Left varicocele at puberty.

A study was made of 160 cases of left varicocele treated during 1979-1980; there were 35 patients under sixteen years old. Also 74.3 per cent of patients already had mono- or bilateral testicular hypotrophy. Testicular biopsy revealed 90 per cent had histologic lesions of varying nature and degree. We believe varicocele formation is related to physiologic changes during puberty. These data suggest that early recognition of varicocele and its correction before the appearance of irreversible testicular lesions may prevent male sterility.

Validity of the pre-operatory trans-scrotal venography of the internal spermatic vein in the presence of idiopathic varicocele.

The Authors refer their experiences regarding the use in the preoperatory phase of the trans-scrotal venography of the left internal spermatic vein in the presence of idiopathic varicocele. The use of such a method, described in detail, and the study of radiograms obtained allow to acquire important notions of varicocele. In this way, it is possible to select the proper surgical approach to be employed in the treatment of varicocele syndrome.

Response of adnexal glands to testosterone stimulation in the normal adult male.

Normal male volunteers were studied in order to set up a functional exploration test based on administration of testosterone propionate for use on the adnexal glands of the male genital tract. Seminal acid phosphatase and citric acid levels showed no significant variations compared with spontaneous physiologic oscillations. Concentration and total quantity of fructose were considerably reduced. Alkaline phosphatase showed an intermediate pattern with constant values of concentration and decrease of total quantity. The hypothesis is advanced that adnexal glands possess a secretory metabolism with a maximal basal response to androgen stimulation, but with a different mechanism of action: partly adaptable at the level of the prostate and of the "all or nothing" type at the level of the vesicles.

Coenzyme Q10 concentrations in normal and pathological human seminal fluid.

Coenzyme Q10 (CoQ10) levels were assayed in total seminal fluid or both in seminal fluid and seminal plasma in 77 subjects with normal or pathological findings at standard semen analysis. CoQ10 levels showed a significant correlation with sperm count and with sperm motility. An interesting exception was constituted by patients with varicocele, in whom the correlation with sperm concentration was preserved, whereas the correlation with sperm motility was lacking. Moreover, they showed an increased ratio of plasma CoQ to total seminal CoQ10 in comparison with the other subjects. These data suggest a pathophysiological meaning of CoQ10 in human seminal fluid and a possible molecular defect in varicocele patients. CoQ10 measurement could represent an important examination in infertile patients; moreover, from these results a rationale might arise for a possible treatment with exogenous CoQ10 in dyspermic patients.

Is it useful to operate on adolescent patients affected by left varicocele?

In order to consider the possibility of avoiding seminal disorders induced by varicocele at puberty, we evaluated 200 patients who were operated on for left varicocele at adolescence. Seventy-five of our 200 adolescent patients had evaluations of seminal fluid prior to their varicocelectomy. Of these patients, 75 were re-evaluated at the age of 21. In this latter group of patients, we observed 24 cases of hydrocele (32%) and 9 cases of varicocele recurrence (12%). These data were compared to 75 patients first presenting with a varicocele at 21-25 years of age. If we exclude the cases with azoospermia, we observed the presence of altered seminal fluid in 35 (46.7%) of the operated at adolescence patients and in 42 (56%) of the adult patients with varicocele. These data support the hypothesis that operating on patients in adolescence may result in maintenance of fertility potential.
