[Experimental studies on the effect of the administration of oxygen into the pericardial cavity in acute myocardial ischemia].

Artificial myocardial ischaemia caused by ligation of the left anterior descending coronary artery was treated in minipigs by the application of oxygen into the pericardial cavity. The results show that in this way acute myocardial ischaemia may be reduced. The application of oxygen into the pericardial cavity brought about, within 15 min, an improvement in the haemodynamic parameters. Several electrolytes, which were estimated from the blood of the sinus venosus, returned to normal.

[Perfusion of the ischemic myocardium in the acute phase of a myocardial ischemia using a stroma-free hemoglobin solution].

Concerning the effect of the perfusion of ischemic areas of the myocardium by a stroma-free hemoglobin solution was reported after the starvation of the interventricular branch of the left coronary artery (RIVA) in minipigs. The effect of the perfusion is dependent on the kind of the vascular course. The perfusion of the distal part of the starvated coronary artery shows the better reaction in face of the artemalization of the concomitant cardiac vein with regard to the resaturation of the myocardium with oxygen and several hemodynamic control parameters. Histologic examinations of the ischemic myocardium after the arterialization of the cardiac veins showed however that the symplasm preserved its vitality and showed only reversible changes. The applied stroma-free hemoglobin solution needs a further development. However, the use is possible on principle according to the bibliography. Whether a new clinical therapy could be developed by a transitory arterialization of the cardiac veins in acute myocardial ischemia, should be discussed.

[Sound spectrographic studies in the diagnosis of dysfunctions of mechanical heart valve prostheses (Björk-Shiley prostheses)].

A newly developed real-time sound spectroanalyzer was found to be capable of diagnosing malfunction of prosthetic heart valves (Björk-Shiley prostheses). Sound spectroanalysis was carried out 430 times on 257 patients with a prosthetic heart valve. The valvular click of prosthetic heart valves shows a typical sound spectroanalysis pattern in real time, with a high-frequency peak which diminishes beyond 25 kHz. Thrombosis of the prosthetic heart valve causes loss of the valvular click and reduction in the intensity of the high-frequency components, as shown in 4 patients. Thus the sound spectroanalysis appears to be a noninvasive technique which may allow early diagnosis of a thrombosed prosthetic heart valve. We believe this method to be superior to phonocardiography and echocardiography.

[Dynamics of the heart rate in active orthostasis of patients with hemodynamically effective mitral valve stenosis].

Heart rate dynamics in response to active orthostasis of 10 min duration were examined in 48 patients with mitral stenosis and compared with the mean and diastolic pulmonary artery pressure. Classification of the characteristics of the heart rate was done according Drischel and co-workers (1963) using amplitude and time values. In contrast to healthy subjects, in patients with mitral stenosis the best regulatory behavior was observed in type B with lower pulmonary artery pressure, the least favorable regulatory dynamics being predominant in type A with higher pulmonary artery pressure. From a functional diagnostic point of view, regulatory efficiency can be graded on the following descending scale: (1) the best regulatory behavior in type B; (2) a decrease in the amplitude values in type B; (3) transition of the dynamics to type A; and (4) prolongation of the time values in type A. The pathophysiological mechanisms are changes in the sensitivity of dilatation receptors in the intrathoracic low-pressure system and of the carotid baroreceptors.

[Evaluation of lung function and pulmonary hypertension preceding surgery of acquired heart defects].

We investigated the connection between the pulmonary hypertension and the lung function in the preoperative time of 176 patients undergoing an operation for prosthetic valve replacement in mitral and aortal position and in both of them. Excluding other causes of restriction we found a distinct coherence between vital capacity and residual volume in heart failures of the mitral valve. The findings of patients with aortic valve disease of the heart show the same trend as in the mitral valve, but there are differences of quantity. The changed respiratory mechanics is dependent of the disturbed haemodynamics and depends on the duration of the development of the valvular defect, too.

[Arterialization of the coronary veins as a therapeutic measure in acute myocardial ischemia. I. Experimental studies and morphology in miniswine].

Mortality and morbidity of the acute myocardial infarction show an increasing tendency. The therapy, on the other hand, is not much satisfying yet. A myocardial ischemia was produced in 48 minipigs by ligature of the ramus interventricularis anterior of the left coronary artery. Its satellite vein was arterialised by means of plastic shunt. The majority of the animals of the control group without arterialisation did not survive. Electron microscopical controls showed only minimal damages of the ultrastructures in animals after arterialisation of the coronary veins.

[Arterialization of the coronary veins as a therapeutic measure in acute myocardial ischemia. Experimental studies on the miniswine. II. Cardiothermography and intramyocardial oxygen tension].

The arterialisation of the coronary veins leads in the acute phase of myocardial ischemia to an effective circulation in the telethermographic picture. The oxygen tension in the tissue increases to 46% of the normal tissue tension in this way. The sinusoids have a nutritive function likely. Although the greatest amount of the blood supplied was drained before becoming metabolised a myocardial necrosis was prevented.

[Thermocardiography--a method for continuous intraoperative surveillance].

The thermography of the cardiac surface offers the possibility for the intraoperative control of cardiosurgical measures. Changes of the coronary blood flow and ischemically induced alterations of the metabolism becomes obvious immediately. The measured values give the heart surgeon topical information and are helpful to carry out protective and therapeutic measurements.

A nuclear mutation conferring aurovertin resistance to yeast mitochondrial adenosine triphosphatase.

A single gene nuclear yeast mutant was isolated whose mitochondrial F1-ATPase was resistant to the specific F1 inhibitor aurovertin. The mutant enzyme was not cross-resistant to other F1 inhibitors. The binding of aurovertin to F1 and to the two largest F1 subunits (alpha and beta) was measured by enhancement of aurovertin fluorescence. Aurovertin bound to wild type F1-ATPase and to its monomeric beta subunit with about the same binding constant. It failed to bind to wild type alpha subunit or to either F1 or F1 subunits from the mutant. The aurovertin-resistant mutant thus contains an altered nuclear gene which specifies the structure of the beta subunit of F1.

[Thymomas (author's transl)].

Immunological investigations concerning pathological autoantibodies and defects of humoral immunity were performed in 7 patients with thymomas, 5 of which showed invasive growth. The number of B and T lymphocytes in blood was determined at the same time using membrane markers as well as blood lymphocyte stimulation with phytohaemagglutinine. Two of the 3 patients with auto-antibodies against striated muscles or nuclei showed the clinical signs of accompanying disease (myasthenia gravis, lupus erythematodes). A humoral immunodisturbance with IgM deficiency was demonstrable in one patient and was accompanied by clinical symptoms. Lymphopenia with decreased numbers and functional disturbance of T and B lymphocytes could be shown in the majority of patients. Immunological investigations simplify proof of accompanying diseases in thymomas. These represent an important prognostic criterium in the same way as does invasive growth.

A protein inhibitor of mitochondrial adenosine triphosphatase (F1) from Saccharomyces cerevisiae.

A heat-stable protein has been detected in Saccharomyces cerevisiae which inhibits mitochondrial ATPase activity. The protein inhibitor has been isolated from extracts prepared by brief heat treatment of unbroken cell suspensions. The isolated inhibitor is a small basic protein (molecular weight close to 7000, isoelectric proint 9.05) devoid of tryptophan, tyrosine, and cysteine as well as proline. The NHP2-terminal amino acid is serine. The ultraviolet absorption spectrum shows the vibrational fine structure of the phenyl-alanine band. Like the ATPase inhibitor from bovine heart mitochondria the yeast inhibitor is rapidly destroyed by trypsin. It is also inactivated by the yeast proteinases A and B. Radioimmunological analysis indicates that the inhibitor is synthesized on cytoplasmic ribosomes. Its accumulation seems to be connected to the formation of the mitochondrial ATPase complex, since its specific activity is greatly reduced both in extracts obtained from the F1-ATPase-deficient nuclear mutant pet 936 and from the cytoplasmic petite mutant D 273-10B-1.
