[The course of pregnancy in lupus anticoagulant syndrome].

More than 95 per cent of the pregnancies with circulating lupus anticoagulant (LA) without therapy lead to abortion or intrauterine death. We report on the course of a pregnancy in a patient, who developed deep venous thrombosis and circulating LA. One of the rare cases of successful pregnancy management without the common prednisolone-aspirin therapy, but with heparin-monotherapy associated with intensive medical and obstetrical surveillance under clinical conditions is reported. In spite of intermittent corticosteroid and immunoglobulin application, a healthy child was born, although normalisation of the aPTT and platelet count was not achieved. It seems, that an alternative concept of therapy may have been established.

[Ultrasound anatomy of the liver].

The authors review the modern concept of liver anatomy, related to the needs of surgeons in segmental resections. Important anatomical structures are described participating in division of the liver into four basic segments. The ultrasound anatomy of portal system, hepatic veins, hepatic fissures and liver hilus are dealt with in detail. Criteria for perfect examination of the liver by ultrasound are defined and representability of key anatomical structure under optimal conditions is analyzed in a group of 100 patients. The only non-constantly represented structure was fissura interlobaris (44%). Hepatic veins proved to be the most variable structures (17%). The results show that segmental anatomy of the liver may be very well represented by ultrasound. The authors discuss the importance of perfect knowledge of anatomy for quality examination.

[Struma ovarii, a rare ovarian tumor].

Teratomas, which consist only or predominantly of thyroid tissue, are termed struma ovarii. Two cases are reported, where after previous strumectomy the histologic diagnosis was struma ovarii. Because of postoperative TSH-rise hormonal activity can be assumed. The proper diagnosis is seldomly made preoperatively, this could be done by abdominal scintigraphic imaging of radioiodine uptake in the tumour.

[Dynamic and duplex ultrasonography].

Dynamic ultrasonography is a diagnostic ultrasound examination which makes possible non-invasive visualization of soft tissue structures of the neck and its large blood vessels. Duplex ultrasonography makes moreover aimed assessment of the blood flow in major vessels possible and thus supplements in a significant way the echotomographic picture. Because of its non-invasive character it is preferable to CT and angiography in diagnostic work.

[Duplex sonography. Examples of its use in otorhinolaryngology].

Duplex sonography not only provides the ENT specialist with valuable information on topographic relations of soft tissue tumours with the main blood vessels of the neck but provides also information on the blood flow in these vessels. It makes it possible to draw reliable conclusions on vascular anomalies or dislocation of the vessels, on affections of the vascular wall, on restricted patency of vessels due to the action of a tumour (compression, penetration) or inflammation (thrombus) or changes in the flow through the jugular vein conditioned by impaired passage in the S-shaped sinus. In some instance ultrasonic examination is a valuable method for evaluation of the dynamic and further development of the basic disease and can replace classical arteriography or phlebography.

[Primary hyperaldosteronism (Conn syndrome) and pregnancy].

Because of the atypical symptomatology, which can mimic an EPH-gestosis, the physiological elevation of serum aldosterone in the third trimester and restricted diagnostic possibilities, the diagnosis of primary hyperaldosteronism during pregnancy is difficult. A case of an adrenal adenoma during pregnancy is reported.
