[Group talk psychotherapy with adolescents (author's transl)].

The authors give a report about their experiences of five years on group talk psychotherapy with adolescents. The indication to take part in this method which is integrated in a multidimensional therapeutic concept, consists in psychosocial disturbances with crises. Mainly adolescents of 14-18 years of age are taken care of in the groups. Meetings take place once a week and last about 90 minutes each. We have to deal with open groups, e.g. the duration of participation is different depending on the patient's problems. New member to the group are admitted continuously, in most cases after clinical preparation. A rapid integration of new patients is possible through members who have already been in the therapeutic process for a certain time. In this paper the importance of the behaviour of the therapeutist is discussed too. Here more activity is necessary than in groups of adults.

Localization of eosinophils in the thymus by the peroxidase reaction.

Thymuses of human fetuses and infants and of young mice were investigated histochemically for peroxidase. Eosinophils were shown to be the only peroxidase-positive cells in the thymus. In human thymuses the eosinophilic cells were predominantly localized in medullar areas, with concentration of cell clusters at the cortico-medullar junction, around or inside Hassall's bodies and occasionally in high numbers in the intraseptal vessels of the cortex. In the normal mouse the eosinophils were evenly distributed throughout the medulla. Treatment with corticosteroids or X-rays produced a severe involution of the thymus with concommitant change in cellular pattern. The central areas of the thymus residue contained lymphocytes while the peripheral regions consisted of reticuloepithelia, macrophages and numerous eosinophils. Azathioprine did not change the morphology of the thymus. The numbers of eosinophils were slightly reduced, the distribution pattern remaining unchanged.

[Abdominal plain films in mesenterial infarct].

86 plain films of the abdomen from 63 patients with acute mesenteric infarction were compared with plain films of 90 patients with acute abdomen of different origin. The airless abdomen, the increased thickness of the gut wall and the hairpin sign proved tbe nonspecific, but more pronounced and more frequent in mesenteric infarction. Gas in the gut wall and in the portal vein system was found only in mesenteric infarction. All 6 patients with gas in the portal veins died. The airless abdomen in a patient with severe abdominal pain is a surprising finding and points to the possibility of mesenteric infarction.

[Arterial fontanel pulsation].

Infants with bounding arterial pulses frequently show a visible or palpable arterial pulsation of the fontanel. This simple clinical sign is neither described in the textbooks nor mentioned in connection with congenital heart diseases (CHD). To investigate its significance 501 infants were examined 178(35,5%) showed an arterial pulse of the fontanel. Out of 421 with no sign of cardiovascular malformation or disease on physical examination, 111(26,4%) had a fontanal pulse (f.p.). Out of 80 infants with CHD, 67(83,8%) exhibited a fontanel pulse (f.p.), while only 13(16,2%) did not. All infants with CHD had an ECG and biplane chest x-rays taken. In some of them also cardiac catheterization and heart or vessel surgery were performed. 23(79,3%) of 29 infants with PDA and eight of twelve with a VSD had a f.p.. Since other cardiac diagnoses were encountered less frequently, their relation to the presence or absence of f.p. could not be evaluated. From the variety of diagnoses, however, it became obvious that not only lesions with "early aortic runoff" may be accompanied by a f.p. Other clinical features like blood pressure, hemoglobin content and body temperature were discussed as possible causes of f.p.. Only the first mentioned, a high systemic pressure, was frequently accompanied by f.p.. Since the percentage of infants with f.p. and CHD was relatively high (37,6%) this clinical sign is considered to be of some value in the early detection of heart disease in infancy.

[The development of coagulopathy in missed abortion (author's transl)].

A case of fetal death is demonstrated, where a primary hyperfibrinolysis induced a bleeding tendency after curettage. Writings on the subject give differing opinions on the pathogenesis of coagulopathy in the case of fetal demise (= dead fetus syndrome). It is shown that the coagulopathy is caused most frequently by a combination of disseminated intravascular coagulation (DIC) and subsequent hyperfibrinolysis. In sporadic cases, however, one of both pathogenetic mechanisms can occur isolated.

[Smoking in the childhood--results of the school study Berlin-Karow 1976 (author's transl)].

The smoking habits and the lung function of 369 boys and 340 girls aged 6-16 were examined in a school. The boys are beginning to smoke at the age of 9, the girls at the age of 13. 62 per cent of the children aged 16 are smoking (20 per cent of the girls and 30 per cent of the boys are smoking regularely more than 1 cigarette/day). 58 per cent of the children have a smoker in their family. Only 10 per cent of the parents have knowledge about the smoking of their child aged 12, and 59 per cent of the parents of their child aged 16. The lung function tests (ventilation, distribution, bodyplethysmography, single-breath-test, blood gases) of the healthy teen-agers dont show differences between smokers and non-smokers.

[Surgical technique in the management of undescenced testis (author's transl)].

In cases of retentio abdominalis or inguinalis and of ectopic testis, surgery is performed about the 2nd year of life. Essentials of the operating technique are: incision of the skin must not be parallel to the spermatic cord; testes and spermatic cord must be mobilized without trauma; ideally a biopsy of the testis should be taken; any pulling of the spermatic cord and torsion of it during fixation of the testis should be avoided. If initial tensionfree placement of the testis in the scrotum is impossible, the operation should be carried out in 2 sessions with an interval of 6-12 months. In 241 cases of children operated on from 1972-1975 with 308 total operations, the author observed 2 recurrences and 1 deep infection (scrotal abscess).
