[The treatment with the antidepressant Lerivon of tension headaches and autonomic crises].

20 patients with vegetative crises (panic attacks) and 20 patients with tension-type headache were treated by Lerivon (tetracyclic antidepressant) administration. The following parameters were investigated: the degree of headache expression according to visual analogous scale, the vegetative crises frequency and depression gravity by means of Spilberg test, the depression intensity by means of Back scale. The areas of different components of contingent negative deviation (CND)-early, late, postimperative negative waves as well as total CND area-were examined either. Both high clinical effects of the preparation and normalisation of psychophysiological parameters were found. 30 mg was the common daily dose for the patients with psychophysiological disorders. The clinical effect was observed quickly (7-10 days). The patients tolerated drug administration well, the sleepiness was the only side-effect. Thus, Lerivon proved highly effective for the treatment of both psychovegetative and algesic disorders and had some advantages as compared with commonly used tricyclic antidepressants.

[The psychophysiological aspects of treating obesity with Izolipan].

20 patients with obesity were examined. Their weight was increased as compared with ideal values by 30-140%. The patients were characterized by anxious or depressive disturbances (Spilberger's and Beck tests) and alterations of alimentary behaviour in form of prevalence of emotional and external alimentary behaviour (EMAB and EAB, respectively). The study of cognitive negative variations (CNV) revealed the decrease of late wave's area, which correlated reversely with levels of anxiety, depression and EMAB expression. The Isolipan administration (15 mg, 2 times/daily during 3 months) together with hypocaloric diet resulted in average weight loss equal to 12.3 kg in the whole group. These changes correlated well with normalization of both alimentary behaviour (in the form of decrease of EMAB and EAB indices) and emotional state, and increase of decreased late CNV with disappearance of pathological psycho-physiological correlations between CNV and emotional state.

[Peripheral autonomic nervous system function in patients with hypothalamic insufficiency].

Peripheral vegetative nervous system state was estimated in 61 patients with constitutional acquired hypothalamic insufficiency by using non-invasive methods. Control group consisted of 10 healthy individuals. Cardiovascular tests application led to the evaluation of subclinical parasympathetic vegetative insufficiency. In was more prominent in patients with early neuroendocrine disorders debut as well as in those with more protracted disease durations. The authors suggest that these findings testify the constitutional acquired character of disturbances. Besides it was also found the conductivity velocity fall in sudomotor preganglionic fibers in arms and legs and the prevalence of such disorders in patients with expressed dysraphic status. The last observation may testify the defect in laying of these peripheral vegetative fibers. The study carried out permits to suppose that subclinical peripheral insufficiency occur to be one of the factors evoking rise either of permanent (tachycardia, arterial pressure elevation, hyperhidrosis) or of paroxysmal vegetative disturbances (panic attacks, migraine) in patients with hypothalamic insufficiency.

[The clinical picture, diagnosis and therapy of headaches in patients with a hypothalamic syndrome].

190 patients with hypothalamic insufficiency were investigated. The character, frequency of various forms of headache as well as its correlation with neuroendocrine emotional motivation and vegetative disorders were analysed. It was found that headache occurred more frequently in patients with hypothalamic insufficiency (66%) than in the whole population (5-20%). This confirms the role of diencephalic disfunction in its genesis. Psychovegetative disturbances were expressed most of all in tension-type headache (TTH). Very likely that some psychoendocrine mechanisms (particularly depression and idiopathic edema) play important role in pathogenesis of TTH. Clinical effect of Lerivon confirms the pathogenetic role of depression in these cases.

[The hypothalamic syndrome in childhood (its epidemiological and clinico-electrophysiological aspects)].

The epidemiological survey covered 2000 city schoolchildren. They ranged in age from 7 to 15 years. Hypothalamic deficiency (HD) was detected in 5% of the examinees. Clinical and physiological findings on HD children are provided. They were found to have the history of hereditary or natal damage evidencing the acquired nature of the disease. Special emphasis is placed on HD manifestations in prepubertal and pubertal age. The authors show defective regulation of nonspecific brain systems in the form of predominant activation of the septohippocampal system and relative insufficiency of the brain stem mesencephalic reticular formation. Activation of cerebral ergotropic vegetative mechanisms combines with peripheral vegetative failure (sympathetic and parasympathetic), probably, of congenital origin.

[Status of nonspecific brain systems in various hypothalamic syndromes].

The nonspecific systems of the brain and their role in neuroendocrine regulation were studied and the nature of some neuroendocrine and metabolic disorders was specified. Examination of 41 women with the hypothalamic syndrome showed that they had ergotrophic insufficiency expressed both in the specific hypothalamus-pituitary system (hyperprolactinemia) and in nonspecific systems (deficiency of the activation systems of the brain sympathetic activity at the periphery and the emotional syndrome), which suggests the deficit of dopamine not only in the tuberoinfundibular but also in mesolimbic systems.

State of the peripheral nervous system in patients with hypothalamic insufficiency.

An investigation was carried out by non-invasive techniques of the state of the peripheral nervous system in patients with a constitutional-acquired form of hypothalamic insufficiency. Ten healthy individuals were included in the control group. The presence of subclinical parasympathetic vegetative insufficiency was identified by cardiovascular tests. It was more marked in patients with an early onset of the neuroendocrine disturbances and longer course of the disease. In the opinion of the authors, this suggests the constitutional-acquired character of the vegetative disturbances. In addition, a slowing of the speed of conduction through the sudomotor preganglionic sympathetic fibers in the upper and lower extremities and the predominance of these disturbances in the group of patients with pronounced dysraphic status were identified. The latter suggests a defect of the laying down of these peripheral vegetative fibers. This investigation makes it possible to hypothesize that subclinical peripheral insufficiency is one of the factors governing the occurrence of both permanent (tachycardia, elevations of AP, hyperhidrosis) and paroxysmal vegetative disturbances (panic attacks, migraine) in hypothalamic insufficiency.

[State of cerebral functions in the persistent lactorrhea-amenorrhea syndrome].

A complex investigation of the brain functions of patients with the idiopathic persisting lactorrhea-amenorrhea syndrome (PLAS) revealed a series of nonspecific changes. Insufficiency of the activating systems was shown according to the EEG data. Anxious depression was also seen. The examination of the vegetative nervous system during relaxation and exercises is indicative of insufficient activity of its symptomatic region. Thereafter, specific neurosecretory deficit in the tuberous infundibular system and disorder of the brain activity, probably resultant of dopamine deficiency are seen in patients with PLAS.
