High risk breast cancer: long-term surgical adjuvant therapy based on predictive tests - preliminary report.

A surgical adjuvant therapy trial was started in 1974, in locoregionally advanced breast cancer. The adjuvant hormonal and cytostatic therapeutic regimen was administered according to the results of predictive tests: estrogen receptor (ER) assay and oncobiogram (group I); in the control group (0) no adjuvant therapy was given until relapse, but tests were performed like in group I. Preliminary results. In a subgroup with adjuvant chemotherapy, using one or more substances according to oncobiogram, with or without hormonal treatment, the treated women did highly-significantly better, compared with the untreated group. No advantage was seen from ovariectomy and/or anabolic steroids, either with or without Cytoxan (Test results: ER +, in vitro not growing tumours). Some doubt exists about the results: resistant, especially in testing cytostatics. This doubt and the rate of tumours, not growing in vitro, limit the use of predictive tests, concerning cytostatics. Further, one has to be cautious in the use of additive hormonal treatment in surgical adjuvant therapy: in some cases stimulation can not be excluded.

[Estradiol receptor in relation to clinical parameters of mammary tumor patients (author's transl)].

Various clinical parameters, as menopausal state of the patients, clinical staging, tumor histology, free interval and survival time are valid for assessment of mammary carcinomas in relation to prognosis and therapeutic regimens. A correlation between receptor status of the tumors and these parameters shows with some exceptions that the percentage of receptor positive tumors is higher in such cases which are representative for a favourable prognosis. Thus the receptor status is an additional marker to evaluating the prognosis of mammary tumor patients. To perform therapeutic regimens it is important that in nearly 67% of all cases repeated receptor investigations in tumor recurrences show the same results as investigations of the corresponding primary tumors do.

HLA-B17 and the HLA-A1, B17 haplotype in acute myelogenous leukemia.

Seventy-nine Caucasians with acute myelogenous leukemia (AML) were genotyped to determine whether AML, the induction of remission or patient survival were associated with particular HLA phenotypes or haplotypes. HLA-B17 and B27 were increased in AML patients over 40 years of age. Combined analysis of four independent studies indicates that HLA-B17 is significantly but weakly associated with AML, relative risk = 1.48 (.01 less than P less than .025). The A1, B17 and Aw24, Bw35 haplotypes occurred more frequently in the AML group as compared to racial and geographic controls (uncorrected P = 0.0068 and 0.0098, respectively Fisher's Exact Test). Induction of remission occurred less frequently in patients with the B17 phenotype as compared to patients lacking this antigen (P = 0.047). Patient survival was associated with remission status (P = 0.002) but was not significantly associated with particular HLA phenotypes or haplotypes. These results indicate that a gene or genes in the HLA-B region of the major histocompatibility complex can influence susceptibility to AML and also the response to chemotherapy.

Chemotherapeutic agents and estradiol receptor binding capacity.

Cytotoxic agents used for the treatment of breast cancer did not inhibit the growth of uterus following estradiol injection to ovariectomized rats. They had no effect on the estradiol binding ability and on the dissociation constant of estradiol receptor binding of rat uterus. In addition, 1,2:5,6-dianhydrogalactitol, a highly active, proximal alkylating agent, applied in vitro did not alter the relative binding ability to tamoxifen to the cytoplasmic estradiol receptor of rat uterus and human breast cancer tissue. In summary, the present experiments did not reveal any disadvantageous interaction between simultaneously administered hormonal and cytotoxic drugs.

Contribution to the mechanism of Stilbostat action.

The hormone-cytostatic drug Stilbostat does not influence the estradiol binding capacity of the rat uterus tissue and tissues of human and experimentally induced mammary tumors. It exhibits an alkylating potency similar to that of other cytostatic drugs used in the therapy of neoplasia. An in vitro splitting of Stillbostat into diethylstilbestrol by tissue slices could not be detected. It was impossible to demonstrate an excretion of splitting products of Stilbostat by rat urine.

Urinary excretion of 17-ketosteroids in patients suffering from mammary cancer.

1. The excretion of dehydroepiandrosterone is lower in patients suffering from mammary cancer than in healthy women. 2. There doesn't exist any relation between the 17-ketosteroid excretion and the age of postmenopausal women. 3. Patients with mammary tumors in stages III and IV are characterized by a lower excretion of dehydroepiandrosterone in percental parts than patients in stages I and II. 4. In patients with mammary tumors in stages III and IV and in patients with recurrences independently from the tumor stages we find higher extrem values for aetiocholanolone and smaller ones for dehydroepiandrosterone than in the other stages. 5. The quotient of percental parts of excreted aetiocholanolone to dehydroepiandrosterone shows significantly higher values for patients with recurrences than without them. 6. The quotient of aetiocholanolone to dehydroepiandrosterone is not a valid factor for a postoperative control of recurrences of the disease.

The excretion of 17-ketosteroids in patients suffering from mastopathia fibrocystica and mammary carcinomas.

Comparative investigations of the urinary excretion of different 17-ketosteroids in healthy women and patients suffering from mastopathia fibrocystica and/or mammary carcinomas had the following results: 1. A clear age dependency of the excretion of 17 ketosteroids during the regenerative period has been shown in all groups investigated, but it could not be observed during the postmenopausal phase. 2. There are no hints by the excretion values for a remarkable contribution of mammary carcinomas to the hormone metabolism of the host. 3. The excretion values of patients suffering from mastopathia fibrocystica confirm epidemiological results, which consider this from of mastopathia a precancerous one.

Estradiol receptor activity in tissues of human breast cancer.

Using the method for receptor determination recommended by the EORTC breast, cancer group we found 33.8% of our tissue specimen experimentally estradiol sensitive. No relation could be seen between receptor binding ability of the tumor tissues and lymph node involvement, tumor histology, clinical stages of the disease and menopausal state of the patients, respectively. In most cases primary tumor tissues and tissues of invaded lymph nodes show a corresponding behaviour in relation to estradiol uptake. The differences observed in the effects of various antiestrogenic substances on estradiol binding ability can not be explained at present.
