We have described a 50-year-old woman with periampullary carcinoma with extensive infiltration of the duodenal wall arising from advanced tumor of the pancreatic head. Endoscopic sphincterotomy induced complete relief from jaundice and pruritus before definitive surgical therapy was done. This procedure should not be restricted to small tumors confined to the ampulla itself, since it may also be of palliative or preoperative value in certain patients with advanced cancer of the pancreatic head.
Comparative haemodynamic investigations with carvedilol and verapamil were carried out on conscious, instrumented dogs. Arterial blood pressure, right atrial pressure (RAP), cardiac output (CO), heart rate, stroke volume and total peripheral resistance (TPR) were determined after intravenous (i.v.) injection of incremental doses (0.01-3 mg/kg) of the drugs. Carvedilol reduced the blood pressure in a dose-dependent manner, concomitant with a reduction in TPR. The RAP and the CO were not affected, indicating that arterial vasodilatation was induced by carvedilol. Verapamil showed a decrease in the blood pressure with a reduction of CO and SV. Moreover, at high doses the RAP was increased, indicating a reduction of the cardiac performance. Thus, in our experimental model remarkable differences between the haemodynamic effects of i.v. injections of carvedilol and verapamil have been observed, whereas after oral administration blood pressure also decreases after verapamil due to a reduction of TPR.

A case of gastric perforation due to a gastric metastasis of a primary bronchogenic carcinoma is presented. While ten cases of small intestinal perforation secondary to bronchogenic tumor spread were identified in the literature, there appears to be only one further report on a gastric perforation caused by bronchogenic carcinoma.
This paper reports on the development, technical improvements, and problems occurring in the application of new morphological techniques--mainly electron microscopy (SEM and TEM)--in legal medicine. By means of examples, preparation techniques are explained with regard to the problems involved and artifact interference caused by unsuitable preparations. The increasing automatization of sample preparation and examination and the progress in microelectronics have together brought about a reduction in personnel costs and increased efficiency of the instruments, as well as a reduction in prices. The question of costs is discussed. Anyone using these techniques must have considerable experience.
A method of investigation on non-decalcified bony tissue is discussed. This method combines the advantages of micro-angiological procedures with those of osteological investigations. By use of this method, a good assignment of angiological results to the differential interference contrast film is possible. The application is indicated for particular osteomyelitis types that are rich in blood vessels, and for some bone tumours.
A 75-year-old woman with multiple polypoid lesions in the gastroduodenum , and one large gastric ulceration all representing large-cell type carcinoma is described. Radiologic and fiberbronchoscopic , together with other clinical findings suggested bronchogenic carcinoma as the primary tumor. Metastatic gastric involvement is an uncommon feature of primary bronchogenic carcinoma, rarely attaining clinical significance. In our patient, gastric symptoms represented the initial clinical manifestation of hitherto undetected bronchogenic cancer disease.
In the period between 1970 and 1982, 6 carcinomas in scar tissue and 2 pseudocarcinomatous skin papillomatosis were found on the basis of chronic osteomyelitides. Amputation specimens of two carcinomas and one papillomatosis were examined by arteriography. Macroscopically carcinomas in scar tissue showed an unspecific appearance and microscopically a scarefied and partly pathological vascular appearance. Pseudocarcinomatous skin papillomatosis was distinctly hypervascular. Therefore, indications with regard to the localisation of an required exploratory excision can only be obtained from the vascular appearance of papillomatosis but not in case of a scar tissue carcinoma after osteomyelitis. Due to the overall bad blood circulation osteomyelitic scar tissue carcinoma scarcely reacts positively to a chemotherapeutic treatment.
Kienböcks disease is one of the most frequent manifestations of avascular necrosis. Multiple different surgical and conservative methods for treatment have been described. The transposition of pisiform on its vascular pedicle to replace the reamed out necrotic portion of the lunate has permitted revascularization and healing of the lunate. In the following article, the operative technique is described and the results of a five year follow-up study on 32 patients who underwent this operation are presented. On the basis of this study, we found, even in advanced cases of Kienböcks disease, that 50% of the 14 followed-up patients became free of symptoms or had pain only under stressful conditions.
The following bibliography of SEM - Application is largely oriented to medicolegal studies. Avoiding overlappings with criminalistic research papers were not always possible because the objects of inquiry as well as the forensic questions often are identical, such as investigations on hair or gunshot analyses. One important aspect could not be considered by us: the contact of principal medicolegal problems with those pathological, for example in structural changes of vessels after traumatization (Forensic Medicine: vital reaction - Pathology: thrombosis, hemostasis, wound healing). Publications have not been selected with regard to their relevance. Finally we have listed the problems and methods researched.
The fine-structural analysis of injuries and injury-related damages of the arteries in the head and neck region, such as vital endothelial injuries resulting from death by hanging, is of considerable importance in legal medicine, as it involves numerous forensic problems. The problems and difficulties of vessel preparation, including the occurrence of artefacts, have been widely discussed in medical journals, indicating the necessity for a special method of preparation to be developed. This method has to take individual differences of the autopsy material into consideration, including osmolarity changes caused by hypoxia, and autolysis. The method developed by us and described here meets these conditions. Optimization and standardization of the method was achieved by perfusion fixation of the vascular system in situ. The particular characteristics of the vessels involved made it necessary to open the skull. After removing the upper half of the brain and applying a partially permeable sealing to the skull base, we proceeded with the perfusion, using slightly hyperosmotic perfusion media (450-680 mOsm) via a hydrostatic system, and keeping pressure and flow rates low (max. 65 cm H2O). The perfusion technique we developed, and which is described in detail, has proven suitable for the preparation of other vessels as well, which is demonstrated on the venae cerebri superiores. The method of preparation was designed to provide conditions for a routine application of SEM, LM and TEM for forensic purposes.
Casuistic contribution about a 47 years old man, on whom we diagnosed, after an accident, radiological, mikroradiographical and histological on Osteopoikilia. Our examinations of this man and of his two sons (also with Osteopoikilia) confirmed with the medical literature, that the anomaly of the bone structure has no value of disease, neither of the skeletal system. The bonehealing of the father also showed no special problems, especially no delayed healing.
