[Effects of midazolam on the delivery and consumption of myocardial oxygen in the postoperative period of children undergoing heart surgery].

INTRODUCTION: The aim of this study was to evaluate the effect of midazolam on the variables which determine myocardial oxygen delivery and consumption relationship (endocardiac viability index) in children undergoing cardiac surgery under extracorporal circulation. METHODS: The study was carried out in 15 children to whom a doses of midazolam of 0.2 mg/kg was given in immediate postoperative period upon hemodynamic stabilization. The determinations were performed prior to administration of the drug, at 5, 15, and 30 minutes thereafter. The variables analyzed were mean systemic blood pressure, mean diastolic pressure in aorta, mean pressure in the left auriculum, diastolic-systolic time relation, arterial blood O2 content and index of endocardiac viability. RESULTS: The following variables were modified with respect to the basal contributing to an improvement in the endocardiac viability index. Mean aortic diastolic pressure decreased 10.05% at 5 min and 6.17% at 15 min (p < 0.05, p < 0.005); mean systemic arterial pressure decreased 10.64% at 5 min and 6.9% at 15 min (p < 0.05, p < 0.005); the mean pressure in the left auriculum decreased 6.28% at 5 and 8.32% at 15 min (p < 0.05); the diastolic-systolic time relation increased 24.04% at 5 min (p < 0.05, p < 0.005). The index of delivery and consumption of endocardiac oxygen increased 23.4% at 5 min (p < 0.05). CONCLUSIONS: The intravenous administration of midazolam to children at doses of 0.2 mg/kg during the postoperative period of cardiac surgery produced an improvement in the delivery and consumption of oxygen to endocardium at 5 minutes of its administration.

Thiolprotease activity in skeletal and myocardial muscle and liver of fasted rats.

The activity of the thiolproteases, cathepsins B, H, and B + L, one of the most important groups of endoproteases, was measured in skeletal and myocardial muscle and liver of Sprague-Dawley rats submitted to fasts of different duration (control and 24, 48, and 72 h). After the fasting period, the animals were killed, and fresh tissue samples were collected. Enzyme activity was determined in vitro with the specific substrates Z-Arg-Arg-MCA for cathepsin B, Z-Phe-Arg-MCA for cathepsin B + L, and Arg-MCA for cathepsin H. Results show different patterns in the organs studied: activity increased linearly in liver, decreased in myocardial muscle, and had no change in skeletal muscle. These results suggest that the expected alteration observed in proteolytic activity in fasted tissues is produced to a certain degree by changes in thiolprotease activity.

[Immunological detection of aflatoxin-albumin adducts in children with chronic hepatitis B infection].

The value of aflatoxins is well known as a carcinogenic, mutagenic and teratogenic, likewise its association with the hepatitis B virus. In addition, it is known the incidence of hepatocellular carcinoma in such viral infection. A study was performed on the albumin adducts-aflatoxins levels in sera determined by ELISA method of children within 3-15 years old at the Service of Pediatric Gastroenterology from the National Institute of Gastroenterology. Samples consisted of 40 patients with chronic active hepatitis (CAH) B, 10 HBsAg+ carriers and 20 controls. The CAH group, showed a 32.5% of positiveness with a maximum levels of 25pg aflatoxin lysine/mg albumin while 20% of HBsAg positive carriers showed levels of un 12.3 pg aflatoxin lysine/mg albumin and 15% of the control group 5pg AF lysine/mg albumin. It can be observed that aflatoxin levels in patients of CAH presented values up to 5 times over the control group. This study suggest the validity of aflatoxin-albumin adducts as a marker of chronic exposure to this carcinogen and its importance in relation with the virus of hepatitis B.

A case of hairy cell leukemia resembling asymptomatic chronic liver disease on presentation.

Hepatic infiltration in hairy cell leukemia is quite common but results of liver test are usually normal. A 44-year-old man with a history of alcohol abuse was evaluated for persistent elevation in serum aminotransferases. A percutaneous liver biopsy showed an extensive mononuclear cell infiltrate in the sinusoids and this finding led to the diagnosis of hairy cell leukemia. Liver involvement in hairy cell leukemia is discussed. Hairy cell leukemia should be considered as a potential cause of persistent increase in liver-related enzymes.
