Dynamic documentation of digital dacryocystography.

Digital dacryocystography has proved to be the optimum method of x-ray diagnostics in connection with managing obstructions of the lacrimal pathways. We present 50 patients in whom digital dacryocystography was performed with a computer-controlled x-ray unit and a C-arc coupled to an image intensifier TV-system. A nonionic water soluble contrast medium was used. The bilateral contrast medium filling process appearing on the monitor was simultaneously recorded on videotape. Videocassettes, with patient data attached, were stored in a videotape library, providing repeatable diagnostic documentation and an excellent base for scientific analyses.

[Movement therapy. With special reference to functional disorders of the locomotor system].

Physical therapy by movements is of importance in several medical fields, in particular in rehabilitation. A great variety of possibilities for treatment by exercises and manual therapy exists. In a first part we outline a useful and problem-oriented nomenclature derived from actual applications of physical therapy by movements based on various concepts. The bases and applications of functional therapy by movements, by training or manipulative procedures as well as other therapeutic means are presented. The therapeutic climate and the relation between doctor and physiotherapist are briefly explored. In a second part we present the most important traditional concepts of treatment by movements in a more detailed way.

Exercise Therapy

Acquired dacryocystitis: microbiology and conservative therapy.

The dacryocystitis in adults is mainly caused by postsaccal stenosis of the lacrimal ducts. The banking up of the lacrimal fluid leads to an accumulation of germs and following infection. This report describes the clinical and microbiological findings in a large consecutive series of patients that presented at the outpatient clinic of the 2nd Department of Ophthalmology of the University of Vienna with the signs of acute, chronic recurrent or chronic infections of the lacrimal system between 1983 and the end of 1990. Within the bacterial genus Staphylococci (S. aureus, S. epidermidis and S. saprophyticus) were the most frequently isolated organisms (74 cultures = 50% out of samples with positive cultures). It was quite interesting that a significant number of gram-negative rods (37 = 25.5%) could be isolated. Of these microorganisms Escherichia coli was most frequently growing on special media (17 cases = 11.7%) when cultures were obtained from acute inflamed lacrimal sacs of patients who suffered from chronic recurrent infections. Conservative therapy of purulent dacryocystitis constitutes the last possible preparation for a necessary surgical intervention and therefore the authors want to point out the importance of microbiological examinations so as to optimize antibiotic therapy.

[Documentation of digital dacryocystography using video technique].

Digital dacryocystography is considered the optimal radiodiagnostic method in obstructions of the lacrimal pathways. The examination is carried out by a computer-controlled X-ray unit with a C-arc attached to an image-intensifying TV system. In this way the advantages of this technique and of modern video recording are linked. The bilateral filling process of the contrast medium is recorded and the findings simultaneously recorded on videotape. Consequently, pictures of the dynamic flow are retrievable at any time. This provides good diagnostic documentation and an outstanding basis for any scientific analysis.

[Patent ductus arteriosus in a dog: modified method of double ligation].

Patent ductus arteriosus, a common congenital cardio-vascular disease, needs surgery for correction. Three methods are described: 1. transsection and suturing of the ductus ends; 2. dissection of the ductus itself and occlusion by double ligation (classical method), 3. modified double ligation described by Jackson and Henderson. A modification of the technique is described, based on the previous methods of double ligation.

Videodocumentation in digital dacryocystography.

The digital dacryocystography proved to be the optimum method of X-ray diagnostics in obstructions of the lacrimal pathways. The examination was performed with a computer controlled X-ray unit with a C-arc coupled to an image intensifier tv-system. In the study the advantages of this technique were combined with the advantages of modern videotechnique. A nonionic water soluble contrast medium was used. The bilateral contrast medium filling process was registrated and the findings were recorded on videotape concurrently. The videocassettes, marked with the data of the patients and the date of examination, are stored in a video-tape library. This provides an always repeatable diagnostic documentation and it is also an excellent base for scientific analyses. The technique and the results of the examinations, performed with 18 patients, are described.

[Capsular sack implantation of heparin-modified posterior chamber lenses].

In a prospective study, heparin surface modified posterior chamber lenses were implanted into the capsular bag in 50 patients. The exact positioning of the biconvex, one-piece lens in the bag is the prerequisite for good centration and for an optimal result. The hydrophilisation of the originally hydrophobic PMMA material is achieved by heparin surface modification and results in less inflammation. The aim of this study was to investigate the postoperative behavior of the HSM posterior chamber lens implanted in the bag, with special consideration given to intraocular inflammation. In the postoperative follow-up time of one to seven months, hundred percent of the patients with physiological fundus reached a postoperative visual acuity of 0.7 or better.

Non-radioactive labeling and detection of nucleic acids. IV. Synthesis and properties of digoxigenin-modified 2'-deoxyuridine-5'-triphosphates and a photoactivatable analog of digoxigenin (photodigoxigenin).

The chemical syntheses of novel digoxigenin-derivatized compounds are described which are modified substrates for enzymatically or photochemically non-radioactive digoxigenin labeling of nucleic acids. Various activated digoxigenin-haptens are coupled to 5-aminoallyl-substituted 2'-deoxyuridine-5'-triphosphate. This results in digoxigenin-modified nucleoside triphosphates of variable spacer lengths (Dig-[4]-dUTP/Dig-[11]-dUTP/Dig-[16]-dUTP) which can be used as substrates for enzymatic labeling of DNA with digoxigenin-haptens by Klenow enzyme-catalysed random-primed synthesis. In addition the synthesis of N-[4-azidobenzoyl]-N'-[(3-O-digoxigeninyl)methylcarbonyl)]-1 ,8-diamino- 3,6-dioxaoctane (photodigoxigenin), a photoactivatable analog of digoxigenin, is described which can be applied for photolabeling of DNA and RNA with digoxigenin-haptens leaving the nucleic acid molecules intact.

The human VK locus. Characterization of a duplicated region encoding 28 different immunoglobulin genes.

Two large regions of the human multigene family coding for the variable parts of the immunoglobulin light chains of the K type (VK) have been characterized on cosmid clones. The two germline regions, called Aa and Ab, span together 250,000 base-pairs and comprise 28 different VK gene segments, nine of which have been sequenced. There is a preponderance of VKII genes but genes belonging to subgroups I and III, and genes that cannot be easily assigned to one of the known subgroups, are interspersed within the VKII gene clusters. A number of pseudogenes have been identified. Within the Aa and Ab regions, all gene segments are organized in the same transcriptional orientation. The regions Aa and Ab, whose restriction maps are highly homologous, were shown not to be allelic structures; they must have arisen by a duplication event. Taken together with previous results, one can conclude that the major part of the VK locus exists in duplicated form. One individual has been found who has only one copy of some of the duplicated regions. By chromosomal walking, the A regions could be linked to the O regions, an analysis of which has been reported. The A regions contribute about one-third of the VK genes so far identified.

Mechanisms of destruction of Aspergillus fumigatus hyphae mediated by human monocytes.

Normal monocytes attached to and destroyed Aspergillus hyphae by morphologic and metabolic criteria. Inhibition by anaerobiosis, azide, cyanide, halide-free conditions, catalase, histidine, and tryptophan suggested mediation of hyphal damage primarily through the myeloperoxidase system. However, myeloperoxidase-independent oxidative or nonoxidative mechanisms appeared active in hyphal damage by monocytes from patients with myeloperoxidase deficiency or chronic granulomatous disease (CGD). Moreover, hyphae were damaged by lysates and granule-enriched fractions of monocytes from patients with CGD, whereas comparable fractions of normal monocytes were active only with added halide and H2O2. Hyphal damage by both whole monocytes and granule-enriched fractions from patients with CGD was inhibited by polyanions, a result suggesting that cationic proteins may be involved. Hyphal damage by normal monocytes or monocytes from patients with CGD was inhibited by cells that lacked antihyphal activity (chlorpromazine-treated normal neutrophils or neutrophils from patients with CGD, respectively). This effect may predispose patients with CGD to chronic, invasive aspergillosis.
