[Familial acute leukemia. Report of 7 cases in 3 families].

Seven members of 3 families developed acute leukemia (AL) between 1969 and 1988. The clinical presentation, course and response to treatment were no different to non-familial cases of AL. The morphology subtypes were identical in every family: myeloblastic in 3 and lymphoblastic in 4. Leukemogenic factors (radiation, drugs use or benzene exposure) were excluded in these patients. Genetical and environmental factors might have a role in the pathogenesis but no clear evidence has been established.

[Anorexia nervosa: endocrine function during the phases of body weight loss and recovery].

In the present study, we investigated the endocrine status of patients with anorexia nervosa during weight loss (WL), as well as, after weight recovery (WR). A comparison between our findings and those obtained from other populations previously described was made. We studied 12 female patients during WL; 7 of them were reevaluated after WR. Stimulation tests with LRH, TRH, ACTH and insulin-induced hypoglycemia were performed in all cases. During the WL phase, basal serum levels of LH and estradiol, as well as the LH response to LRH, were diminished in comparison with normal values. Basal serum levels of FSH were low or normal. The function of the hypothalamic pituitary-ovarian axis was recovered in all patients restudied; six out of seven returned to ovulation within the first 4 months after WR. The remaining patient presented hypothalamic amenorrhea because of excessive physical activity. Four patients exhibited basal low T3 and T4 levels with normal TSH and a retarded response to TRH during WL. At WR some patients completely recovered their thyroid function while others developed clinical hypothyroidism. Six months after WR all patients were euthyroid. Prolactin response to TRH was unaffected in 10 patients. One patient had basal hyperprolactinemia and hyperesponsiveness to TRH, and the remaining one had only a PRL hyperesponsiveness; this latter finding persisted in one of these patients during the WR phase. This abnormality was attributed to changes in the dopaminergic tone secondary to stress. Although serum growth hormone concentrations were normal in all patients during WL, two of them had basal hypersecretion and hyperesponsiveness to hypoglycemia during WR, which was attributed to protein deficiency.(ABSTRACT TRUNCATED AT 250 WORDS)

[Reiter's syndrome and urinary infection in a case of AIDS].

Herein we describe a case of Reiter's syndrome coexisting with HIV infection in a female parenteral drug addict who was also predisposed to multiple genitourinary involvement. In our view. the manifestation of additional autoimmune states could herald the worsening of the prognosis of HIV infection and the nephrological and urological course of these patients.

[Lytic enzymes of the pneumococcal system].

The cell wall lytic enzymes of Streptococcus pneumoniae and its bacteriophages provide a reliable experimental model to support the extended idea concerning the modular organization of most proteins. The comparative analysis of the deduced amino acid sequences of these enzymes, as well as the construction of functional chimeric phage-bacterial enzymes, demonstrate that the C-terminal domain, which contains a large number of repeated amino acid motifs, is the substrate-binding domain, whereas the N-terminal domain provides enzymatic specificity. The development of chimeric proteins from genes encoding enzymes that specifically degrade cell wall of bacteria belonging to different genera (e.g. Streptococcus and Clostridium), supports the hypothesis concerning domainal interchange as a driving force for the construction of novel functional proteins that bacteria might create to allow them to become adapted to new environmental situations.

[Pathogenicity and pneumococcal capsular genes].

Pneumococci remain to be one of the most prominent human pathogens. Increasing efforts are being dedicated to the development of improved vaccines with wider specificity. Since a clear understanding of the genetics of capsular types in Streptococcus pneumoniae is missing, our efforts are oriented to characterize, at the molecular level, the genes involved in capsular polysaccharide biosynthesis. We have cloned and sequenced a chromosomal DNA fragment of a clinical isolate of type 3 pneumococcus and showed that it contains a type 3 specific gene as well as genes common to other serotypes.

[Usefulness of Doppler echocardiography in the analysis of left ventricular isometric relaxation time in an experimental model of myocardial reperfusion].

UNLABELLED: Modifications of left ventricular isometric relaxation time (LVIRT) were analyzed in an experimental model of myocardial reperfusion (MR). In a prospective study, 19 mongrel dogs were studied with open chest, mechanical ventilation and hemodynamic monitoring. A catheter was inserted in the middle third of the anterior descending artery (ADA) for registry of its pressure. The ADA was ligated distal to the first diagonal branch for 15 min and subsequently untied. Epicardial Echo/Doppler registries were obtained in basal conditions, at 5 and 15 min of ischemia and at 5, 15, 30 and 60 min after MR. Mobility and changes in thickness of the interventricular septum (IVS) were analyzed, and LVIRT was measured in all of the periods mentioned. There were no significant changes in the thickness of IVS. LVIRT increased in comparison to the basal registry at 5 and 15 min of ischemia (p < 0.0001) and returned to basal values with MR (p < 0.05). CONCLUSIONS: Acute myocardial ischemia prolongs LVIRT, in this model, during MRLVIRT returned to normal. In the experimental model, measurement of LVIRT may be used as criterion for reperfusion.

[Vascular lesions in surgery of the lumbar spine].

We present two cases of iliac artery damage arising from surgery to correct a lumbar disc hernia due to ventral perforation. The first was a pseudoaneurysm of the left iliac artery with a retroperitoneal hematoma diagnosed by ultrasound and TAC on the third day after surgery. The second was a retroperitoneal hematoma diagnosed by intraoperative ultrasound after the patient was shifted to a new position. Emergency laparotomy was performed on both patients to repair the damage. This is an unusual but serious complication, and in most cases requires intuitive diagnosis and lifesaving surgery.
