Ocular manifestations of syphilis.

Since the mid-1970s, the number of new cases of infectious syphilis has begun to increase. Ocular manifestations of secondary and late syphilis occur. The optometric role in the management of infectious syphilis is one of detection, consultation with internists and neurologists for systemic treatment, topical treatment of anterior uveitis, and monitoring for recurrence of disease. This report describes an elderly patient presenting with recurrent, bilateral iritis and optic atrophy as initial clinical manifestations of syphilitic infection.

Aged

Vision recovery in amblyopia after contralateral subretinal hemorrhage.

Amblyopia is most effectively treated, in most cases, during the first and second decade of life. Generally, the earlier the treatment, the higher probability of success. The frequency of vision loss in the good eye in patients with monocular amblyopia is between 0.1 and 0.2%. This case report describes a 65-year-old male with successful recovery of vision in the amblyopic eye after a subretinal hemorrhage occurred in the better eye.

Age Factors

Visual dysfunction in closed head injury.

Damage to the visual system often occurs after head injury. Clinical manifestations of visual system dysfunction may be monocular or binocular, and may include reduced visual acuity, visual field loss, and in some cases complete blindness. This paper reviews and updates nine cases of visual dysfunction secondary to closed head trauma. Pathophysiology, prognosis and optometric co-management with neurology and neuro-ophthalmology are highlighted. Optometric roles in visual rehabilitation and the prevention of head injury are addressed.

Ocular chrysiasis.

Elemental gold, administered parenterally and orally, is used to a moderate extent to treat rheumatoid arthritis, Sjögren's syndrome and nondisseminated lupus erythematosus. Deposition of gold salts are observable in ocular tissues during chrysotherapy. These tissues are the cornea, conjunctiva, skin, lens and retina. This case report describes ocular chrysiasis and discusses its clinical management.

Betaxolol and levobunolol: new beta-blocking antiglaucoma agents.

The Food and Drug Administration has recently approved the use of two new ophthalmic beta-adrenergic antagonistic agents: betaxolol hydrochloride (Betoptic) and levobunolol hydrochloride (Betagan). This paper reviews the history, pharmacologic properties, clinical efficacy and potential side effects of this expanding class of antiglaucoma medication.

Basal cell epithelioma.

This report describes a basal cell epithelioma found on the lid of an unusually young white male. The epidemiology, pathophysiology and histology are discussed outlining clinical implications. Clinical appearance and management is reviewed highlighting complications. Conclusions are that age is not an excluding factor in the differential diagnosis of basal cell epithelioma and that any anomalous growth in the head and neck areas should be treated as suspicious until proven otherwise by a biopsy.

The reinforcement of calm sitting behavior: a method used to reduce the self-injurious behavior of a profoundly retarded boy.

The frequency and intensity of a 14-year-old profoundly retarded male's self-injurious behavior was significantly reduced by the reinforcement of calm sitting behavior. Social validation ratings confirmed the effectiveness of treatment, and showed that gains generalized across treatment and non-treatment settings and across people associated or not with treatment. Treatment gains were maintained at 6- and 12-month follow-ups.

Ophthalmic disodium cromoglycate.

Disodium cromoglycate has recently been approved for ophthalmic treatment of certain types of conjunctivitis in the United States. This mast cell inhibitor is effective in the treatment of vernal keratoconjunctivitis, allergic conjunctivitis, chronic conjunctivitis, and giant papillary conjunctivitis, especially when a history of atopic disorders or moderately low blood IgE levels are present. This literature review provides a foundation for understanding the balance between the therapeutic efficacy, clinical benefits and side effects in treating IgE-mediated conjunctivitis with disodium cromoglycate.
