxel-epirubicin in advanced breast cancer.

Breast cancer remains a major cause of morbidity and early death in women worldwide. Despite the responsiveness of advanced breast cancer to a number of chemotherapeutic and hormonal agents, long term outcome remains poor. The introduction of paclitaxel with a novel mechanism of action has kindled a ray of hope. Combination of paclitaxel with anthracyclines are being tried, with varying degree of success. Twenty patients with metastatic or locally advanced breast cancer were treated with Paclitaxel (175 mg/m2) and Epirubicin (80 mg/m2) administered sequentially. Each patient received 3 to 6 such cycles at 3 weekly intervals. A response rate of 85% (95% Confidence Interval (CI) 69%-100%) was observed in these patients with 25% (95% CI 6%-44%) achieving complete response. A response rate of 100% was observed in the six patients with locally advanced disease who had not received any chemotherapy earlier. Grade III neutropenia occurred in 5 patients and was reversible in all the cases. This combination is well tolerated. Its efficacy is being compared in a randomised trial with CAF regime in advanced breast cancer in our center.

Radiation therapy in skeletal metastases.

From June 1993 to September 1995, 132 case files of patients who received palliative radiotherapy (RT) for skeletal metastases were reviewed. Majority of the patients (75/132) was in the age range of 41-60 years. Common sites of metastases were the vertebrae (86 patients) and the pelvic bones (40 patients). The major primary tumors encountered were as follows: multiple myeloma (30), breast cancer (25) and prostate cancer (20). Pain was the commonest symptom of presentation. Doses of palliative RT ranged from 8-40 Gy in different fractionation schedules. Ninety-four patients showed more than 50% symptomatic response, 26 had no response and 12 were lost to follow up immediately after the treatment. Single fraction treatments resulted in almost similar responses compared to various multiple fraction treatments. To conclude, short course radiation therapy regimes are optimum in our local circumstances where RT resources are limited and patients have to travel long distances to attend hospital.

Therapeutic value of bovine saliva in wound healing: a histomorphological study.

Biopsies were collected from the control (normal saline solution treated) and saliva treated wounds (back and loin region) of calves on 3rd, 7th, 14th, 21st and 28th days post-wounding. A faster connective tissue and epidermal regeneration was observed in saliva treated group. The inflammatory response was subdued as early as on 3rd day. On the 14th day the test wounds were covered with a dense layer of fibroblast and collagen and epidermal growth started. On 21st day the condensation of collagen and vascularization of the scar tissue was further augmented. By 28th day, the wounds were completely filled and nearly 85% of the wound surface was covered with epidermis.

Effects of testosterone buciclate on testicular and epididymal sperm functions in bonnet monkeys (Macaca radiata)

The effects of testosterone buciclate (TB), a long-acting androgen ester given i.m. at four sites (20 mg/site) on days 1 and 91 of the study period (360 days), on reproductive and hormonal parameters were evaluated in five adult male bonnet monkeys; untreated animals (n = 5) acted as controls to monitor seasonal changes in these parameters. In control animals, testicular volume remained unchanged throughout the study; sperm count, motility and gel penetrability decreased while the percentage of spermatozoa showing retention of cytoplasmic droplet and coiled tail increased in June-July (days 210-240), preceded by reduction in serum testosterone (T) levels on days 120-150 (March-April). The TB-treated animals showed reduced testicular volume (days 90-270), suppressed sperm motility and gel penetrability (days 45-240 except on day 120), decreased sperm count (days 75-270), and an increased percentage of spermatozoa showing retention of cytoplasmic droplet and coiled tail (days 45-240 except on day 120). Even though serum T levels remained elevated until day 300, these levels were within the physiological range. The changes induced by TB were reversible. The suppression of testicular and epididymal functions by TB indicates that this long-acting androgen may have the potentiality to induce and maintain reversible sterility, but further evaluation needs to be carried out to develop an appropriate dosage regimen that would prevent return to normal functions in order to develop this long-acting androgen as a hormonal male contraceptive.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of the ability of a new long-acting androgen ester to maintain accessory gland function in castrated rhesus monkey.

The ability of 40 mg of milled suspension of a new long-acting androgen ester (20 Aet-1) to restore and maintain accessory gland function was compared with that of testosterone enanthate (TE) in castrated adult rhesus monkeys. Castration did not abolish the ejaculatory response since only two animals did not void semen in the postcastration period. A single intramuscular injection of 40 mg of these compounds stimulated accessory gland function at levels lower than in the pretreatment period. 20 Aet-1-induced stimulation of prostatic acid phosphatase activity never exceeded control levels unlike that induced by testosterone enanthate which caused hyperstimulation on day 21 of drug treatment. In terms of support of accessory gland function, 20 Aet-1 would appear to offer hope of being a successful androgen for supplementation therapy in male animals.

Time-controlled injectable occlusion of the vas deferens.

An injectable method of obtaining vas deferens occlusion is presented. The agent used is styrene maleic anhydride (SMA) with dimethyl sulphoxide as the solvent. The special feature of the approach is that after a period of time, spermatozoa passage is spontaneously reestablished. By varying the dose of SMA administered, the interval between injection and restoration of spermatozoa flow can be regulated from as short a period as about 3 months to more than five years.
