Comparison of magnetic resonance imaging and ultrasonography in the evaluation of abdominal aortic aneurysms.

Magnetic resonance imaging (MRI) was used to evaluate abdominal aortic aneurysms in 27 patients. The findings were compared retrospectively with CT, ultrasound (US), and angiography in 17 cases and prospectively with US in 10 cases. MRI identified the renal arteries in all cases, demonstrated involvement at or above the origin of the renal arteries in eight patients, and showed extension of the aneurysm into the iliac arteries in 12 cases. The outer dimension of the aneurysm, the diameter of the residual lumen, and the length of the aneurysm were measured easily from the MR images. The measurements of transverse dimension of the abdominal aortic aneurysm were similar for MRI, CT, and US. MRI more accurately defined extension above the renal arteries and below the aortic bifurcation. It is concluded that MRI provides the necessary information for the surveillance and preoperative evaluation of abdominal aortic aneurysms.

Fibrinolysis in chronic arteriosclerotic occlusions: intrathrombotic injections of streptokinase. Work in progress.

Forty-seven patients with chronic arteriosclerotic occlusions of iliac and femoropopliteal arteries were treated by intrathrombotic fibrinolysis. The occlusions were 10-65 cm (mean, 22 cm) long and 6 weeks to 2 years (mean, 4.5 months) old. By means of consistent intrathrombotic injections of 2,500 units of streptokinase every 5 minutes, the thrombi were recanalized within 1-7 hours (mean, 2.5 hours). The primary recanalization rate was 75% (35/47), the patency rate after 2 weeks, 68%. In 29 patients (62%), a residual stenosis had to be dilated by balloon angioplasty. Because of the low total dose of streptokinase (mean, 70,000 units), the thrombin time was elevated up to twice the normal value in only one patient. Bleeding that required transfusions was observed in only two patients (4%). Advantages of intrathrombotic fibrinolysis include higher recanalization rate, lower total dose of streptokinase, fewer bleeding complications, and shorter therapy time than previously reported with other treatments.

[Sonographic visualization of the left ovarian vein. A contribution to the diagnosis of compression syndrome of the left renal vein].

Amongst 737 sonographic and CT examinations of women aged 18 to 60 years of age, seven cases of ectasia of the left ovarian vein were found (0.95%). The aetiology and significance of this finding in the context of the "compression syndrome of the left renal vein" are discussed. The value to sonography in elucidating this condition is stressed.

[Iatrogenic vascular injuries in childhood].

From 1973 until 1982, 616 arterial angiographies were done in children. 13 thromboses in the early state could be diagnosed and were treated surgically. Five times a haemorrhage after angiography made surgical intervention necessary. During the performance of a venesection there was an injury of the brachial artery three times. Two intraoperative iatrogenic vascular injuries were also reconstructed primarily. In the late stage three occlusions of the femoral artery and the distal external iliac artery were reconstructed. A decrease in the growth of the extremity could be seen in these cases. In this respect we want to emphasize the importance of an early diagnosis to prevent late complications.

CT diagnosis of thrombosis of dural sinuses in childhood.

This paper describes the CT findings in five cases of cerebral and cerebellar sinovenous occlusion. Characteristic CT findings are the visualisation of the thrombosed sinuses and veins, known as the "filled triangle" and the "cord sign" on the control scan, and the "empty triangle" with tentorial and gyral enhancement after contrast enhancement. The high incidence of thrombosed internal cerebral veins and superior cerebellar veins with sinus thrombosis in children is remarkable. CT with both control and contrast enhanced scans will probably provide the correct diagnosis in the majority of cases.

[Long-term results after endoscopic papillotomy].

A long-term control study after endoscopic papillotomy was performed in 102 patients at least 12 months after the procedure. 14 patients were no longer alive, from those only one patient died due to biliary problems. Complications occurred in 10 patients (9,8%). There was no cholangitis, but 2 cases of an acute cholecystitis some time after papillotomy. Therefore a cholecystectomy after papillotomy is suggested in the interval.

[Therapeutic embolization with polyvinyl alcohol (Ivalon). Clinical experiences].

Polyvinyl alcohol is rigid foam used in pressed or unpressed form for the purpose of embolisation. It is suitable for both micro-embolisation in the vascular periphery and for central vascular occlusion. Polyvinyl alcohol will result in permanent vascular occlusion. Basing on the experience collected with 71 embolizations, the article reports on the technique applied and on the results obtained with craniofacial, thoracic, abdominal, pelvic and peripheral embolisation.

[Transcatheteral embolization with polyvinyl alcohol--technic and experimental studies].

The article presents hardened polyvinyl alcohol foam (PVA) as embolisation material. PVA is well tolerated by the tissue and produces permanent vascular sealing. It is possible to achieve peripheral embolisation of arteries having a diameter of approx. 100 microns, as well as sealing of vessels with diameter of up to 8 mm. Pressed PVA is used for occluding larger vessels. This PVA re-assumes its original shape and dimensions in liquids, depending on their osmolarity and molecular size. Depending upon the pressure exercised, compression and re-expansion can be fourfold to tenfold.

[Percutaneous transthoracic fine-needle biopsy of the lung. Indications, results and complications in 578 punctures].

578 fine-needle biopsies of lung lesions in 319 patients were performed. This investigation was mainly done by means of Rotex-biopsy instruments. The procedure proved to be efficient in 84%, complicated in 19,7% of the patients. Major complications were seen in 5,6% only. Percutaneous needle biopsy in appropriate for obtaining a cytological diagnosis to clarify the etiology of localized lung lesions.

[Endoscopic manometry in the common bile duct].

Reproducible results were obtained by endoscopic transpapillary pressure measurements with the help of a constantly perfused catheter. Manometric measurements were performed in 51 patients without any premedication. 14 patients with a normal biliary system were compared with 17 patients after cholecystectomy, 14 patients with stones in the common bile duct and 6 patients after papillotomy. Although there were significant differences in the diameter of the common bile duct in the four groups, only the papillotomized patients demonstrated a distinct change of the pressure in the bile duct. In these patients almost no pressure gradient existed between the biliary ducts and the duodenum. On the other hand, there was an increase in intraluminal pressure in the 2 patients with papillary stenosis. Endoscopic manometric measurements in the common bile duct are indicated to obtain reliable data on stenosis of the papilla duodeni, insufficient papillotomy or recurrent stenosis.

[The compression syndrome of the left renal vein (author's transl)].

Severe compression of the left renal vein produces a pressure gradient between it and the inferior vena cava and results in changes in haemodynamics. The cause of the narrowing is usually the aorta, less commonly the superior mesenteric artery. Compression of the left renal vein may be responsible for a number of abnormalities such as primary varicoceles, primary varices of the ovarian, renal, pelvic and ureteric veins on the left, the more frequent occurrence of unilateral renal vein thrombosis on the left and the development of renovascular hypertension. One hundred and twenty-three selective phlebograms of the left renal vein and CT examinations of this structure in a further 87 patients acting as a control group were carried out. The significance of compression of the left renal vein as an aetiological factor in the development of the above mentioned abnormalities is discussed.

The significance of efficient scatter removal in chest radiography.

The conventional approach to the removal of secondary radiation in chest radiography is by using an air gap or a radiographic grid. However, grids become ineffective at high kVp levels. This study evaluated efficient scatter removal by radiographic scanography versus the conventional technique. Nylon spheres simulating lung nodules were attached to a standard radiographic thorax phantom and to a special chest phantom of our own design. Conventional and scanographic films were made of the phantoms using the same conditions. The films were read in a double-blind fashion. The results indicated that scanograms permitted appreciation of the simulated nodules over the lungs better than did the conventional films, particularly at higher kVp levels. However, the marked increase of contrast resulted in an underpenetration of the mediastinum. If properly penetrated, the lung fields were overexposed, which resulted in a decrease of the detectability of lesions in spite of a higher normalized contrast. Scanography is a promising technique for improving the detection rate of lung nodules on chest films, provided that means can be found to penetrate the mediastinum without overexposing the lungs.
