Fertilization with human testicular spermatids: four successful pregnancies.

Between July 1995 and May 1996, 36 patients with non-obstructive azoospermia of secretory origin underwent intracytoplasmic injection of spermatids. A previous histological biopsy was performed on all patients: 15 had spermatogenic arrest, a further 13 had Sertoli cell-only syndrome, and the remaining eight had post-cryptorchidism tubal atrophy. The ejaculate was duly examined and a complete absence of spermatozoa and spermatids was confirmed, with only bacteria and debris being found. Testicular sperm extraction (TESE) was then performed. In 19 out of 36 cases round spermatids only were found, while elongated spermatids were found in the remaining 17. Both round and elongated spermatids were isolated and used for injection. A total of 135 oocytes at metaphase II were recovered from 19 partners and injected with round spermatids, while 123 mature oocytes from 17 partners were injected with elongated spermatids. The number of oocytes fertilized, as judged by the presence of two pronuclei, was 75 (55.5%) and 71 (57.7%) respectively. By 34 h after injection, the number of embryos which had cleaved to the 2-cell stage was 56 (74.6%) with round spermatids and 55 (77.4%) with elongated spermatids. All cleaved embryos were transferred into the uterus of the partners. Clinical pregnancies were established in two cases of round spermatid cycles (10.5%) (both are still ongoing), and three cases of elongated spermatid cycles (17.6%) (two are still ongoing; one was lost after 8 weeks of gestation). Chromosomal analysis showed that all fetuses had a normal karyotype (three male and one female) with no chromosomal abnormalities.

Nocturnal and real-time Rigiscan monitoring in patients with venous incompetence.

OBJECTIVES: In cases of failure of the vascular occlusive system, testing with both nocturnal and real-time monitoring will offer the possibility of making an accurate diagnosis of venous occlusive dysfunction. METHODS/RESULTS: The study was conducted in 124 cases suspected as having venous occlusive dysfunction (VOD). All patients were submitted to cavernometry, which confirmed VOD in 41 cases; however, the existence of a venous disorder could not be established in 83 cases. Nocturnal monitoring with the Rigiscan System was performed in 68 cases; no organic disorder was found in 6 cases. CONCLUSIONS: Although the results of nocturnal monitoring with the Rigiscan is not always consistent, in our experience it has been useful in both nocturnal and real-time monitoring of the erectile responses during many diagnostic tests such as VSS or intracavernosal injection of vasoactive agents.

[Intra-cavernous injections of vasoactive drugs. A 10-year experience at an andrologic center].

The clinical experience of an andrological center in the use of the intra cavernous injections of vaso-active drugs is revisited. Clinical data concerning 1513 ICIs performed, in the last ten years, in 1038 patients have been considered. Various vasoactive drugs have been utilized. Mainly papaverine and prostaglandin E1 are usually employed. The experience and the establishment of the modalities of use both as diagnostic tool and as therapeutic procedure of this particular type of stimulation of the intracavernous vascular system has greatly reduced the possibility of complications and collateral side effects.

[Bladder hernia: a clinical case].

The Authors report a case of inguinal bladder hernia and briefly refer about the different kinds of this rare pathology which is often associated to the commonest inguinal hernia. They, moreover, point out the surgical treatment which has to aim to the repositioning of the bladder and to solve the cervicovesical obstruction.

Coenzyme Q10 levels in human seminal fluid: diagnostic and clinical implications.

The levels of Coenzyme Q10 (CoQ10) were determined by HPLC in seminal fluid samples obtained from 77 patients who performed a standard semen analysis for infertility, previous phlogosis or varicocele. CoQ10 was determined in total seminal fluid (n = 60), in seminal plasma (n = 44) and in the cell pellet (n = 37). The molecule, in total fluid, showed a linear correlation with sperm count and motility. In the pellet of spermatozoa, a trend toward an inverse correlation between CoQ10 (expressed as ng/10(6) cells) and semen parameters could be observed. A different pattern was shown in varicocele patients, in whom, in total fluid, the correlation between CoQ10 and sperm count was preserved, but the one between CoQ10 and sperm motility was lacking; moreover, a higher proportion of CoQ10 was present in seminal plasma, and the inverse trend between cellular CoQ10 and sperm count and motility was not observed. These data suggest a pathophysiological role of ubiquinone in human seminal fluid and a molecular defect in the spermatozoa of varicocele patients.

Papillary cystadenoma of the epididymis as a cause of obstructive azoospermia.

We report a case of infertility due to obstructive azoospermia determined by papillary cystadenoma of the epididymis. The 28-year-old patient had other signs of von Hippel-Lindau syndrome and died from neurological complications. The possibility of finding an epididymal neoplasm should be kept in mind in any case of azoospermia with epididymal enlargement.

Surgical treatment of varicocele at adolescence.

The problem of the surgical treatment of varicocele at adolescence is discussed. We tried to establish a good method for surgical operation capable eliminating the renal-spermatic reflux, the stasis of blood into the pampiniformis plexus with the increase of temperature and the altered metabolic exchanges with the spermatogenetic tissue. The technique of high ligation and division of the spermatic veins with Bilateral Scrotal Exploration (BSE), spermatic venography, microsurgical excision of dilated veins of the plexus and eversion of vaginalis membrane in order to avoid the frequent appearance of postoperative hydrocele is presented in a group of 150 adolescent patients.

Azoospermia. Surgical prospectives.

For a long period of time, azoospermia has been considered a condition with poor therapeutical possibilities. Even in successful cases the number of pregnancies has been very low. With the new techniques in artificial insemination and fertilization there is new interest in treating azoospermic patients. A complete diagnostic procedure should be performed. The use of bilateral scrotal exploration and testicular biopsy allow better chances of diagnosis and treatment. More than 25% of azoospermic patients observed in an Andrological Centre could be surgically treated. Positive results were obtained in 80% of cases submitted to surgery.

[Life habits and impotence].

The authors propose their experience of 595 men complaining of erectile dysfunction. Relationships among erectile dysfunction, taking specific medicaments (hypotensive agents) and habits (smoking) are considered. A collaboration among specialists in the management of impotent patients has been proposed.

[Limitations and complications of the use of the basket].

During the last 15 years the Dormia stone dislodger has been used in 352 cases (80%) out of 435 of ureteral stones: 318 were at ileo pelvic, 34 at lumbar level. 303 ileo pelvic and 21 lumbar stones have been successfully removed (92%). Considering that the probability of stone removal is directly proportional to the ureteral expansibility and indirectly proportional to the volume of the stone, the Basket technique isn't indicated: When the stone is more than 1 cm.; When the ureter is widely stiff (as in Ormond d. or post-radiotherapy stiffness); When stenosis is present. The impossibility of bypassing the stone, the insufficient expansion of the Basket, the stone irremovability and the difficult extraction through the meatus, my limit the successes of this technique while iperating. The immediate complications have been: 4 sectoral rips of the mucosa surrounding the ureteral end, cured by catheterisation without aftereffects (1.2%); 1 sliding of the ureter through the trigone up to urethral meatus (in a woman), which has been repaired, without aftereffects, by direct catheterisation of the ureter, out of the urethra, after removal of the Basket and stone from the ureteral extremity. (0.3%) Strictures occurred in 5 cases (1.5%) and vescicoureteral reflux in 6 (1.8%). These results of the sole Basket technique (performed according to the well-known directions) seem to be much better than those (which have been illustrated) obtained with the endoscopic lithotripsy.

Spermatic cord liposarcoma in a young boy.

We report on a spermatic cord liposarcoma arising in a 16-year-old boy. Treatment consisted of radical orchiectomy without retroperitoneal lymph node dissection, and adjuvant radiation and chemotherapy. The patient has survived 14 months without evidence of recurrence. Paratesticular localization of liposarcoma in patients less than 20 years old is rare. The tumor has no particular characteristic. Treatment consists of radical orchiectomy and the role of retroperitoneal lymphadenectomy is controversial.
