White piedra and Trichosporon species in equatorial Africa. III. Identification of Trichosporon species by slide agglutination test.

Fifty-two Trichosporon strains isolated from Gabonese female patients 15-60 years, were studied. The identity of these strains was established by two different methods: the method proposed by Guého et al. (1992), based on mycological criteria, and a slide agglutination method performed with monospecific antisera prepared in our laboratory. The final results show a perfect correlation between the two methods, which allowed us to identify 25 strains of T. mucoides, 21 strains of T. inkin and seven strains of T. asahii. The results of the agglutination tests performed with 24-h-old subcultures grown on Sabouraud glucose agar are available in less than 15 mins. In the light of these results, it appears that this method, which is rapid and easy to perform and reproduce, may readily be used in hospital laboratories. In addition, this method allowed us to verify the presence of antigens common to the genera Cryptococcus and Trichosporon, which were easily shown by the use of crude sera. The fact that the anti-Trichosporon monospecific sera lose their capacity to agglutinate Cryptococcus neoformans proves their high specificity.

[Drepanocytosis and navigation personnel].

In West and Central Africa one adult in 4 or 5 carries AS trait. This haemoglobin abnormality therefore underlines the question of aptitude of navigation personnel in national or international air-lines. Among these personnel one should separate commercial navigation personnel (CNP) and technical navigation personnel (TNP), if the aptitude in flight of CNP may perhaps be acquired without systematic previous submission to files of Civil Aeronautics Medical Advice, on the other hand a definite and irrefutable decision on the aptitude of TNP can only be envisaged after proving tests for placing the personnel carrying sickle cell trait in the context of airling work.

[Human trichinosis. A recent epidemic in Vientiane (Laos) (apropos of 32 cases)].

Fifty patients were hospitalised at Vientiane during May and June 1975. Infection was caused by consumption of pork meat (som-mou, lap mou and lap leuat). After an incubation period of 8 to 11 days intermittent diarrhoea followed by constant fever and orbital oedema occurred. Several days later diffuse myalgias occurred. Three patients presented neurological symptoms: left hemiplegia flexible paraplegia and limb paresis; these symptoms regressed spontaneously. 87% of the clinically suspected cases had specific serum antibodies (IFI, ID, IE). Hyperleucocytosis and hypereosinophilia were a constant factor. Anti-AH antibodies were detected by the Widal test. The histopathology characteristic of myositis was seen in the 12 muscle biopsies. Also observed was the precystic state of the larvae which is in agreement with the early timing of the samples which were obtained during the 3rd and 4rd week of infection.

[Use of peroxidase markers in the research and diagnosis of paludism].

The indirect peroxidase labelled technic for the diagnosis and research in malaria rests on the same principle than immunofluorescence test (I. F.). Homologous or heterologous antigens are incubated with patient serum in the same conditions than for I. F., and then with peroxydase, labelled, conjugate diluted 1/200. The resulting complex is colored by the diaminobenzidine method of Graham and Karnovski. The coloration is observed in light microscopy. This technic as sensitive as immunofluorescence test can be done, with a cheaper microscopic material. The possibility of studying the conjugate fixation in electron microscopy opens new direction for research.

[Heminoglobinopathies of young adults migrating from the Antilles to the Metropolis (apropos of 1,000 cases of electrophoresis)].

The systematic research of abnormal haemoglobins has been done on 1,000 subjects of both sexes (mostly 18 to 35 years) from Guadeloupe (500) and Martinique (500), examined in Paris. The following were found without significent difference in both islands : 86,9% heterozygote AS, 4,5% heterozygote AC, 0,3% double heterozygote SC, one homozygote CC, 0,3% increase of the fraction A2 and one increase of haemoglobin F. Those results were comparable to the few anterior studies in French West Indies.

[Enzymatic and serologic study of human trichinosis. Apropos of a recent epidemic in a suburb south of Paris].

An enzymatic and immunologic study of 18 patients with trichinosis leads to the following conclusions: The stage of muscular invasion in trichinosis is accompanied by a release of cellular enzymes representative of striated muscle fibres in nearly all the cases. This release can be observed by a study of the LDH iso-enzymes at a time when immunological techniques are not always significantly positive. The specific aspect of this phenomenon can be proposed with reservations since there does not exist any interference with other enzymatic systems such as the gamma-GT and furthermore no other evident cause of muscular lysis is present. The existence of a blood hypereosinophilia completes the biological picture. These early modifications of the enzymatic activities are most probably transient.

[Breast-feeding and intestinal parasites].

The extent of intestinal parasitic infection was determined using 4 methods of fecal examination in 400 Gabonese infants aged 0.5 to 24 months (86 exclusively and 151 partially breast-fed and 163 artificially fed) and in 170 of their mothers in an African urban community. Parasitic prevalence was the same in the stools of the mothers from the 3 infant groups, providing evidence for the same level of exposure to parasites in all infants. Exclusively and partially breast-fed infants excreted parasites in numbers significantly lower than artificially fed infants of the same ages, during the first and the second years of life. This antiparasitic activity of human milk concerned mostly Giardia and ascaris, was not exclusively due to a decreased exposure to parasites in breast-fed infants and did not seem exclusively antibody-mediated; the role of lipids is possible.

[Curative treatment of malaria Plasmodium falciparum, P. vivax and P. ovale malaria with mefloquine].

Mefloquine (WR 142-490, RO 21.5998), and antimalarial 4-quinoleine-methanol, active on multiresistant strains of P. falciparum is a resourceful product because of its pharmacocinetics and the possibility it opens of a single day curative therapy. Its mean half-live is 15 days, with important individual variations from 8 to 23 days as well as racial one within North-American, Asiatic or African patients. A 1,25 to 1,50 g dose divided in 2 or 3 intakes during 16 hours has proved to be effective in 34 P. falciparum cases. Lower doses, near 1 g seem sufficient for P. vivax (6 cases) and P. ovale fits (4 cases). Clinical and biological acceptance are good, according to this limited study. The use of mefloquine might nowadays be reserved for geographical areas where resistance to P. falciparum is significantly high. Its diffusion in Africa, south of the Sahara, though conceivable, might be discarded for the time beeing.

[Prevalence of human filariasis with microfilaremia in Gabon].

Direct examinations of blood and serologic tests were used to assess the intensity of the filarial endemicity in Gabon. Both methods revealed that the prevalence of filariasis with microfilaremia (Loa loa and Dipetalonema perstans) is presently very elevated. The areas that are more involved are the forest regions as well as the highest ones (ogoue, Lolo, Ngounie). Women appeared to be suffering from loasis to a lesser extent than men. On the other hand, the prevalence was identical for Dipetalonema perstans. For the gabonese population one can approximately say that 3 out of 5 adults patients have filariasis, two being D. perstans and one L. loa. Consequently these affections represent a major public health problem. Finally one must note that no case of lymphatic filariasis has been identified.

[Five cases of chromomycosis in Gabon (author's transl)].

Five cases of chromomycosis were studied in the parasitological and mycological department of the National Laboratory and C.U.S.S. in Libreville (Gabon). Different types of lesions were observed which were more often maculous than verrucous. In one case, it was localized on the upper limb, and in four cases on the lower limbs. They all were diagnosed after a 2 to 25 years evolution. Fonsecaea pedrosoï was isolated in every case. Direct examinations always recovered the classical scleratic cells : in one case it was budding, and in two cases abundant mycelium was seen. No epidemiological argument can explain the geographical distribution.

[Mebendazole and intestinal nematodiasis in Gabon. Tolerance and efficacy in single and multiple doses].

Mebendazole was given following the classical regimen i. e. a three days course with a single intake per day for patients suffering from intestinal nematodosias in Gabon. Efficiency which is remarkable on this three days basis proved to be insufficient however for ankylostomiasis with a single intake per day. Tolerance is excellent on any therapeutical regimen or with any type of survey considered.
