[Serum ferritin in pregnancy at term and in newborn (author's transl)].

Serum ferritin, -iron, and -transferrin, and red cell blood count in 53 non-selected pregnant women at term, and from the respective cord blood samples were determined. In addition, serum ferritin was measured in 20 infants longitudinally on the first and sixth day of life. The mean cord blood concentrations of fetal serum ferritin were 135 micrograms/l, (maternal: 30,5 micrograms/l), and of fetal serum iron 27,7 mumol/l (maternal: 17,3 mumol/l), and of fetal transferrin 1,6 g/l (maternal: 4,43 g/l). A correlation between maternal and fetal serum ferritin levels could not be demonstrated. There was also no relationship between maternal serum ferritin concentrations and fetal serum iron, transferrin, and red blood count. Within the first week of life serum ferritin levels of the newborn showed a tendency to increase. From these results and from experimental data in the literature, we conclude that the feto-placental unit covers its demand on iron at the mother's expense, and independent from maternal iron stores. It is probably only an additional insufficient nutritional supply of iron, combined with depleted iron stores which might have unfavourable effects to the fetal and neonatal iron metabolism.

[The possibilities of optimal therapy of corpus uteri cancer (author's transl)].

During the years 1960-1974 out of 1002 patients with endometrial carcinoma 256 underwent primary surgery and 846 primary irradiation. The evaluation of the results of 4 different surgical methods showed despite a different distribution of the stages of the cancer that Werthein's operation and abdominal hysterectomy and adnexectomy with simultaneous lymphnodectomy had statistically significant better results than abdominal hysterectomy without lymphonodectomy. By surgery a 5 years survival time was achieved in 83.6% (214 cases) with a primary mortality rate of 0.39% and an incidence of histologically proved lymphnode-metastases of 6.78%. Out of the 846 patients with generally or locally inoperable endometrial carcinomas, who underwent primary intracavitary irradiation 411 (48.6%) achieved a 5 years survival time. After irradiation in 9.9% reversible reactions (severe proctitis and cystitis) and in 2% irreversible complications (fistulas) were observed. After an observation period of 5 years 625 of the total of 1002 treated patients (62.4%) were alive and free of symptoms.

[Perioperative metronidazole-prophylaxis for cesarian section (author's transl].

A prospective, randomized clinical trial was conducted in 103 patients undergoing cesarian section to assess the efficacy of prophylactic, intravenously administered Metronidazole on the infectious morbidity. A group of 53 patients with perioperative Metronidazol-prophylaxis was compared to a similar controll-group without prophylaxis. Bacteriologic swabs were taken from the cervix pre- and postoperatively, using anaerobic transport media. Prophylactic Metronidazole reduced postoperative fever of more than 38 degrees C on two subsequent days from 60% in the controll-group to 30,2% in the Metronidazole-group (p less than 0,01) wound infections were reduced from 18% without to 5,7% with prophylaxis (p less than 0,05) and Endometritis from 30% without to 13,2% with prophylaxis (p less than 0,05). An additional antibiotic therapy was necessary in 44% of the cases in the controllgroup, compared to 24,5% of the cases in the Metronidazolegroup (p less than 0,05). The mean duration of hospitalisation was reduced from 12,1 +/- 3,2 days in the controll-group to 11,2 +/- 2,1 in the Metronidazole-group (p less than 0,01). Anaerobic bacteria were isolated from the servical swabs in 60% preoperatively, with a still increasing incidence to 72% postoperatively, compared to 7% in the Metronidazole-group. Our results suggest, that prophylactic, intravenously administered Metronidazol reduces the infectious morbidity following cesarian section due to the reduction of the anaerobic flora at the female genital-tract.

[Roentgenologic determination of maturity in low birthweight infants].

121 infants with low birthweight and a well known gestational age were divided into four groups: preterm infants, small for gestational age (SGA) infants, appropriate for gestational age (AGA) infants and infants with Clifford syndrome. With the 100 mm spot film technique X-rays of the right knee and ancle of the infants were taken and the size of the epiphyseal centers of the distal femur, the proximal tibia, the talus and the calcaneus was measured. The mean organ dose of the leg was only 2 mrad. All epiphyseal centers examined showed an increase from the 33rd to the 42nd week of gestation, which is in good agreement to previous studies. There was no evidence of a statistical difference of epiphyseal size between the four examined groups of neonates of low birthweight. Thus, in the present material with very close mean birthweights of the four groups, we were unable to distinguish preterm born infants from SGA infants with this method alone. This result suggests, that bone age is a more resistant criteria to fetal malnutrition than mass growth.

[Maturity of lungs and bones at delivery. Comparison between surface tension measurement of the amniotic fluid and determination of size of infant knee-joint nuclei (author's transl)].

In 47 deliveries of term infants lung maturity studies (surface tension measurements in the Wilhelmy balance) were performed in the amniotic fluid collected at amniotomy. During the first three days post partum the bone age was assessed by radiological measurements of the epiphyseal centers. There was no correlation between the degree of lung- and scelettal maturity. These unexpected findings can be explained by 1) the biological variability in the degree of maturation of both organ systems 2) the false negative rate of both methods 3) the interesting fact, that those cases with spontaneous onset of contractions presented with a higher degree of lung maturity than those in whom labor was induced by amniotomy despite the fact that their pregnancies were of shorter duration. From the results of this study a certain link between the complete maturation of the lung and the spontaneous onset of uterine contractions might be assumed. With the concommitant use of both methods the false negative rate dropped to 1.2%. Consequently a radiological estimation of fetal bone age should be performed in case lung maturity studies are unequivocal.

[Heme synthesis and iron status in pregnancy].

In 130 pregnant women (41 without complications, 40 with prematurity, 49 with EPH-Gestosis) Serumferritin, ferrochelatase (FCH), delta-aminolaevulinic-aciddehydratase (D-ALA-D), porphobilinogendeaminase (PBG-D) as well as hematologic routine parameters were measured. Regarding serumferritin, both uncomplicated and cases with pathologic conditions revealed a significant decrease in group II (28th week of pregnancy) as compared to group I (12th to 28th week of pregnancy). Women with EPH-Gestosis showed the lowest values. Activities of hemesynthesizing enzymes did not show any significant differences, neither between the two groups nor between uncomplicated and pathologic cases. Compared to healthy, non pregnant women; in pregnant women we found a significant increase in D-ALA-D and PBG-D, but a significant decrease in FCH. Enzyme pattern in pregnancy reveals an increased synthesis of porphobilinogen and an increased conversion of porphobilinogen to porphyrin. The low activity of FCH we measured in our study could be a reason for the elevation of free protoporphyrin in the erythrocytes.

[Conisation during pregnancy (author's transl)].

The paper reviews 15 cases undergoing conisation during pregnancy. In 2 conisation was immediately followed by radical operation for invasive squamous carcinoma. 11 patients had pre-invasive carcinoma by microscopic evidence on serial step sections, while the remaining 2 showed high-grade dysplasia of the cervix. Atypical tissue was successfully removed in toto in 6 of the 8 cases with lesions predominantly localizing at the cervical surface. Of the 5 abnormal epithelial processes which were almost entirely endocervical only 1 was completely removed. Abortion occurred in 4 of the 13 patients undergoing conisation without prophylactic cerclage. Shirodkar's operation was done at the time of conisation in 7 cases, in one after an interval of 10 weeks. This prophylactic measure failed to prevent prematurity in 2 cases, while the remaining 6 gave birth to living and mature infants between the 37th and the 40th week of gestation.

[Pregnancy after conisation for atypical epithelial processes of the uterine cervix (author's transl)].

At the First Department of Obstetrics and Gynecology, University of Vienna Medical School, 148 females aged under 35 years at the time of conisation were examined between 1963 and 1974 to determine their desire for children, their fertility status and the course of pregnancy. After conisation a total of only 42 females (28,4%) wanted children. Of 104 patients with preinvasive carcinoma, a desire for children was reported by no more than 21 (20,2%); among the cases with dysplasia, the number of women desiring children was equally 21 (47,7%). By the end of 1976, after a followup period of 1 to 13 years, 16 of 42 females wanting children failed to conceive, while the remaining 26 gave birth to 20 children. Judging from our admittedly small material, conisation thus apparently has no effect on conception. However, the course of pregnancy was found to be impaired in some cases by the incompetent cervical function. Twelve patients required a Shirodkar operation, which failed to prevent late abortion in 2 cases and premature delivery in 4. Nine women gave birth at or at best 3 weeks before term without preventive methods.

[Effect of aminopropyl-aminoethylthiphosphate on radio-iron utilization following whole-body irradiation of mice in the sublethal dose range].

The effectiveness of the thiophosphate compound WR 2721 was investigated with regard to the radiosensitivity of X-irradiated female mice in the sublethal dose range of 50 to 150 R using the radioiron test (59Fe). An increase of the radioresistance with regard to the radioiron uptake in young erythrocyte populations was obtained only beyond radiation doses of 75 R. In lower dose ranges the animals treated with thiophosphate became even more radiosensitive. At dose values of 100 R and 150 R dose reduction factors (DRF) of 1.3 and 1.5 respectively were obtained. These factors are considerably smaller than the DRF-values found for the survival rate at LD 50/30. A possible mechanism for this result may be due to the different dephosphorylation rate of the thiophosphate in various tissues, as described in literature.
