Urogenital infection due to meningococcus in men and women.

The authors report six cases of acute urethritis and three cases of acute cervicitis caused in all instances by Neisseria meningitidis group B. The patients, five heterosexual men, one homosexual man, and three female prostitutes were seen at a venereal clinic in Mexico City. All of them were initially diagnosed as having gonorrhea. Treatment with procaine-penicillin G cured all cases, both clinically and bacteriologically. These experiences reinforce the need to distinguish N. meningitidis from N. gonorrhoeae in the setting of sexually transmitted diseases.

[Comparative study of inhalation induction by vital capacity breath in adults using 6% sevoflurane with oxygen or 4.5%sevoflurane in 50% nitrous oxide].

OBJECTIVE: To evaluate the efficacy, side effects and hemodynamic characteristics of induction by vital capacity breath in adults using 6% sevoflurane and oxygen versus 4.5% sevoflurane and 50% nitrous oxide. PATIENTS AND METHODS: We assigned 50 ASA I-II patients aged 20 to 70 years old randomly to two groups of 25 to receive either 6% sevoflurane in oxygen or 4.5% sevoflurane in nitrous oxide. All patients were premedicated with oral bromazepam (1.5 to 3 mg). Induction was by vital capacity breath using a Mapleson A circuit (8 l. min-1) for 5 min. We recorded induction time, side effects, hemodynamic variables and patient opinion after surgery. RESULTS: Induction time was significantly faster for the sevoflurane-oxygen group (60 +/- 10 s) than for the sevoflurane-nitrous oxide group (71 +/- 8 s) (p < 0.001). Complications were minor and hemodynamic variables stable in both groups, with no statistically significant differences. The patients expressed satisfaction with both induction techniques. CONCLUSIONS: A vital capacity breath of 6% sevoflurane provided rapid induction. Induction was no more rapid when 50% nitrous oxide was added and the incidence of side effects did not decrease. Hemodynamic variables are stable during induction with sevoflurane with or without nitrous oxide, making this a well-tolerated alternative technique that is positively evaluated by patients.

[Prospective study of patients with prolonged fever].

A prospective study was made in 283 patients who attended IMAN's Children's Hospital, with fever the main symptom. A clinical and paraclinical procedure was designed for the study of each patient. 112 patients were eliminated because they did not follow the established criteria. All patients had acute infectious diseases considered trivial; 85% were 3 weeks to 2 years of age. They all had an antibacterial treatment without precise diagnosis. It was considered that on admission the patients showed a normal course in the natural history of the basic disease. The study group included 171 patients 2 months to 13 years of age; 62.5% had fever due to infection, 12.2% to collagenopathies, 7% to neoplasias 5.2% to miscellaneous causes and 12.8% were not diagnosed. The most common infectious causes for prolonged fever were tuberculosis, upper respiratory infections, amoebic liver abscess, typhoid fever and malaria. Careful questioning and clinical examination were enough to enlighten diagnosis in more than 80% of the patients.

[Infectious hepatitis. I. Presence of HBs antigen].

A prospective study in 268 patients of different pediatric ages affected with icteric hepatitis is presented, with a longitudinal follow-up of one year minimum. Different types of clinical evolution are described and related to the presence of HBs antigen. In 34 of the 268 patients HBs antigen was positive; in 20 of 28 patients with acute and long evolution, positivity of the antigen was transitory with an average of 46 days; in the remaining 8 of 28 patients it extended from 6 months to less than 2 years. The presence of HBs antigen is a risk that may be correlated with the tendency to extend the prolonged.

[Epidemiological aspects of gastroenteritis dut to Rotavirus].

During December 1976 to January-March 1977, 13/53 children less than 5 years of age were admitted with acute enteritis caused by rotaviruses to the Hospital del Niño, DIF. There is already convincing evidence that this new virus will prove to be the most important etiological agent of sporadic acute enteritis in young children.

[Existence of 2 types of Rotavirus associated with acute gastroenteritis in children].

Rotaviruses found in the stools of different infants, hospitalized in the Hospital del Niño del DIF, may be grouped into at least two types according to their RNA composition. The viral RNA composition was analyzed by electrophoresis in agarose gels. The different types can be distinguished by the mobility of the second larger of the 11 segments that make up the viral RNA.

[Diagnosis of Rotavirus using viral RNA electrophoresis].

Rotavirus has emerged as a major agent of acute enteritis of infants. A rapid and simple technique has been developed for its detection, based on the visualization of the viral RNA electrophoretic pattern. This technique takes advantage of the large amount of rotavirus present in the stools and the high sensitivity for detection of RNA obtained with ethidium bromide.

[Pleuropulmonary lesions due to Haemophilus influenzae].

Empyema due to Hemophilus influenzae has been reported only rarely. This report describes the cases of ten children with empyema due to this organism. Diagnosis was made through growth of the organism in pure culture from pleural effusion. H. influenzae empyema may occur in older children as well as in infants and, in contrast to certain other infections with this organism, empyema due to H. influenzae may be more common than is generally suspected.

[Intracranial abscesses in children].

An analysis of 23 consecutive cases of brain abscess is reported. The most frequent early symptoms and signs were headache, vomiting, papiledema, focal neurological abnormalities and pyrexia. Roentgenogram of skull, electroencephahologram and ecoencephalogram were abnormal in all cases in which they were obtained. Therapy was based on early diagnosis with accurate location, aspiration or excision of the abscess. Lumbar puncture is hazardous and yields only suggestive information.

[Finding of anaerobic bacteria in blood cultures].

Out of the blood cultures sent to the Bacteriology Laboratory of Hospital del Niño DIF, during the period of one year, in 1.4% of samples, isolation of anaerobic bacteria was obtained. Isolation was more frequent in infants under one year of age, but specially in newborns. The species most frequently found were Propionibacterium acnes, Bacteroides melaninogenicus and Clostridium subterminale. The presence of this finding in blood cultures of infants is noted, but there is no purpose to establish a clinical relationship with the finding of this group of bacteria.
