[The importance of the serumlipaseevocationtest and the retrograde pancreaticography for the diagnosis of chronic pancreatic affections (author's transl)].

Comparative studies between activity of serumlipase, lipaseevocationstest and endoscopic retrograde cholangiopancreaticography (ERCP) in 45 patients with chronic pancreatitis, pancreatic cysts or pancreatic carcinoma confirm, that elevated enzyme values after caerulein stimulation indicate an alteration of the pancreatic parenchyma. However, this findings does not give information on the etiology of the damage. On the basis of the presented results it seems that the lipase-evocationstest has only limited value as screening method within the total of clinical methods for the assessment of light and medium severe stages of chronic pancreatitis and accompanying forms of pancreatitis. There is no evidence that this method may be used for screening for carcinoma of the pancreas. A positive result gives a valuable information, but the lack of an increase of lipase activity does not exclude the existence of pancreatic affection. In each case with clinical suspicion for pancreatic disease further diagnostic investigation by the ERCP and additional radiographic methods should be performed.

[Cancer and advanced pancreatitis of the head of pancreas in the ERCP (endoscopic retrograde cholangio-pancreaticography) (author's transl)].

3 cases each of carcinoma and chronic pancreatitis of the head of pancreas show tha difficulties of differential diagnosis in retrograde cholangio-pancreaticography. When both duct-systems are shown, diagnosis is facilitated when changes are typical. In favor of cancer--apart from well-known criteria--is the more frequent occurrence in the choledochus and, possibly, the higher age of the patients. All other possible techniques have to be utilized side by side with ERCP.

[Intrapulmonary fibromas (author's transl)].

Based on 5 individual cases over a period of 8 years (1 fibroma, 1 angiofibroma, 2 adenofibromas, 1 xanthous neurofibroma) the characteristics of the rare intrapulmonary fibroma and the therapeutic consequences are discussed. Any suspicious tumor demands a thoracotomy with careful preoperative treatment and the subsegment or segment resection depending on the histological diagnosis reported during the procedure.

[In vitro T1- and T2-relaxation times of coagulating blood and thromboses].

A comparison between blood and thromboses is made on behalf of T1 and T2 relaxation data. Thereby, the time interval between the withdrawal and the measurement of blood forms a parameter of investigation. Among the thromboses the surgically removed ones are compared with those aggregated in blood during course of time. As the most outstanding result one can see that the formation of thromboses changes significantly the T1 and T2 relaxation times of 1H und 23Na. The values of the relaxation times obtained for those thromboses which have aggregated within the blood are thereby comparable with those relaxation times found for the surgically removed ones.

Multiple sclerosis in children: value of serial MR studies to monitor patients.

A series of six children with clinical (4) and laboratory (2) evidence of multiple sclerosis is described. The mean age at onset was 12 years and the female-male ratio was 5:1. All patients had white matter abnormalities on initial MR scans. On follow-up MR studies, performed every 3 to 5 months, all children exhibited changing patterns of CNS signal abnormalities. In three cases, clinically silent brain lesions were detected. In four patients with an acute clinical attack, large lesions were present, demonstrating a lamellar structure on T1- and T2-weighted images. The lesions were seen best on long TR/short TE spin-echo sequences. Combined sagittal and axial series with EKG gating and flow-compensation technique were best for MR follow-up studies. Our results show that MR is useful for monitoring patients with multiple sclerosis.

MR findings in methotrexate-induced CNS abnormalities.

MR of the brain was performed in eight patients (mean age, 14.9 years) with osteogenic sarcoma during or after IV treatment with high-dose methotrexate. MR detected brain abnormalities in four patients, three of whom had concomitant neurologic dysfunction. Pathologic findings demonstrated on MR were (1) chronic brain edema, demonstrable over a period of 3-14 months (proved by autopsy in one patient); (2) multifocal white matter necrosis; and (3) deep brain atrophy. MR appears to be valuable in the detection of abnormalities induced by treatment with high-dose methotrexate.
