[Experience in the use of finoptin (verapamil) in cerebrovascular diseases].

Twenty-seven patients with dyscirculatory encephalopathies grades I and II and 20 acute ischemic stroke patients received finoptin (verapamil) at daily 5 to 10 mg i. v. or 120 to 240 mg oral doses. The course lasted for 2 months. Rheoencephalography (RhEG) revealed an increase in cerebral blood filling 3 to 5 min after the single finoptin administration. The effect lasted for an average of 40 to 60 min. Central hemodynamic changes were represented by lowering blood pressure and total peripheral vascular resistance, increasing stroke volume and cardiac output. The effect was observable 40 min after a single i. v. injection of the drug. Beginning from the 2nd to 3rd day of the treatment, the patients' subjective conditions improved with reduced strength of general cerebral signs of stroke, RhEG and ECG changes indicating the permanent increase in cerebral circulation and reduced left ventricular overload.

Cerebrovascular Disorders↗

[Status of the ventricular system and dynamics of the cerebrospinal fluid changes in chronic brain diseases].

Using noninvasive (echoventriculometry (Echo-VM), REG and invasive (planimetric PEG, graphic recording of the CSF pressure) methods of examination, the authors determined the size of cerebral ventricles and the status of the cerebral hemo- and CSF dynamics in 606 patients with various chronic diseases of the brain (consequences of craniocerebral injury, epilepsy, discirculatory encephalopathy, etc.). According to PEG and Echo-VM findings, two groups of patients were distinguished. In moderate dilatation of cerebral ventricles the most significant finding was an increase in the pulse pressure of the CSF, whereas its mean pressure was normal or slightly elevated. In patients with pronounced hydrocephaly the pulse and mean pressure of the CSF tended to decrease. The progress of hydrocephaly was parallelled by increasing disorders of the cerebral hemodynamics expressed in hindered venous outflow from the cranial cavity and elevated peripheral vascular resistance. Four CSF-related syndromes have been identified (normotension, total CSF hypertension, intraventricular tension, total CSF hypotension) differing in their diagnostic and prognostic significance and in the pathogenesis of disorders of the hemo- and CSF dynamics.

[Effectiveness of using cavinton in patients of different ages with chronic forms of cerebrovascular diseases (clinico-rheographic research)].

A total of 171 patients of young, middle, and old age presenting initial manifestations of cerebral circulatory insufficiency and early disordered-circulatory encephalopathy were treated with cavinton. The clinical effect of the drug mostly expressed in a considerable decrease of subjective manifestations of the disease was fairly high in most patients of different age groups. The most pronounced positive shifts (according to REG) of the cerebral hemodynamics upon cavinton therapy were observed in young and middle-aged patients. A lower efficacy in elderly patients was explained by the fact that there were grave organic changes in the cerebral vessels, predominantly in those of small and medium caliber. A comprehensive nature of the drug action, as well as the absence of any adverse effect on the cerebral hemodynamics recommend cavinton as a drug of choice in the treatment of patients with chronic forms of cerebral ischemia, including elderly ones. The drug should not be administered to patients over 60 years of age with marked disorders of cardiac activity.

[Status of cerebral hemodynamics during measured physical loading in healthy subjects and patients with cerebral atherosclerosis].

The authors analyzed the results of the rheoencephalographic examination of the cerebral hemodynamics under graded exercise (GE) in 42 normal subjects and 60 patients with cerebral atherosclerosis. All individuals were males aged 40-59 years. It was shown that an increase in the blood content of the brain in the process of GE in the patients with cerebral atherosclerosis was significantly lower than in the normal subjects. With a greater volume of the work done the difference became more pronounced. According to the nature of the response to GE the cerebral hemodynamics in the patients was divided into compensated, subcompensated and decompensated types.

[Changes in the ventricular and vascular systems of the brain in latent syphilis].

Thirty-four patients with early latent syphilis were studied using clinico-laboratory methods including pneumoencephalography, echoventriculometry, rheoencephalography, radiocirculography and angiography. Examination revealed distinct changes in the ventricular and vascular systems of the brain in the form of marked hydrocephalus, atrophy of the cerebral matter, inhibition of the cerebral blood flow, tortuosity of the intracerebral blood flow, tortuosity of the intracerebral vessels and their irregular calibre. The results of invasive (pneumoencephalo- and angiography) and noninvasive ultrasonic and radioisotopic methods were in good correlation. These data indicate an early and fairly distinctive involvement of the nervous system in syphilis and the necessity of a wider use of non-invasive methods of examination for diagnosing latent and subclinical forms of neurosyphilis in conditions of dermatovenerological and neurological clinics.

[Diagnosis and sanatorium treatment of the initial manifestations of inadequate blood supply to the brain].

The authors studied the parameters of the central and cerebral hemodynamics in 66 males aged 38-58 years who were treated at a health resort for initial manifestations of cerebral circulation insufficiency (CCI). A test with graded exercise (GE) revealed in all patients an increase in the total peripheral vascular resistance (TPVR) which was the greatest in cases of initial manifestations of CCI due to borderline arterial hypertension (AH). In the period of recovery and following the nitroglycerin test the TPVR returned to the initial level in patients with AH and vegetovascular dystonia whereas there was no such decrease in patients with initial cerebral atherosclerosis. Already at the 1st minute of the recovery period following GE patients with atherosclerosis (A) and essential hypertension (EH) presented a considerable increase in the arterial tone (AT) which remained stable even after 10 min of rest. These changes in AT indicated a decrease in the adaptational possibilities of the cerebral hemodynamics and simultaneously made it possible to differentiate them from vegetovascular dystonia when AT increased insignificantly only during the first min after GE and then rapidly returned to the initial level and diminished even more during the nitroglycerin test. The proposed method of comprehensive examination of parameters of the central and cerebral hemodynamics when combined with neuropsychological findings makes it possible not only to improve the diagnosis of diseases responsible for initial manifestations of CCI but also to considerably increase the objectivity of the assessment of the sanatorium treatment.

[Neuropathologic syndromes in the late period of closed cranio-cerebral injuries].

Using a comprehensive psychoneurological examination including a number of paraclinical methods (EEG, REG, echoventriculometry, thromboelastography, audiography, etc.) the authors studied 160 war veterans who had sustained a closed craniocerebral injury of a mild or moderate degree during the Great Patriotic War. The following neuropathological syndromes most characteristic for its long-term period were identified: cochleo-vestibular disorders, oculomotor and optico-vestibular disorders, as well as vegetovascular, hypothalamic, Parkinsonism, dyssomnic, epileptic and hydrocephalohypertensive syndromes. The authors also describe a number of the features of these syndromes related to the type of the course of the long-term period of the injury and to the predominance in the clinical picture of the disease of posttraumatic or cerebrovascular disturbances.

[Therapeutic effect of cavinton in chronic cerebral circulatory insufficiency].

Eighty-one patients with chronic forms of cerebral ischemia were treated with cavinton administered by two combined routes (parenteral and oral). The treatment brought about a rapid and significant regression of the subjective manifestations of the disease, particularly of emotionally-volitional and asthenic disturbances. REG findings show that the drug improved the cerebral blood supply through increasing the volume of the fraction of the cardiac output which reached the brain and a simultaneous decrease in the peripheral resistance of the cerebral vessels. The beneficial properties of the drug include a prolonged therapeutic effect (from 40 min to 6 hours with intravenous infusion), the absence of the intracerebral steal syndrome, a mild effect on the central hemodynamics allowing its administration in increasing doses even in elderly and old patients.

[Variants of the clinical course of the late period of military closed craniocerebral injuries].

As a result of a comprehensive psychoneurological examination of 160 war veterans who had sustained a cranicocerebral trauma of a mild or moderate degree during the Great Patriotic War, the authors identified 3 variants of the clinical course of its long-term period: (1) late (at the age of 45-59 years), protracted (of many years) decompensation of a complex (polymorphic) posttraumatic neuropsychic defect; (2) a comparatively early (by age) and accelerated development of cerebral atherosclerosis and arterial hypertension with unstable compensation of posttraumatic cerebral, predominantly vegetative-angiodystonic disorders; (3) the prevalence in the clinical picture of the disease of marked manifestations of chronic cerebrovascular failure or stable sequels of brain stroke in the presence of individual reduced psychopathological posttraumatic symptoms. The article also presents electrophysiological, biochemical and psychological changes characteristic of each variant of the course, which should be taken into consideration in making the diagnosis and predicting the clinical and employment-related outcome.

[Features of lipid metabolism in patients of different sexes and ages with dyscirculatory encephalopathy].

Lipid metabolism was studied in 312 males and females of young, middle, elderly and senile ages with dyscirculatory Stage I and II encephalopathy induced by atherosclerosis, arterial hypertension, or their combination. The control group was composed of 216 clinically healthy age-matched subjects. Marked hyperlipidemic shifts in the form of hypertriglyceridemia (HTG), hypercholesterolemia (HC) and hyperbetalipoproteinemia (HBL) were found in patients of both sexes under 45 years of age: HC and marked HTG were significantly more frequent in males. In middle-aged patients marked HTG was more common in males and HC in females. Elderly and senile patients as compared with young and middle-aged ones were characterized by a low cholesterol coefficient of atherogenicity and high levels of high density lipoproteins with antiatherogenic action. The most frequent type of hyperlipidemias in patients of all age groups was IIa, Types IIb and IV were observed less commonly.

[Value of paraclinical methods of examination in the differential evaluation of posttraumatic and vascular cerebral pathology in the late period of wartime closed craniocerebral injuries].

Using a number of electrophysiological (EEG, REG, echoventriculometry and audiometry) and biochemical (thromboelastography and determination of blood lipids) methods the authors examined comprehensively 95 men aged 56-70 years who had sustained a closed craniocerebral injury including blast contusion during the Great Patriotic War. The most informative signs were identified which may be used as additional criteria of the severity of traumatic and vascular changes of the brain in a long-term period of a war-related closed craniocerebral injury both for the purpose of therapy and expert medical evaluation of the working capacity.

[Physical work capacity among the early manifestations of cerebral atherosclerosis].

A bicycle ergometric study of 54 males, aged 40-59 years, suffering from atherosclerotic dyscirculatory encephalopathy of Stage I-II and of 57 age-matched healthy males showed a considerable decrease in exercise tolerance of the patients. The fall in their capacity for physical work was attended by an inadequate hemodynamic response toward exercise in the form of a faster increase of the heart rate and a greater elevation of the arterial pressure. It is pointed out that level of exercise and the nature in which it is ensured hemodynamically may serve as additional criteria in assessing the functional potentialities of the cardiovascular system in early manifestations of cerebral atherosclerosis.
