The authors examined the action of fine newly synthesized xanthine derivatives on the activity of the enzyme 3',5' AMP phosphodiesterase (at high substrate concentrations) in homogenates, obtained from the brain lung and myocardium of a rat. The compounds were compared with theophylline for their activity. They showed low inhibiting activity in brain homogenates in contrast to the manifested activity of some of them in lungs and myocardium. Especially manifested difference was observed for the compound with a code 1t which was find times stronger inhibitor in the lung than in the brain. The authors make an inference that some of the compounds manifect selective activity as inhibitors of the enzyme phosphodiesterase and in this sense the search for xanthine derivatives with specific activity is prespective.
Six newly-synthetized 7-N-basically substituted xanthine derivatives were studied for inhibitory action on phosphodiesterase with high Km in homogenates from brain, lung and epididymal fatty tissue. Theophylline is used for comparative purposes. The effect of part of the compounds on lipolysis in vivo is also studied. Some of the newly-synthetized compounds are found to manifest a marked ability to inhibit the enzyme studied in brain and lung homogenates, and compound with code V has equal action to that of theophylline. The newly-synthetized compounds are poor inhibitors of the enzyme in epididymal fatty tissue, unlike theophylline which demonstrates the same inhibitory capacity as in the other homogenates. These data correlate with the results obtained when studying lipolysis. Theophylline in a dose of 50 mg/kg i. p. increases twice the serum content of the free fatty acids, while the newly-synthetized compounds have no effect on it, even when applied in high doses.
The authors examined the activity of the enzyme phosphorilase in the myocardium of rats with spontaneous hypertension of the Okamoto Aoki type and of Wistar rats with experimental metacorticoid hypertension. They found higher basal activity of phosphorilase "a" in the both groups of hypertonic rats. After treatment of rats with isoprenaline in a dose of I mg/kg, administered intraperitonealy, the phosphorilase "a" in the myocardium of hypertensive animals was activated to a degree equal to that of the normotensive rats. This correlated also with the maximal isoprenaline chronotropic response in the both groups of hypertensive rats. The authors discussed the possible causes and mechanisms for the observed changes and effects.

The study of the interrelationship between the chemical structure of the drug molecule and their biologic activity is one of the most modern aspects of current pharmacology and pharmacochemistry. The information, obtained by these studies, contributes for more precise and detailed understanding of the mechanisms of drug molecule interaction with some biologic structures, and this helps to clarify a series of biologic and physiological phenomena. The authors describe in this paper some methods, which are used, when the dependences between structure and action are search for as well as the basic physiocochemical and quantum -- chemical values, which determine the pharmacological effect of drugs.
The authors studied the incidence and character of cardiac rupture in 109 deceased of myocardial infarction. Rupture was encountered in 21.1 per cent. Out of the 23 ruptures -- 21 were external and 2 -- internal at the intraventricular septum. Cardiac rupture is an occurence mainly in subjects over 60. Extensive transmural infarction was diagnosed in all cases of external rupture. The diagnosis was made while still alive in a little more than a half of the cases. In seven of the cases, the external rupture was with a protracted progress with the following clinical manifestations -- preceeding strong, refractory, continuous pain and anxiety, protracted shock state, prolonged pericardial friction. Characteristic ECG changes are described in case of protracted ruptures enabling the admittance of the rupture while still living. The authors express the opinion that in cases with protracted rupture, surgical life-saving intervention is possible.
The authors investigated the pharmacokinetics of the pressor substance 5,8-dimetoxy-trans-2-/2-hydroxyethyl/-amino-3-hydroxy-1,2,3,4-tetrahydronaphthaline hydrochloride (1b), which possessed strong pressor action. Blood concentrations after venous administration in a dose of 1 mg/kg of body weight were determined by means of exctration spectrofluorometric method. The data complied with the biexponential equation, meeting the requirements of an open two-compartment model. There was a quick elimination of the substance from the central compartment, which correlated also with the data for quick disappearances of the effect on the arterial pressure.
The authors examined 90 patients with myocardial infarction with a view to the lung changes developed and especially to the manifestations of pulmonary stasis. Series of face roentgenographies of the lungs were made of the patients in lying down position. In 76,7 per cent of the patients X-ray findings of pulmonary stasis were found. The dilated veins in the upper lung lobes are the earliest and most frequent manifestations, suggesting congestion. They are found in 23,3 per cent of the cases. Most important, among the X-ray findings, according to the authors, are those of perivascular edema--hilus images with widened and indistinct lines (24,4%). The X-ray findings of lung congestion and interstitial edema are early symptoms of pulmonary edema, in case the physical data from the lung examinations are negative. The therapeutic measures, at that stage, could prevent the development of a severe and very often fatal edema of the lungs.
